Eligibility & Enrollment Who can elect benefits and when and how to elect them Eligibility Enrolling for Coverage All regular employees scheduled to work at least 30 You will make your benefit enrollment elections through hours per week are benefit-eligible as of hire date. Health Reimbursement Account For your medical deductible The Health Reimbursement Account (HRA) is 100% funded by Sage and provides direct reimbursement to your providers for your calendar year deductible. About the HRA What You Should Know Beginning January 1, 2021, HealthEquity will now be the administrator of the HRA. It’s important to know that the HRA is a company-funded account. You are also responsible for your deductible and coinsurance (if applicable), and charges beyond your calendar-year or lifetime benefit maximum. Accumulated Maximum Rollover With the Dental Blue plan, you can accumulate benefit dollars to help offset out-of-pocket costs for complex procedures. Disability Benefits Providing income replacement when you can’t work Reliance Standard Short-Term and Long-Term Disability Disability insurance helps protect you and your family if you become disabled due to illness or injury and are unable to work. Short-Term Disability (STD) and basic Long-Term Disability (LTD) insurance are provided by Sage at no cost to you. Eligible employees are enrolled automatically. Short-Term Disability STD provides 100% of your base salary for up to a maximum of 13 weeks for an approved disability. If eligible and approved, benefits begin on the 1st day for an injury and on the 8th day for an illness. Note: There is no elimination period for disabilities caused by an accident. Long-Term Disability LTD replaces 60% of your base salary, up to $12,500 per month. The elimination period is the later of 90 days or the end of your STD payments. Note: Reliance Standard manages the claim review and processing for all STD and LTD claims. A Note About LTD Premiums While Sage pays the premium for LTD, you have two options that can impact your benefits. — Option 1: Sage pays the LTD premium, which means your LTD benefit is taxable in the event you file an LTD claim. — Option 2: You pay the taxes (reflected on your paystub) on the Sage-paid LTD premium, which means your LTD benefit is tax-free in the event you file an LTD claim. 10
Retirement Savings Build your nest egg for the future 401(k) Savings Plan through Principal Principal Plan Features Description Eligibility You are eligible to participate in the plan as of your date of hire. Your Contributions Contribute up to the IRS annual limit ($19,500 for 2021) on a pre-tax and/or Roth 401(k) (after-tax) basis. If you are age 50 or older, you may make additional “catch-up” contributions ($6,500 for 2021) per year. Limits subject to change by IRS. Company Matching Sage matches 50% up to the first 6% you contribute. Contributions Note: You will receive the company match each pay period for the year. Sage has a true- up feature on the match, which ensures you receive the maximum company match if you contribute at least 6% of your eligible compensation for the year—regardless of the timing of your deposits. With the true-up, Sage will look back at your Year-to-Date (YTD) contributions. If your contributions qualify for any match that you didn’t receive on a per-pay-period basis, Sage will add money to your Plan account to match your YTD contributions. Vesting You are always 100% vested in your contributions. You will vested in company contributions 25% per year; 100% after 4 years. Investments You can invest your contributions and account balance among a variety of investment fund choices offered through the Principal. Employee Stock Purchase Plan (ESPP) Take stock in the company The ESPP plan allows eligible employees the opportunity to purchase shares of the Company’s common stock at a discounted price. Principal Plan Features Description Eligibility You are eligible to participate in the plan after 30 days of employment (prior to January 1 and July 1) and must work 20 or more hours per week.
