Tour Itinerary DAY We begin our adventure by departing Salisbury and heading north- west towards Mitchell. We will stop in Dalby on the way for lunch before 1 arriving in Mitchell. Here, we have dinner under the stars and swim in the warm Artesian Waters that Mitchell is famous for. DAY Wfe continue travelling north-west towards the town of Winton. We will stop in Blackall for lunch before carrying on getting to Winton in the 2 afternoon. DAY Today, we will hop into the buses for a day trip to Longreach. In the morning we will get a tour of the QANTAS Aviation Museum. After lunch 3 in Longreach, we will have a look around town and get a tour of the Stockman’s Hall of Fame, before heading back to Winton. DAY After breakfast, we will visit the Waltzing Matilda Centre, where we will learn about Banjo Paterson. After lunch and a tour of Winton, we will 4 go to the House of Dinosaurs, as this area is famous for finding dinosaur fossils. DAY Today we pack up and say goodbye to Winton as we make our way to Mount Isa. For lunch, we will stop in Cloncurry before reaching Mount 5 Isa mid-afternoon. After settling in, we will take a tour around Mount Isa, including the Mount Isa lookout before having dinner. DAY First, we head to the Mount Isa replica Mine tour and then get an underground hospital tour. After lunch in town, we visit to the leisure 6 centre where we will have a game of Futsal against a local team. After the game, we will go back to the accommodation for the evening. DAY Our next stop is Julia Creek where we will stop for lunch before continuing on 7 to Richmond where we will stay for the night. DAY After breakfast, we will visit Cambridge downs Heritage Centre where we 8 see Cobb and Co Coaches. Next, we get lunch in town before visiting Kronosaurus Korner in the afternoon. Futsal 2
Itinerary continued... DAY Heading east toward Townsville, we stop along the way in the gold mining town of Charters Towers for lunch and a tour. Then in the 9 afternoon, we keep driving to Townsville where we settle in for the night. DAY Today we will go to the Castle Hill lookout, before going to REEFHQ to learn all about the Great Barrier Reef. Then we will have some lunch and 10 take a walk along the strand. In the afternoon, we will play a game of Futsal against Townsville Futsal Club. DAY Next we will head south towards Mackay. We will stop for lunch in Bow- en; the home of mangoes and tomatoes. We will make a second stop 11 in Serena before getting to Mackay where we play a game of Futsal against the Mackay Futsal Club. DAY Today we are going to a Cattle Station. At the Station, we will get a tour and watch cattle being rounded up followed by a whip cracking 12 display. We will eat damper and drink billy tea before going into Mackay for a tour and lunch. DAY Next we head south to Rockhampton where we first visit the Dreamtime Centre and learn about Indigenous Australian Culture. After lunch, we 13 drive to Maryborough where we will be staying for the night. DAY First we go to the Fraser Coast Wildlife Sanctuary where we will get to see lots of Australian animals. We will have lunch in Maryborough, 14 visit the Whistlestop Museum, and if we are lucky ride the train! In the evening, we will play futsal against Maryborough Futsal Team. DAY Today we are heading home, arriving back in Brisbane at approximately 3:00pm 15 Mitchell Winton Charters Towers 3

Risk Management TAG 5 has an On Call Officer available for the duration of the tour. Any situations where medical assistance is required will be reported to the Medical Officer. The following Risk assessment and control measures have been considered and put in place to ensure the safety of all participants. Identified Risk Risk Control Measures Swimming • Participants will only swim in designated areas monitored by TAG 5 staff. • Participants will be accompanied by a staff member in the wa- ter at all times. Consuming • Participants over 18 who wish to drink must bring photo ID and Alcohol will be issued by TAG 5 with a colour coded wristband. • All consumption of alcohol will be two drink maximum per day and under the supervision of TAG 5 staff. • All alcohol purchaced will be at your own cost. Physical Injury • All TAG 5 staff are CPR and First Aid Certified. • Participants will be warned of potential hazards. • TAG 5 will assist participants will minor injuries (i.e. cuts, grazes). • In the case of serious injury, emergency services will be called. • TAG 5 staff will adhere to the NDIS incident reporting process. • Parents/guardians/carers will be informed of any injuries sustained. Illness • Hygiene prompting (i.e. hand washing and hand sanitizing) will (including be increased. Coronavirus) • Staff will monitor for signs of illness in participants (particularly coronavirus symptoms). • If symptoms of illness are present Tag 5 will seek professional medical advice and act accordingly. • Parents/guardians/carers will be informed of any illness. 7
Our Aim We want to provide you with a 15-day fun filled adventure that takes you to some of Queensland’s most iconic outback towns and play a few games of futsal along the way. TAG 5 will facilitate activities that are not only enjoyable but aim to: • Encourage personal growth and boost self confidence • Help you build social and interpersonal skills and form life-long friendships • Provide opportunities to explore new surroundings and engage with the community On Tour Supervision TAG 5 prides itself on being a company of skilled and experienced professionals who provide high quality care to people with a disability and their families. Our staff members have extensive experience working with people with a disability and possess the following qualifications: • Positive Blue Card Notice • Yellow Card with Police Check • Senior First Aid & CPR • Positive Behaviour Support Training • Mental Illness Awareness • Suicide Intervention and Prevention For the entire duration of the tour our trained staff and program facilitators will be available 24/7 to ensure you are safe, assist with personal care and make sure you have an unforgettable experience. What’s Included Support • As above, 24/7 staff support and supervision will be provided • Support is provided at a ratio of one Support Staff member to three participants. We are able to provide a higher level of support if you require, however, your cost will need to be revised based on an individual cost assessment which we will provide. • In addition to our Support Staff, two Tour coordinators will be in attendance. Accommodation • Hotel accommodation will be provided Transport • On the tour, we will all travel together by bus. • Please note you will be required to arrange your own travel arrangement to and from the departure point. Food • All meals will be fully catered for including snacks. • Healthy eating options will always be made available. • We are able to cater for your individual dietary requirements and needs. 8
When special CARE is needed TAG 5 have trained CARE teams to assist participants and their families in the event of a traumatic incident or experience. A streamlined process is also in place to deal with customer complaints including guidelines for response times and handling. Cancellation Policy • Cancellation must be given in writing.The deposit is non-refundable. • In the event of cancelation before 10 working days of the departure date: all payments except the deposit will be refunded. • In the event of cancelation within 10 working days of departure date: all deposits and payment will be forfeited. Terms and Conditions Acknowledgement Please ensure that you read, understand and acknowledge all terms and conditions listed below. 1. The deposit must be paid to secure your place on the tour. 2. the deposit is non-refundable; there is no cooling off period. 3. Travel Insurance is not included. It is your responsibility to organise your own travel insurance. 4. Personal spending money is NOT included in the cost. 5. I understand that the activities that participants will undertake while on tour may involve a degree of risk which could result in personal injury. 6. TAG 5 staff will supervise the use of personal spending money if requested. 7. All medication must be contained in a Webster Pack. 8. Clients are liable for any damage they cause to property. 9. Clients will be liable for any violent behaviour towards another person; regardless of if they are staff, another client or a member of the public 10. TAG 5 will take images and videos on tour and reserves the right to use these in promotional material and on social media, if the participant has provided written media consent to the TAG 5 office. 11. TAG 5 is not liable for the loss or damage of personal belongings. We recommend leaving personal items of value at home. 12.
