San Antonio Geriatric & Palliative Education (SAGE) Symposium: 2018 Optimizing the Care of the Chronically Ill September 26-28, 2018 La Quinta Inn & Suites Medical Center Course Description The San Antonio Geriatric and Palliative Education (SAGE) Symposium is an innovative 3 day inter- professional continuing education conference for primary care physicians, nurses, and social workers focused on improving healthcare for older adults and the chronically ill. How SAGE is Different: Skills Sessions & Health Fair Most continuing education conferences require attendees to passively sit in their seats 8 hours a day while experts click through PowerPoint slides. We at SAGE, feel that busy healthcare providers should expect more from their continuing education conferences. During a conference, providers should be able to actively engage with faculty. After a conference providers should feel energized and able to apply new knowledge and skills when they return to work. Skills Sessions: The first two days of the conference include breakout skills sessions where at- tendees will learn geriatric and palliative care skills they can immediately apply to their patients. Health Fair: The last day of the conference includes a health fair for community seniors where con- ference attendees will practice the skills learned in the breakout skills sessions—cementing their knowledge before they return home. Learning Outcomes Upon completion of this continuing educational activity, participants will be able to: Identify, assess and adequately treat major geriatric syndromes Master communication techniques used among inter-professional team members, families, and patients in geriatrics and palliative care Perform geriatric and palliative patient care skills and incorporate community member feedback Target Audience This activity is designed to meet the educational goals of healthcare providers working in internal medicine, family medicine, hospital medicine, long term care, hospice care, geriatric medicine, and palliative medicine. The course is geared towards practicing healthcare professionals (physicians, nurses, social workers, etc.) who care for complex older adults and chronically ill patients and want to update their clinical knowledge and skills in geriatric and palliative medicine. MAINTENANCE OF CERTIFICATION Successful completion of this CME activity, which includes participation in the evaluation component, enables the participant to earn MOC points in the American Board of Internal Medicine’s (ABIM) Maintenance of Certification (MOC) program. Participants will earn MOC points equivalent to the amount of CME credits claimed for the activity. It is the CME activity provider’s responsibility to submit participant completion information to ACCME for the purpose of granting ABIM MOC credit. The San Antonio Geriatric & Palliative Education (SAGE) Symposium is NOT approved for VA Travel Reim- bursement or Authorized Absence. If you are a VA employee and not local to San Antonio, you may attend this conference in person if you are on Annual Leave and you will be required to pay for your travel costs. The Office of Continuing Medical Education reserves the right to limit enrollment and cancel any course no less than one week prior to the activity. Should circumstances make this necessary, your registration fee would be refunded in full. If you must cancel, the registration fee will be refunded less a $50 handling charge if notice is received by August 31, 2018.
