UFS has cared for generations of families in Ballarat and the western region since opening its first dispensary in Ballarat in 1881. Today UFS owns and operates nine pharmacies in Ballarat, one in Ballan, Bacchus Marsh and Beaufort, two in Gisborne and two in Melton. UFS also operates three pharmacies in Coburg in Melbourne’s inner-north, on behalf of another mutual organisation, Friendly Pharmacy (Victoria) Ltd., with these pharmacies operating under the UFS banner. UFS owns and operates three medical centres and other complementary businesses including the Peace & Quiet Day Spa, and Cherub Gifts. UFS is a significant local employer of choice with over 500 full-time, part-time and casual staff including pharmacists, dispensary assistants, pharmacy assistants, retail staff, contracted general practitioners, practice nurses, allied health practitioners, beauty therapists and remedial therapists, as well as positions in management, administration, warehouse and delivery. As a mutual organisation, UFS is a public company limited by guarantee and is governed by an elected Board of Directors. Any profits generated are reinvested in the business, and are used to improve services for its members, offer member discounts, invest in its people and support the local communities in which it operates through its sponsorship and donations program. UFS Medical The UFS general practice model will support GP and Practice Nurse led and directed multidisciplinary teams that will be central to a population health approach, community development activities and practice based clinical encounters. The UFS general practice model of team-based care will increase community access and enhance the range of primary health care services available in the general practice setting. Through practice systems and e- health applications the GP and Practice Nurse and allied health team will manage people with chronic conditions, promote safety and quality programs and practices, and support a viable primary health care workforce. The UFS general practice model will take the best of the emerging national experiences regarding primary health care while remaining grounded in the traditional arrangements that place the GPs, practice nurses and practice managers at the core of the general practice environment. In the UFS general practice model the
UFS Dispensaries Ltd GP’s role is to manage and oversee care through detecting, diagnosing and identifying whole-of-patient issues and care planning. Practice nurses and allied health professionals will implement care plans and other appropriate chronic disease management processes under the GP’s direction. The UFS general practice model will provide: A GP led, wellness oriented, primary health care system as a necessary pre-requisite to an effective health system. A comprehensive approach to general practice and primary health care that includes health promotion, illness prevention, treatment and care of the sick, community development, advocacy, rehabilitation and population health based approaches. GP-centered, multidisciplinary teams, as the main access point for population health initiatives, community development activities and clinical encounters. UFS Dispensaries Ltd About the Position │ Purpose The primary purpose of the Practice Nurse role is to provide clinical services to Practice patients in collaboration with GPs and other healthcare professionals. Professional management of these services including required administration and compliance requirements in line with Practice policies and procedures. About the Position │ Outcomes and Responsibilities Outcome 1 │ Business Unit Success Responsibilities: Provide a range of clinical services required by patients in collaboration with GPs. Perform these duties within the required level of clinical competency according to best available evidence. Provide patient services including assisting with triage, data management, networking with other providers, planning and management of patient care and staff advocacy. Assist with patient education and health promotion activities to facilitate patient health outcomes including coordinating and reviewing patient recall and register systems, conducting preventative and screening procedures and providing healthcare services in accordance with the medical benefit schedule. Assist in achieving annual operational goals of the Practice in conjunction with the Clinical Nurse Manager. Provide support to the Clinical Nurse Manager to develop, implement, review and improve practice systems that promote and environment of continuous quality improvement. Act within the Practice and nursing code of ethics, work with an appropriate level of clinical competence at all times, maintain awareness of current evidence and research on clinical practice, and the education of other practice staff. Manage patient flow and daily appointment schedules. Flexibility to work across all 3 medical sites if required. Accept and deal with/refer complaints. Outcome 2 │ Clinical and Professional Responsibilities: Clinical specific duties including, but not limited to, triage, immunisations, collection of pathology samples, wound management, ECG, chronic disease management, aged care assessment, health screening, assisting doctors with clinical procedures and maintaining all associated clinical documentation. Order and manage appropriate stock of clinical supplies. Ensure sterilisation, storage and maintenance of clinical equipment. Participate in team staff meetings and business review processes. Ensure high standards of customer service and professional healthcare practice are continually achieved. Centre housekeeping including cleaning and Practice presentation. Outcome 3 │ Learning and Development Responsibilities: Participate in continuing professional and cultural development supporting exemplary team performance relative to the Practice’s objectives and the values of UFS. Maintain and actively seek opportunities for your own professional development. Attend relevant training sessions and programs as required to ensure mandatory CPD requirements are met as per AHPRA guidelines. UFS Dispensaries Ltd Outcome 4 │ Administration and Reporting Responsibilities: Operate administration structures, policies and procedures with a particular focus on providing professional support for all medical practice staff. Provide supervision to other team members in relation to patient records including the delegation of tasks to medical and non-medical staff. Outcome 5 │ Compliance and Legislation Responsibilities: Maintain current registration with Australian Health Practitioners Registration Agency (AHPRA) including adherence to mandatory requirements of AHPRA. Maintain awareness of current and new legislation to ensure compliance with all statutory and regulatory obligations, including cold chain management, infection control, sterilisation, record management, safe handling and disposable of medical waste, storage of medicines, OHS, privacy and confidentiality and accreditation. Adhere to Practice specific policies and procedures. Conduct work in a manner which is compliant to all relevant industry regulations. Comply with procedures to meet occupational health and safety requirements. Apply relevant security procedures. Report potential breaches of safety or security to appropriate senior management. About the Position │ Other Requirements Other tasks as directed by the Clinical Nurse Manager, Practice Manager or GPs and Health Professionals. Measures Feedback from Clinical Nurse Manager/Practice Nurse Coordinator Feedback from Practice Manager About You │ Key Competencies Required Division 1 registered nurse with minimum of one year post graduate experience. Relevant experience in a primary healthcare team. Professional indemnities cover as per AHPRA guidelines. Competent in the use of relevant equipment and software to satisfactorily perform the duties of the position. A sound understanding of the general practice sector and its relationship to the broader health sector and local community. Demonstrated patient-focused approach to service provision including a commitment to the maintenance of absolute confidentiality regarding client and practice information. Outstanding interpersonal and communication skills including written, verbal and negotiation skills, and ability to work as a member of a high-functioning team. Desirable Additional qualifications such as asthma/spirometry education, diabetes education certificate, accredited women’s health certificate, nurse immuniser. Knowledge of Commonwealth and State health systems, policy and funding arrangements including experience in the Medical Benefits Schedule. 4 | P ag e
