REQUIREMENTS FOR CLINICAL EXPERIENCE
Must be completed and documentation submitted to instructor for items 1-7,
PRIOR TO END OF 1st SEMESTER (1/29/2020). Late submissions will result in
deductions in participation grade.

If student has any barriers to completing these requirements, including financial concerns,
please discuss with course instructor or with School Director Wes Allen or Assistant Director
Kim Brown. Assistance is available.

1. Mandatory Tuberculosis (TB) Screening: required by the Federal government for all
healthcare workers. Two options – 2 step skin test OR blood test.
Where to get a TB Test?
   https://www.doh.wa.gov/YouandYourFamily/IllnessandDisease/Tuberculosis/wheredoig
   etatbtest)

TB Skin test is a 2-step procedure: can be done at primary care provide or a community
health clinic, may be covered by health insurance.
The TB or PPD “two step test” requires 4 visits:
 Step 1: A small injection is placed on the inner aspect of the forearm
 Step 2: It is “read” (looking for a bump) 2 – 3 days later
 Step 3: 1-3 weeks later a small injection is placed on the inner aspect of the
          forearm
 Step 4: It is “read” (looking for a bump) 2 – 3 days later

TB Quantiferon Gold blood test – one time only blood draw, may be covered by insurance.
Available at Any Lab Test Now for $199 (https://www.anylabtestnow.com/everett-
  98208/tests/quantiferon-tb-gold-plus/). No provider order needed.

2. Vaccination record: Students will provide a copy of their vaccination record to include:
MMR, Varicella, Hepatitis B and seasonal influenza.
How to obtain your immunization record:
   https://www.doh.wa.gov/YouandYourFamily/Immunization/ChildProfileHealthPromotion/
   ForParents/AccessyourFamilysImmunizationInformation
If student has a religious or medical exemption, documentation of the exemption will be
   required. https://www.doh.wa.gov/Portals/1/Documents/Pubs/348-
   106_CertificateofExemption.pdf

3. Flu Vaccine: Students are strongly encouraged to get this vaccine; it is covered by
medical insurance. Given the current pandemic, it is highly likely that clinical facilities
will REQUIRE a flu shot, unless the student has a medical or religious exemption. If

                                                8
there is a flu outbreak in the community or at the clinical facility, students that have
not been vaccinated will not be able to complete their required 40 hours of internship
(clinical rotation) and will not be able to take the state CNA exam.

4. Mandatory 5 panel urine drug screening: This is a 5-panel urine drug screen that can be
done at some provider office laboratories, community health clinic or community lab.
This test is for the presence of drugs in the urine. Most clinical facilities have a Drug Free
policy for all clinical workers and employees.

Available at Any Lab Test Now for $49 (https://www.anylabtestnow.com/everett-
98208/tests/urine-5-panel-standard/). No provider order needed if done at ALTN.

5. Criminal Background Check: This short online questionnaire is a Federal
requirement for healthcare workers. The form will be completed in class.

6. CPR: This is a one day, in person 3 ½ - 4-hour Healthcare Provider course that requires
passing a written test and skill demonstration. It must be BLS for Healthcare Providers
through American Heart Association. The cost is usually around $50 - $75 and the card is
   good for 2 years.

Class may be available at Sno-Isle, check with course instructor.

7. Social Security Number: Students must have a valid social security number to be
eligible to sit for the state certification exam. Please discuss with instructor if you have
   any concerns.

8. Licensing Fees.

Washington State Certification Exam: $124 (written and skills test)

License Fee: Initial Application $65, annual renewal thereafter $70

9. Transportation is required on the 5 clinical days due to the clinical hours of 6 am to
2:45 pm. There is limited parking at the Skilled Nursing facility and all students will
meet at the school and be driven in the clinical van. Note 4 of the 5 days will be on
