         If you do not have the energy invoice in hand, you can issue your
         duplicate in the following ways:

         a) Through Copel app, by clicking on the “consultar débitos” (consult
         debts) icon: You can select the invoice on the “PDF” icon and click
         on “copiar toda a linha digitável” (copy the entire digitizable line).

           the appropriate consultation systems. If your registration in the “Cadastro Único” (Single Registry) or in the BPC is recent, it is possible that your

           information is not yet available for consultation. In this case, a new search will be carried out within 30 days, as required by the legislation.

          Only energy consumption values will be paid.

          Values such as fines and others are not covered by the program.

          Important:

          To maintain the benefit, your registration with the “Cadastro Único” must

          be updated at least every 2 years. Indigenous and quilombola families have

          different discounts.

           User Registration
           What is it?
           Is the register of consumer units in which people use electrical

           equipment that are essential to their survival. This registration

           is only made if there is a clear request from the consumer unit

           holder, with medical proof of the user's condition.

           Copel needs to know these cases in order to make preferential

           and early warnings about scheduled power outages and

           suspension of supply, in order to avoid problems with the

           operation of electrical appliances that preserve the user's life.

              See the list of equipment that, added to the user's clinical condition, fit into the vital
              condition (survival):

                         ∙ Respirators or lung ventilators;
                         ∙ Vital parameter monitors;
                         ∙ Secretion aspirators;
                         ∙ Automatic Peritoneal Dialysis Equipment that can be found in the following modalities: Continuous Ambulatory
              Peritoneal Dialysis (CAPD), Nocturnal Intermittent Peritoneal Dialysis (NIPD) and Continuous Cycling Peritoneal Dialysis (CCPD);

                         ∙ Chemotherapy device;
                         ∙ Oxygen Concentrator;
                         ∙ Infusion pump;
                         ∙ Oximeter ;
                         ∙ CPAP and BIPAP;
                         ∙ Special situations based on medical evaluation.

              In the event of a power outage at the residence included in the Vital Equipment User Register, the person in

              charge can contact 0800 643 5445 (exclusive service).

           How to apply?
           To apply registration in the Register of Vital Equipment Users, the following

           documents must be submitted to Copel:

                     1) Copy of the CRMN and CPF of the consumer unit holder and the

           user of the equipment (if they are not the same person);

                     2) Form – “Usuários de Equipamentos Eletromédicos” (Users of

           Electromedical Equipment). It must be completed, stamped and signed by

           the responsible doctor;

                     3) “Termo de Compromisso” (Term of Commitment);

                     4) "Declaração de Comprovação de Vínculo com o Responsável pelo

           Imóvel” (Proof of Commitment with the Responsible for the Property).

           It must be presented in cases in which the user of the equipment and the

           holder of the consumer unit are not the same person.

              After registered, the request will be analyzed within 5 working days. Copel may request
              the opinion of the company's medical area, as well as request the verification of the
              appliances on site, when deemed necessary.

              Important:

              The Vital Equipment User Registration must be renewed every year or whenever requested
              by Copel.

              This registration does not exempt the user from regular payment of the invoice, or from
              collection actions in the event of non-payment, including suspension of supply.

              In the event of a change of address, Copel must be notified to transfer the registration, if
              the equipment is still needed.
