The Plaza Foyer is accessible by stairs or elevator. All posters and storyboards must be set-up by 8am, Thursday, September 26. Internet Wi-Fi is available throughout the meeting space. Ken Rockwood This presentation will consider the differences between screening and assessment in relation to what clinicians need to know when (older) patients are acutely ill; distinguish between risk stratification and care planning in the evaluation of frailty in people who are acutely ill; discuss the role of clinical judgment (which is variable) from standardized testing (which purports not to be) in the evaluation of people who are frail and acutely ill. 9:00am-10:00am Assessing Frailty Here, There, and Everywhere Presenters will discuss the challenges and opportunities for identification of frailty in the home and within healthcare settings. 10:00am-10:35am What’s Happening at CFN? Frailty Outcomes Consensus (FOCUS) Project Jeanette Prorok CFN has recruited individuals from almost 20 countries to take part in a global Delphi survey to identify a minimum set of common data elements and outcome measures for frailty studies that can be collectively analyzed to better inform patient care. Nutrition and Physical Activity Clinical Practice Guidelines for Older Adults Living with Frailty Jayna Holroyd-Leduc Healthcare providers will soon have a valuable tool at their fingertips: evidence-based nutrition and physical activity clinical practice guidelines for older adults living with frailty. CFN has partnered with the McMaster Evidence Review Synthesis Team (MERST) and McMaster Institute for Research on Aging (MIRA) to undertake the rigorous process of developing guidelines. Important outcomes from this CFN collaboration will be physical activity and nutrition recommendations that are associated with achieving various health benefits among older adults living with frailty. Frailty Biomarkers – A Path to Precision Medicine Parminder Raina CFN has partnered with the Canadian Longitudinal Study of Aging (CLSA), the McMaster Institute for Research on Aging (MIRA) and Metabolon, Inc. to discover the frailty biomarkers that may shed light on why some people become frail, determine the severity of frailty and what can be done to help avoid the condition. 10:35am-10:50am Aging Veterans: Past, Present and Future Mary Beth MacLean This presentation will explore the history of programs for aging Veterans and the potential of data sources on Veteran health and functioning to examine the care needs of Veterans. 10:50am-11:10am BREAK 11:10am-12:40pm Connection is a Choice James Rea This interactive session introduces delegates to general principles in how to craft short, clear, conversational statements, intelligible to non-scientists, about what you do and why it matters. The session consists of an interactive presentation and discussion on interpreting technical material using examples and analogies to illuminate unfamiliar concepts to your audience.
