From the ME edition two | twenty twenty The world we live in has changed completely since the last message I wrote for the newsletter. The implications of COVID-19 have taken the world by storm and the impact on healthcare has been profound. We can only applaud all our brave healthcare workers who are working tirelessly in the face of the pandemic. While MediSwitch is not in the frontline of healthcare delivery, we are a very important part of the healthcare service delivery chain, particularly in this time, as a substantial number of pharmacy claims are being switched in South Africa. From the ME | Account Managers | Training Tips | Did You Know | Scheme Updates | Industry Insights | Workshops | Information Update During the lockdown MediSwitch remained open, but it was “business as unusual”, with all our staff working remotely from home. It has been a very interesting time for all of us, getting used to interacting exclusively via Microsoft Teams for chats, teleconferences and videoconferences. I am proud of how our team have adapted, and that we have managed to keep the Switch operating flawlessly. Not only are we supporting our customers, we have also continued to work on new projects. Working remotely has not been without its stresses for our staff, be it coping with home schooling, having network and data issues and, of course, dealing with the general levels of uncertainty we are all feeling. The World Health Organisation has published a series of messages to support the mental and psychosocial wellbeing of healthcare professionals during the COVID-19 outbreak. Three of these posts are relevant to the rest of us as well, and I urge you to take note of them as part of your plan to look after yourself during this time: 1. It is quite normal to be feeling under pressure in the current situation. Stress and the feelings associated with it are by no means a suggestion that you are weak. Managing your mental health and psychosocial wellbeing during this time is as important as managing your physical health. 2. Make sure you get sufficient rest and respite during work or between shifts, eat sufficient and healthy food and engage in physical activity. 3. Training Tips edition two | twenty twenty Three easy steps to create your own one-page quarterly action plan for your medical practice To grow your medical practice in the right way, you must be flexible on the one hand with the ability to adapt your plan as you go. On the other hand, if you change your focus too frequently, you will lose momentum and your staff will lose focus. The answer to execution lies in the quarter Three months is a long enough period to get meaningful units of work done, From the ME | Account Managers | Training Tips | Did You Know | Scheme Updates | Industry Insights | Workshops | Information Update bringing you closer to your long-term goals. It is also a short enough to course correct while holding your focus. Step 1: Pick your top three focus areas for the quarter Potential focus areas for your practice can be: Improve collections Increase your clinical capacity Increase patient volume Recruit another provider Conduct a margin analysis by procedure type or by payer category Hire an operations manager Mitigate a key legal exposure for your practice Negotiate a new contract with the hospital Find a location to build your surgical centre You should only choose three focus areas per quarter to ensure that you don’t spread your practice too thin. Step 2: Determine the success criteria for your focus Once you’ve chosen three focus areas, take a moment to pause and clarify what you see as being a successful outcome for each area. Ask the following questions to make it easier: What do you want to accomplish during this quarter to consider it being successful? How will you measure whether it’s successful? You should have a clear sense of how this will work and what it will look like. Choose three or four definite criteria of success for each of your focus areas. PG|03
Training Tips edition two | twenty twenty Step 3: Lay out the key action steps and milestones for the quarter Finally, determine the key actions and milestones required to meet your success criteria for each of your chosen focus areas during the quarter. To do this, break each focus area into five to seven action steps. Your plan should be detailed enough to guide your actions while not getting lost in infinite detail. Now, for each of the action steps, choose a specific team member that will be responsible for executing that step by a specific date. System software updates: While the ICD-10 Master Industry Table (MIT) descriptions will continue to be as per the column above on the NDoH website, we suggest that all internal software is updated to be in line with the updated descriptions for U07.1 and U07.2 as provided by MRC and WHO. The description of ICD-10 coding is not sent out electronically, and it should therefore not impact on any accounts sent and / or received, but will allow the user to be able to easily identify, code and draw statistical data as needed. Industry Insights edition two | twenty twenty Unbelievable times, but there are positives… Who would have had an idea that, when President Cyril Ramaphosa publicly urged South Africans on 25 February to mobilise behind the NHI to ensure that it is implemented, not four weeks later he would be on national TV formally announcing that each and every individual in the country would have to “lockdown” in their homes for three weeks to safeguard not only their own health, but the health of others…? Such has been the way of the world! At the time of the NHI plea, the local healthcare community was becoming somewhat weary of the ongoing bickering over aspects of the NHI Bill as it moved through its final phases. From the ME | Account Managers | Training Tips | Did You Know | Scheme Updates | Industry Insights | Workshops | Information Update Public hearings on the Bill were wrapped up in the same week in what appeared to be a cooling off period as most resigned themselves to simply waiting for it to take its course. Finance Minister Tito Mboweni’s concurrent budget speech did little to help the prevailing mood. In a media report at the time it was noted that despite the president’s plea, “it seems as if the country’s precarious economic position will push government into reconsidering its current timeframe of full implementation (of NHI) by 2026”: “Both yesterday’s Budget Speech and the State of the Nation address earlier this month were surprisingly silent on what Ramaphosa earlier described as the most far-reaching act of social transformation this country has experienced since 1994.” And that’s how things were until 5 March when the following statement was released: “The Department of Health has confirmed South Africa’s first positive case of the coronavirus, COVID-19. The patient is a 38-year-old male who travelled to Italy with his wife. They were part of a group of 10 people and they arrived back in South Africa on Sunday.” Dare it be said “and the rest is history”…? While impossible to dwell on specifics with COVID-19 dominating literally all media until who knows when, there have been some notable observations in the context of healthcare accessibility and delivery in South Africa. Probably the most prominent - and obvious at that - must be the opinion of one commentator that “any NHI plans have now pretty much been scuppered for the next decade. The South African government’s lending costs and interest payments will increase by as much as 50% due to the downgrade (Moody’s), leaving a fiscal hole that leaves no hope for a tax funded NHI”. Not good news for the disciples of NHI, but on the other side of the coin: a nation pulling together in the interests of the health, welfare and safety of all its peoples. COVID-19 has unleashed a collaborative effort of “in your wildest dreams” proportions. Health Minister Dr Zweli Mkhize, who has led this effort in PG|16
Industry Insights edition two | twenty twenty exemplary fashion from the outset, has been blatant in stressing the need for the public and private sectors to “become one” – a classic example being his early April meeting with the private hospitals, after which he announced to the media that hospital services, as with laboratory services, would become a single operational entity in facing the virus challenges. Unable to spend too much time elaborating on the prospects of this due to COVID-19 response commitments, Dr Nicholas Crisp, the Ministry of Health’s NHI Fund Officer, was able to tell one media representative: “But it is clear why NHI is essential and I think that this experience will make it easier for us all to work together after the epidemic.” From the ME | Account Managers | Training Tips | Did You Know | Scheme Updates | Industry Insights | Workshops | Information Update Similar sentiments were shared by well-known health sector critique, Dr Johann Serfontein, who in a 1 April media opinion piece wrote: “The COVID-19 outbreak has already done more in two weeks to acquire public sector patient access to private facilities than 10 years of National Health Insurance (NHI) planning and pilot projects have done. This is a clear indicator that political will was the lacking element in making progress with NHI.” Also showing that it was possible for government to negotiate access to beds and facilities in the private sector “without a massive overhaul of the entire healthcare sector” and the willingness of the private sector to step up and treat public patients when called upon, Serfontein added: “These interim agreements during the time of COVID-19 may well become permanent arrangements between government and the private sector to ensure continued access.” Another potential positive COVID-19 spin-off, this time more on the clinical side, came from the minister himself in a pre-disaster/lockdown live stream presentation. Acknowledging that while it was both “unfortunate and bad luck” that COVID-19 had reached pandemic proportions at a time when South Africa was already grappling with concomitant disease issues such as HIV and tuberculosis, there was a silver lining: “There is no other country in the world that can give us much information about the response of various individuals to an infection of this nature (COVID-19) in the presence of, for example, HIV infection and concomitant TB. So there is space and scope for new research,” he said, stressing the “huge opportunity” to discover the conduct of concomitant viral infections and ultimately research into their management and treatment. And to end on a sad note: the death of well-known IPA pioneer, “diving doctor”, practice management specialist and medicines formulary architect, Dr Daan (Danie) Struwig (64), at his home in Richards Bay on 14 February. Well-known among provider group, funding and pharmaceutical industry representatives particularly for his unifying Circle of Friends initiative in the early 2000s, he continued to run his practices in Richards Bay, KwaMbonambi and Empangeni until his passing.
