HEALTH AND HUMAN SERVICES PRIVATE NON-PROFIT GRANTS GRANT APPLICATION INFORMATION AND TECHNICAL ASSISTANCE MEETING Thursday, September 20, 2018 3:30 – 5 p.m. Note: This presentation will be posted online at broomfield.org/hhsoperations and emailed to the PNP contact list.
● Phase 1 for 2019 grants ○ Align grant funding with HHS priorities: ● Whole Family Success: Utilizing a multi generational approach, create a thriving PNP PROCESS CHANGES community that promotes whole family success, employment and economic resiliency, and healthy childhood development. ● Physical and Behavioral Health: Create healthy families and community through increased access to care and support for physical, emotional, and behavioral health. ● Collaboration and Integration: Work together as a department and a community to efficiently use resources by collaborating on shared goals. Leverage technology systems and personal partnerships to support whole family care and success. ○ HHS improved timeliness
● Phase 2 for 2020 grants ○ Collaboration with community partners to identify strategies to collect and assess data ○ PNP PROCESS CHANGES Performance metrics and measurable outcome indicators ○ Potential for multi-year contracts

• Private Non-Profit (PNP) grant application evaluation committee – Comprised of Health and Human Services (HHS) staff division representatives, HHS advisory committee members, other Department staff representatives 2019 INFORMATION – PNP committee makes decision objectively – HHS management and City Manager’s Office has final dollar amount discretion, objective funding formula used • Because of application scoring and funding limits, it is possible that some agencies will not be funded or will not receive the full amount of their funding request • Funds awarded to selected agencies must be expended by December 31, 2019
• Review of 2019 Application and Spending Plan • Changes to 2019 Application: 2019 APPLICATION – Section 2: HHS Priorities - NEW – Section 3: Narrative • (If applicable) If agency provides health services, how has the Regional Accountable Entity (RAE) and/or Affordable Care Act (ACA) impacted the provision of services and agency funding? – Section 5: Miscellaneous Documents • No points allotted - committee will review to ensure agency viability, but these points are redistributed to the other sections
• Tip #1 – Please answer all components of each prompt/question. Failure to answer all components may result in point deductions. • Tip #2 – Applications must specify how funds will be used for Broomfield Residents living within the City and County boundaries (i.e. not TIPS students of Broomfield schools, Broomfield community, etc.). • Tip #3 – Use resources and research to validate your application (e.g. census data, needs assessments, community surveys, etc.). • Tip #4 – For Outcomes and Data, make sure your data sources exist and can be tied back to support your proposed outcome. • Tip #5 – Only provide accurate, relevant, and meaningful data. It’s okay to be succinct. • Tip #6 – Please adhere to the application space limitations.
• Agencies that are awarded funding will need to submit a mid-year and year-end report that includes: 1. Residents served • Unduplicated number of Broomfield residents served; of those served, number of which were funded by City and County funds • e.g., 289 Broomfield residents received food from REPORTING the agency. Of those residents, 176 were funded by the City and County funds. 2. Amount of Broomfield funds expended 1. Nature and/or type of services provided (refer to grant application Results-Oriented Management and Accountability [ROMA] model) and outcomes 1. Other client demographics that reflect the services provided by the funds 1. Any changes to federal tax exempt status
