GREAT WOLF LODGE COMMUNITY TRIP When: July 22nd – 25th, 2015 Registration and Installment Dates: Sunday May 24th Registration and Down Payment As per Gaming Revenue Funding: 10:00 am – 11:30 am Registration for Community Members who have never attended the Community trip to Great Wolf Lodge. 11:30 am – 1:00 pm Open registration. Saturday June 20th 2nd Installment Due 9:30 am – 11:30 am Curve Lake Health Centre. (please see payment schedule) Saturday July 4th Meals Payment- Optional 9:30 am – 11:00 am Curve Lake Health Centre. (please see payment schedule) PLEASE NOTE: • GRF approved funding is one night’s accommodation at $ 150.00 per family and bus transportation. Any other costs are the responsibility of the participant. • FOR JULY 22-25, we are limited to 10 FAMILY SUITES, 5 KID KAMP, 5 WOLF DENS AND 10 MAJESTIC BEAR SUITES* Rooms will be disbursed on first come first serve basis. • Room cost is for 4 people, unless it is a Majestic Bear Suite, in which 6 people can be accommodated. Any persons over this would be an additional cost of $40.00 per person per night. ***If you cannot commit to the payment dates, please do not register. If payment is not made on the dates as set out, your spot will be forfeited, therefore forfeiting your initial deposits. NO EARLY REGISTRATION WILL BE ACCEPTED **NEW** Transportation Options 1. Bus July 22 departing Curve Lake at 10:00 am July 25 departing Great Wolf Lodge at 11:00 am 2. Own Vehicle Families may drive own vehicles at own cost.

THESE MEALS ARE OFFERED AT A DISCOUNTED RATE. Total cost is dependent on room selection. 2) We will require a one night accommodation down payment non-refundable CASH deposit on the day of registration per room on May 23rd. No registration will be deemed complete without deposit. (see payment schedule) 3) The second night accommodation plus the remaining cost for your third night will be required on June 20th. (see payment schedule) 4) Should you wish to purchase your breakfasts and/or dinner vouchers, you will be able to do so on July 4th. (see payment schedule)
5) Any costs exceeding the allotted funding of $150.00 per room will be the responsibility of the registrants. 6) If you are attending the trip as part of the Big Trip Initiatives Group, you will have the option to utilize the transportation offered by the group or take your own vehicle, we will be taking coach buses which will have toileting facilities available and will also be stopping half way for a break. You are responsible for your own vehicle, meaning should any damages occur while at or on route, Curve Lake First Nation does not take responsibility. 7) We expect that because this is a family oriented trip, you will be expected to act in a responsible manner. 8) As per Section 79(3) of the Child Welfare Act: Leaving child unattended No person having charge of a child less than sixteen years of age shall leave the child without making provision for his or her supervision and care that is reasonable in the circumstances. Reverse onus (4) Where a person is charge with contravening subsection (3) and the child is less than ten years of age, the onus of establishing that the person made provision for the child’s supervision and care that was reasonable in the circumstances rests with the person. Allowing child to loiter, etc. (5) no parent of a child less than sixteen years of age shall permit the child to, (a) loiter in a public place between the hours of midnight and 6 a.m.; or (b)be in a place of public entertainment between the hours of midnight and 6 a.m., unless the parent accompanies the child or authorizes a specified individual eighteen years of age or older to accompany the child. As such any child under the age of 16 years of age MUST be under the supervision of an adult at all times. 7) Per the Activities and Events Participation Policy section 5.1 Cancellations 1. If a person signs up to attend/participate in an event which requires advanced purchase tickets, and then does not attend the event, other than in emergency situations the person will be required to pay the cost per ticket of the event (not including the bus cost). Participation in future events/activities will be limited until Curve Lake First Nation has been reimburse for this cost. As such any person who signs up for the trip and cancels will be responsible for 100% of the cost (including Gaming Revenue Funding) and will forfeit all monies previously paid into the cost of the trip.
