What the program offers: Your feedback is important! Expand Your Horizons is a program Let us know how we are doing and that supports individuals’ rights to what you would like to see added to make choices. Munchie Mondays April– June Who doesn’t love a good meal? Session Period: Join us for Munchie Mondays as we April 6– June 22 explore different restaurants in and *Excludes Easter Mon and Victoria Day around our community. This program Day: Mondays (10weeks) is sure to offer a taste for every individual’s palate. Don’t forget to Time: 9:00 am — 3:00 pm bring your appetite! Fun Fridays April-June Out and about in our local Session Period: communities– a little bit of this and April 17-June 26 a little bit of that. Day: Fridays (11 weeks) During this session, you will be going to farm/animal themed activities/ Time: 9:00 am — 3:00 pm events…. Location: Creative Barracks– art 2256 River Road, Cayuga Triple C Farm Cost: $1166 Killman zoo Twin Valley zoo This session will include staffing, fees and transportation right from your home. A light lunch will be included. 8
Medieval Times Toronto’s famous dinner/show with Activity Date: live action in front of your eyes! Taste Thursday, April 2nd the excitement of medieval Spain, with horses and falcons and knights in Time: 5:00pm– 12:00am combat. Swordplay, jousting and Location: Pick up and drop off at pageantry! Plus dinner is included—eat 2256 River Road, Cayuga with your hands! Visit the “dungeon of doom” as part of Cost: $217 the adventure. This is a 7:30pm show. Dinner and a Movie These movies will be on a weekly basis Activity Dates: since they are so popular. IMPORTANT INFORMATION Missed Activities: In the event that a participant is unable to attend an activity, we regret that a make-up activity, refunds, or credits cannot be offered. Payment: Full payment of activity fees must be made at the time of registration. Payment by cash, cheque or money order are acceptable methods of payment for all activities. Health & Safety: We reserve the right to ensure the health and well-being of all participants. Participants will be sent home, if there is infectious illness identified that may pose a risk to either staff or participants. Waiver of Liability: We shall not be liable, either directly or indirectly, for any claims, damages, costs, expenses, including but not limited to personal injury, property damage or lost or stolen property, arising from or connected with participation in any activity contemplated by a registration, misrepresentation, breach of statutory duty, breach of the Occupier’s Liability Act and /or breach of contract on the part of the organization. Inclement Weather: In the event of inclement weather we may need to cancel for your safety and will make every effort to add an additional class at a later date to make up for the missed class. Cancellation: We understand that there are times when cancellation is necessary due to personal reasons, health and inclement weather. Please be aware that there are times where full refunds can not be provided due to pre-purchase of tickets. Please see the following page for further details. 21
CANCELLATION & WITHDRAWAL GUIDELINES FOR ACTIVITIES AND EVENTS Event cancellation by agency: Cancellation of an event or activity may occur due to low enrollment or unforeseen circumstances such as inclement weather. You will be provided with an alternate activity, date or a refund*. Event/activity cancellation by participant: Should you need to withdraw from an activity after registering and submitting payment you must do so at least 14 days before the planned date of the activity or the beginning of the session. If the withdrawal is due to medical reasons a medical note should accompany your request for refund. Request must be made by no later than midpoint of a 10-11 week session or prior to the date of a single event. In both cases you will receive a 50% refund less a $35 administration fee*. Please note: all requests for a refund must be made in writing accompanied by the original receipt. Non– attendance does not constitute notice of withdrawal. *If tickets have been preordered and paid for; we may not be able to refund costs. Cancellation & withdrawal guidelines for staffing of 1:1: When notification of cancellation for staffing is 48 hours or more in advance— No charge for service When notification of cancellation for staffing is less than 48 hours in advance of start time — A 3 hour charge of service will occur 22
