2022 Annual U.S. Benefits Enrollment Bulletin For COBRA Participants OCTOBER 2021 Welcome to the annual enrollment period for the JPMorgan Chase Enrollment ends November 12! 2022 U.S. Benefits Program. You have the opportunity once a year to review your Health Care and Insurance Plan coverage and make any necessary Check your enclosed Personalized Fact changes. If You Are Currently Enrolled in Subsidized COBRA and Are Eligible for Retiree Coverage If you are on COBRA due to severance and your COBRA subsidy continues into 2022 due to your severance, you may continue on COBRA coverage until the subsidy runs out. Once the subsidy expires, you have a one- time opportunity to elect retiree health care coverage through the JPMorgan Chase U.S. Retiree Benefits Program (if you are pre-Medicare-eligible) or through Via Benefits (if you are Medicare-eligible) for medical, prescription drug, dental and/or vision coverage. There is no deferral of retiree coverage; if you don’t enroll when you are first eligible or when your subsidy first expires, you will permanently forfeit your participation in the JPMorgan Chase U.S. Retiree Benefits Program. After your COBRA subsidy expires, you can continue with COBRA coverage for the remainder of your COBRA period, paying the full cost of coverage. However, at the end of your COBRA coverage (generally 18 months), you will not be able to elect retiree coverage through JPMorgan Chase (or receive a Health Reimbursement Arrangement [HRA] with Via Benefits). See the As You Leave Guide on My Health for more details. Please note that when your COBRA subsidy expires, this does not constitute a qualified status change, and therefore you may not elect a different medical plan. If you have questions about the Medicare-eligible coverage through Via Benefits, they can be reached at 1-844-448-7300, 8 a.m. to 9 p.m. Eastern Time, Monday through Friday. You may also access the Via Benefits website for JPMorgan Chase members at https://my.viabenefits.com/jpmc. Important reminder about paying for your COBRA coverage Your monthly payments are due on the first day of each month. To ensure that payment is received by the due date and to avoid cancellation of your COBRA coverage, you may wish to have your payments deducted directly from your checking or savings account via direct debit, if you have not already done so. Please call HR Answers for more information (see page 13 for contact information). Please note: If you fail to remit your monthly payment within 31 days of the due date, your coverage will end retroactive to the last day of the month for which payment was received. Coverage cannot be reinstated. For example, your January 1 payment is past due after January 31. Your February 1 payment is past due after March 2. A word about dependent eligibility If you are enrolled in benefits, you may also cover eligible dependents (e.g., spouse/domestic partner, children). Children are eligible until the end of the month in which they turn age 26. For more on eligibility requirements, access the 2022 Dependent Eligibility Requirements on My Health > 2022 Benefits Resources. What’s Changing for 2022 No major changes to the Medical Plan Great news! For 2022, there are no major changes in the types of services covered under the JPMorgan Chase Medical Plan with Aetna and Cigna or the Prescription Drug Plan with CVS Caremark®. In 2022, the Medical Plan will continue to offer: • Eligible in-network preventive care (including physical exams and recommended preventive screenings) and eligible preventive generic drugs covered at 100% with no deductible, coinsurance or copayments. • In-network primary care office visits covered at 90% with no deductible. • The same deductibles and coinsurance maximums. • The same types of covered medical services. 2022 benefits changes and enhancements Benefits enhancements and changes include: • Infertility benefits support: As of July 1, 2021, if you are enrolled in the JPMC Medical Plan, you and your covered family members can receive help from WINFertility nurses with selecting a high-quality provider, understanding treatment options, and receiving clinical and emotional support. Enroll with WINFertility and complete a nurse consultation in order to have access to a $30,000 medical lifetime maximum benefit ($10,000 medical benefit if member does not engage with WINFertility). Medical benefits (e.g., provider network, claims administration) will continue to be managed by your health care company — Aetna or Cigna. Prescription drug benefits will continue to be managed by CVS Caremark. You can reach WINFertility by calling 1-833-439-1517, Monday through Friday, 9 a.m. to 7:30 p.m. Eastern Time. • Prescription Drug Plan: Review the 2022 formulary drug lists for the latest information on covered drugs. Remember to check frequently throughout the year for up-to-date information. • Beginning January 1, 2022, if you are enrolled in the JPMC Core Medical Plan and move to Arizona or Ohio during the calendar year, you’ll automatically be enrolled in the JPMC Simplified Medical Plan, effective when you submit your address change. Please note that your health care company (Aetna or Cigna), election (Option 1 or 2) and dependent coverage will remain the same and cannot be changed as a result of your move. • 2022 MRA Action Plan: New resilience training through meQuilibrium and My Finances and Me activities, which you can complete to earn funds in your 2022 MRA. See MRA details on page 4 for more information. No change to the Prescription Drug Plan You’ll continue to have the same prescription drug coverage administered by CVS Caremark, regardless of the health care company you choose. Additionally, there are no Prescription Drug Plan changes for 2022. That means you won’t receive a new prescription drug ID card for 2022. As always, you can check to see if your prescription drug is covered on CVS Caremark’s Covered Drug List on the CVS Caremark website, which you can access through My Health or at www.caremark.com. Drug lists are subject to change quarterly by CVS Caremark. If your drug is not on the list, ask your doctor if there’s an alternative you can take. Note: Your cost will be lowest for generic and preferred brand name prescriptions (versus non-preferred brand name or drugs that are not covered). Your Medical Reimbursement Account Your Medical Reimbursement Account (MRA) will be managed by the health care company you choose for 2022 — either Aetna or Cigna. Reminders: • Your MRA is funded by JPMorgan Chase when you and your covered spouse/domestic partner complete wellness activities. You cannot contribute to the MRA. MRA funds can only be used to pay for eligible medical and prescription drug expenses. • Payment methods: During Annual Benefits Enrollment, you can choose to have expenses paid from your MRA through automatic claim payment or with a debit card. Automatic claim payment vs. debit card You have two ways to pay claims from your MRA: Automatic claim payment and debit card. During Annual Benefits Enrollment, you’ll be able to select one of these payment methods: • Automatic claim payment method. Both Aetna and Cigna will automatically use your MRA funds to pay for your portion of eligible out-of-pocket medical and prescription drug expenses. • Debit card payment method. You can choose to use your debit card or pay with your personal funds for eligible expenses — giving you more control over when you’re using your MRA funds. Keep in mind that you’ll need to keep your receipts and be prepared to substantiate any debit card claims, as required by the IRS. If you’re currently enrolled in the Medical Plan and do not change your payment method election or take no action during Annual Benefits Enrollment, your 2021 payment method will carry over to 2022. To make a change to your 2022 payment method, select View/Change on Your Benefit Summary. Please keep in mind that automatic claim payment assumes you did not pay your portion of your eligible medical expense at the time of service and pays your provider/facility your portion directly. If you pay your portion at the time of service, your provider/facility will be double-paid — once from you at the point of service and once from your MRA (assuming funds are available). It will be your responsibility to recoup the overpayment from your provider/facility (although your health care company can try to assist you). Therefore, if the providers or facilities you visit are requiring you to pay at the point of service, the automatic claim payment method may not be right for you; you should consider the debit card. Wellness Rewards A key part of our Medical Plan and Wellness strategy is to engage employees in specific wellness activities by providing Medical Reimbursement Account (MRA) funds. If you enroll in COBRA Medical coverage, you can continue to earn Wellness Rewards to increase the value of your MRA. While enrolled in COBRA, you can use any remaining balance in your MRA through automatic claim payment or debit card payment method depending on the method you elect. Complete your Wellness Activities by November 19, 2021 (11:59 p.m. Eastern Time) Last year, we paused on asking you and your spouse/domestic partner to complete biometric Wellness Screenings as part of our annual wellness campaign. Plus, many of us delayed receiving other preventive care, like annual physicals and health screenings. We believe it’s important to return to more usual routines when it comes to your health — that includes preventive care. Because knowing their numbers (e.g., blood pressure, body mass index [BMI], blood sugar, cholesterol, triglycerides and A1C) has helped so many people understand their health, uncover health risks and receive earlier treatment, biometric Wellness Screenings are back. This means you’ll need to complete both the biometric Wellness Screening and online Wellness Assessment by November 19, 2021 (11:59 p.m. Eastern Time), to earn $200 in your 2022 Medical Reimbursement Account (MRA). If your covered spouse/domestic partner completes both of these activities during this time frame, you will earn another $100 in your 2022 MRA. Important note: If you terminate employment during 2021, you must elect COBRA to be eligible to earn Wellness Rewards for completing a biometric Wellness Screening and online Wellness Assessment plus continue as a COBRA participant through the award date in mid-January 2022. If you terminate before the award date in mid-January 2022 and do not elect COBRA, you will not receive funds for completing Wellness Activities by November 19, 2021 (11:59 p.m. Eastern Time), even if you are eligible for retiree medical coverage. If you terminate employment with an MRA balance and do not elect COBRA, you can still submit claims to your health care company for reimbursement of eligible medical and prescription drug expenses that were incurred before the end of the month in which your employment terminated. How to complete the biometric Wellness Screening You have several options to complete your biometric screening, details of which can be found on the Wellness Screening Portal run by Quest Diagnostics. Options available to you if you have COBRA coverage under the JPMorgan Chase Medical Plan are: • Download a Physician Wellness Screening form and take it to your doctor. Have them complete ALL biometric fields (plus Date of Service) and sign the form. You can submit the completed form via fax or upload it to the Wellness Screening Portal. OR • Click on the Quest Patient Service Center section to locate and schedule a screening at a lab or retail location (advance registration is required). How to complete the online Wellness Assessment To complete your assessment, go to mycigna.com. Everyone will complete the online Wellness Assessment with Cigna — no matter your health care Have questions? Contact company. Cigna at 1-800-790-3086. Former employees: The first time you access mycigna.com, you’ll need to register and provide your name, date of birth, ZIP code and Social Security number (or Cigna Member ID if Cigna is your health care company). Covered spouses/domestic partners: You will need to register on mycigna.com as yourself, if you haven’t already. You’ll be asked to provide the JPMorgan Chase former employee’s Social Security number and ZIP code, as well as your name and date of birth. Additional Wellness Activities Changes for 2022: • Online training, Mindfully Managing Stress through meQuilibrium: Earn $50 in your MRA and learn about the powerful science of resilience and techniques to help gain control over your emotions, thoughts and stress. Please note: Spouses/domestic partners can participate, but cannot earn MRA dollars for doing so. • My Finances and Me activities: Earn $50 in your MRA by (1) completing a short online Financial Wellness Assessment* and (2) attending a group education session on a variety of financial topics, such as basic money management, navigating student loans and retirement planning. Please note: Spouses/domestic partners can participate, but cannot earn MRA dollars for doing so. *The Financial Wellness Assessment is not part of the online Wellness Assessment that, along with the biometric Wellness Screening, is part of the Initial Wellness Activities. Don’t forget your 2021 MRA funds! Remember, you have until December 31, 2021, to maximize your earnings in your 2021 Medical Reimbursement Account (MRA) when you and your covered spouse/domestic partner complete Additional Wellness Activities — like health coaching, online learning, a stress/resiliency program and more — by December 31, 2021. Visit My Health > 2021 MRA Additional Wellness Activities for more information. Get ready to maximize your 2022 MRA by completing Additional Wellness Activities throughout the year While on COBRA coverage, you have the opportunity to earn up to $800 when you complete Additional Wellness Activities during 2022. In December 2021, go to My Health and check out the 2022 MRA Action Plan (available in December 2021) to decide which moves you’d like to make. Your Privacy Is Important The privacy of your health information is important to you and to JPMorgan Chase. We are committed to protecting your personal health information and complying with privacy laws, including the Health Insurance Portability and Accountability Act (HIPAA). This means that when you complete a Wellness Screening or a Wellness Assessment, participate in any health coaching activities or receive health care treatment of any kind, your personal health information is not disclosed to anyone, including JPMorgan Chase, without your authorization and except as permitted by HIPAA. (For detailed information about your HIPAA Privacy Rights, please see the Privacy Notice found on My Health). Dental Plan There are no changes to the Dental Plan for 2022. For more information about the Dental Plan, go to the Benefits Web Center on My Health. Vision Plan There are no changes to the Vision Plan for 2022. Electing COBRA Coverage for JPMorgan Chase Onsite Health & Wellness Centers, Wellness Screenings, Tobacco Cessation and Employee Assistance Program (EAP) Counseling You may elect access to JPMorgan Chase’s onsite Health & Wellness Centers, participate in Wellness Screenings, Tobacco Cessation Program and utilize EAP counseling while on COBRA — even if you decline COBRA coverage under the JPMorgan Chase Medical Plan. These benefits are offered as a single election. • If elected, all covered dependents may access EAP counseling telephonically. • Access to JPMorgan Chase’s onsite Health & Wellness Centers or Wellness Screenings at your doctor’s office or qualified labs is only available to former employees and not to covered spouses/domestic partners or children. Note: If elected, you would access the JPMorgan Chase onsite Health & Wellness Centers only after coordinating with that facility’s security personnel. Also, the Centers offer limited medical services. Therefore, give careful consideration as to whether this is a service you would use. • If you are interested in participating, please contact HR Answers and speak to a representative. Please note: • If you elect COBRA coverage for the JPMorgan Chase Medical Plan, that coverage includes Wellness Screenings at your doctor’s office or qualified labs and the Tobacco Cessation Program for you and your covered spouse/domestic partner. • If you do not elect the Medical Plan COBRA coverage, but would like COBRA coverage for onsite Health & Wellness Centers/Wellness Screenings, Tobacco Cessation and EAP counseling, you must make the COBRA election for those services. − Please note: If you elect both COBRA Medical Plan coverage and COBRA “bundled” coverage, you will not be charged twice for the Wellness Screening and Tobacco Cessation Program. My Health to Help You Choose and Enroll In addition to this Annual Benefits Enrollment Bulletin, several resources are available on My Health to help you during this benefits enrollment, including a link to the Benefits Web Center, where you will make your enrollment choices. Getting to My Health is easy; you can access the site anytime, and the Single Sign-On feature gets you around without having to re-enter passwords. If you want to make changes or use the online tools, you can access My Health via the Internet at myhealth.jpmorganchase.com and select “Current or Former Employee” (or select appropriate option under “Covered spouses, domestic partners and dependents of employees” for your covered family members). Once on My Health, click the green Enroll now button. Look for the Summaries of Benefits and Coverage, which allow you to easily compare your JPMorgan Chase Medical Plan options to your spouse’s/domestic partner’s or child’s medical plan options that may be available through another employer. Summaries of Benefits and Coverage are found on the Benefits Web Center accessible through My Health. If you are not a former JPMorgan Chase employee but are the spouse/domestic partner or surviving spouse/domestic partner of a JPMorgan Chase employee: My Health is available to you for information only. You must enroll for benefits through HR Answers at 1-877-JPMChase (1-877-576-2427). Now Is the Time to Enroll Your first stop for enrollment is My Health via the Internet at myhealth.jpmorganchase.com. Click Enroll now to access the Benefits Web Center. You will need to enter your Standard ID number, shown on your enclosed Personalized Fact Sheet, and your JPMorgan Chase Single Sign-On password (see “How to Use ‘Single Sign-On’” on page 15). If you are not a former JPMorgan Chase employee, you cannot enroll through My Health. You may request any of the information in this bulletin by calling HR Answers at 1-877-JPMChase (1-877-576-2427). You must enroll through HR Answers; representatives are available to assist you from 8 a.m. to 7 p.m. Eastern Time, Monday through Friday, except certain U.S. holidays. Confirming your coverage If you make any changes to your assigned coverage or you enroll a dependent for the first time in 2022, you must confirm your elections: • Online through the Benefits Web Center. You’ll need to “Confirm” your choices in order to complete your enrollment. You’ll know that your elections have been saved when you see the “Completed Successfully” screen. Please print a copy of this screen and retain it for your records. You will also be able to view and print your 2022 coverage through the Benefits Web Center. This will serve as your confirmation of your 2022 benefits elections. You will not receive a paper confirmation statement in the mail. OR • By calling HR Answers. A confirmation of your 2022 benefits elections will be mailed to your home address. Reminder If you do not make any changes during your designated enrollment period, the information shown on the “Enroll in Your Benefits” screen on the Benefits Web Center, available through My Health, and the enclosed Personalized Fact Sheet will serve as confirmation of your 2022 benefits elections. Benefit Reminders HIPAA privacy rights and protected health information The Health Insurance Portability and Accountability Act (HIPAA) requires employers to communicate how certain protected health information under employee health care plans may be used and disclosed, as well as how plan participants can get access to their protected health information. Accordingly, JPMorgan Chase will distribute once every three years a “Privacy Notice of Protected Health Information under the JPMorgan Chase Health Care Plans” to you that describes in detail how your personal health information may be used and your rights with regard to this information. (This notice was distributed in 2020.) Newborns’ and Mothers’ Health Protection Act In accordance with the Newborns’ and Mothers’ Health Protection Act, group medical plans and health insurance issuers may not, under federal law, restrict benefits for any hospital length of stay in connection with childbirth for the mother or newborn child to less than 48 hours following a normal delivery, or to less than 96 hours following a cesarean section. Further, the plan cannot require that any medical provider obtain authorization from the plan or any insurance issuer for prescribing a length of stay less than these periods. Medical Plan post-mastectomy benefits All options under the JPMorgan Chase Medical Plan cover certain breast reconstructive benefits in conjunction with a mastectomy for eligible participants. Coverage under the Medical Plan is available for: • Reconstruction of the breast on which the mastectomy was performed; • Surgery and reconstruction for the other breast to produce a symmetrical appearance; and • Prosthesis and treatment of physical complications for all stages of mastectomy, including lymphedema. This coverage is subject to the terms of the Medical Plan option in which you participate, including relevant deductibles and coinsurance provisions. How to Use “Single Sign-On” To ensure the security of the Benefits Web Center, available through My Health, a “Single Sign-On” password is required before access can be processed. You’ll know to use the Single Sign-On password when you see this image: How to log on from home using the Internet: Step 1 A. ENTER YOUR STANDARD ID ON THE SINGLE SIGN-ON SCREEN. The Standard ID that was in effect at the time you left JPMorgan Chase will remain in effect after you leave. B. ENTER YOUR DEFAULT PASSWORD, which is composed of: • The first three characters of your Standard ID; • The last four digits of your Social Security number or national ID number; and • The first letter of your last name. • For example, if Joe Smith’s Standard ID is U123456 and his Social Security number is 123-45-6789, his initial password is U126789S. C. CLICK “LOGIN.” Step 2 A. CHANGE YOUR PASSWORD. Re-enter your initial password in the “Current Password” field, and then create and confirm a new password following the rules outlined at the top of the screen. You will need to change your password on your first visit only. Please remember the password you choose for future access. B. CLICK “CONTINUE.” You’ll now access the requested website or tool. Please note: You will be able to create a new password during your initial visit only. You will be unable to reset your password automatically through Single Sign-On during subsequent visits.
