                             2021 Benefits Enrollment
                             At Bell County, we truly value the dedication that goes into your work every day. We

                             you. That’s why as a Bell County employee, you have access to a comprehensive, quality
                             benefits package that offers flexibility and security. Please take the time to read and
                             understand this overview so you can gain a better understanding of your options.

                             The information provided in this overview is for illustrative purposes only. Refer to the
                             applicable carrier material and plan documents for exact descriptions of plan benefits
                             and conditions.

                             Insurance Changes
                             Bell County will continue health insurance through the Scott & White Health Plan and
                             dental insurance/vision reimbursement through Ameritas for the 2021-2022 contract
                             year (effective 11/01/2021). There will be no changes in the plan designs. There are
                             changes to the rates for the Scott & White Health Plans but there are no changes to
                             the rates for the Ameritas Dental Plans. Bell County employees are given the choice
                             of two (2) network plans for health insurance, Baylor Scott & White Preferred Network
                             (Smaller Network) or Scott & White HMO (Broader Network). The benefits are the same,
                             so the only difference in the two (2) options are the network and the rates. Employees
                             will pay 1.5% of the premium and Bell County will contribute (pay) the remainder
                             of the premium for the EMPLOYEE ONLY, Baylor Scott & White Preferred Network for
                             health. Bell County employees are given the choice of two plans for the Ameritas
                             dental insurance, the Base Plan, or the Buy-Up Plan. The benefits are the same, so the
                             only difference in the two (2) options are the network and the rates. Bell County will
                             contribute (pay) the EMPLOYEE ONLY, Base Plan for dental. You can also enroll in a
                             Flexible Spending Account for Medical and Dependent Care.

                             Please be sure to bring social security numbers and dates of birth for your dependents
                             to be enrolled. If adding dependents, verification (such as providing marriage
                             certificate and/or birth certificates) must be completed with Human Resources prior to
                             your benefit counseling session.
