How to Register 1. Complete the Registration Form and Waiver in this packet. 2. Register in person with your first month’s payment at the Northbrook Sports Center (non-refundable). Additional information including the parent handbook, school calendar and how to register for your ePACT account will be emailed in July 2021. Proof of immunizations is due November 1, 2021. ePACT ePACT is a secure emergency network that will allow us to collect medical and emergency contact information electronically. Not only will ePACT replace paper forms, it will ensure we have a way to communicate with you in the event of any changes at the program or an illness, injury or larger scale emergency. Payments Unless paid in full, your first monthly payment is due at the time of registration. This payment is non-refundable. Monthly payments will be processed electronically through a debit/credit card in eight additional equal installments based on your balance. The second monthly installment will be charged on September 1, 2021; the last monthly installment will be charged on April 1, 2022. Parent Night A $25 late fee will be applied if payment is not received by the 15th of the month. Any Parent Night will be held on August 31, 2021 at 6pm for parents of enrolled children. This payment returned for non-sufficient funds will incur a fee of $15 in addition to the late fee. presentation will cover the Preschool Parent Handbook, introduce our teachers and allow time for questions. Attendance is highly encouraged. Parent Night may be held virtually. Information Failure to keep an account current may result in dismissal from the program.
