Why We Do This 4 The world is missing out on the full potential of so many brilliant people who Don’t Feel Well. We need a new branch of true health-care with an army of practitioners who are skilled in and passionate about proactive, preventive, patient-centric, © Purpose Inc. personalized, systems-minded, root-cause medical science. By honoring and alleviating dis-ease, we minimize disease and make medicine affordable, but more importantly, we give back to the world the riches of all its peoples able to rise to their full potential. We want all SAFM-trained practitioners to be confident, credible, and competent in pursuing the work they love and creating a wildly successful – and fulfilling - practice.

SAFM’s Certification Program 8 Applied Functional Medicine for Coaching (AFMC)TM Acknowledges advanced training and validated competency in the practical application of functional medicine principles and science. © Purpose Inc. Distinguishes a practitioner from his or her peers. Earns the practitioner specific benefits to support their continuing education and business. Continuing education, not a foundational credential. All SAFM students already have their foundational license or certification and a practice in which they may progressively utilize their knowledge and skills while an SAFM student. Not a minimal competency credential threshold that the student should intend to achieve prior to using their skills. Requires both demonstrated foundational knowledge and demonstrated expertise in applying astute, targeted knowledge in clinical practice with a complex patient/client. AFMC Certification Requirements 9 A three-step certification process, each of which must be completed during a separate official AFMC exam window (offered in October, February, and July). Level 1 Examination Requires completion of four Core 101 semester courses* (and self-test submission for each) and completion of at least six All Levels Open Coaching Calls prior to registration. Level 2 Examination © Purpose Inc. Requires successful completion of all Level 1 prerequisites and examination plus completion of four Advanced 202 semester courses** (and self-test submission for each) and completion of at least six additional All Levels Open Coaching Calls prior to registration. Level 3 Competency Case Study Requires successful completion of all Level 1 and Level 2 prerequisites and examinations. Three-year recertification requirement to sustain credential and validate continued competence. Requires completion of at least six additional All Levels Open Coaching Calls (since initial Certification awarded) prior to registration. Recertification exam content will cover assessment and interpretation of a standard case. There will be a Review class offered in advance about the featured case (with no other required preparation for this exam). * Disease 101, Nutrients/Supplements 101, Labwork 101, Disease Begins in the Gut 101. ** Disease 202 Nutrients/Supplements 202, Labwork 202, Cellular Metabolism.

AFMC Timing and Flexibility 12 Flexible timing to support the needs of each unique practitioner. Typical, example certification plan: Experience Core 101 Semester fully (Jan-Aug, Year 1) Prepare for and pass Level 1 Examination (Sept-Oct, Year 1) Take a break from studies or reinforce prior learning (Nov-Dec, Year 1) Experience Advanced 202 Semester fully (Jan-Aug, Year 2) Prepare for and pass Level 2 Examination (Sept-Oct, Year 2) © Purpose Inc. Take a break from studies or reinforce prior learning(Nov-Dec, Year 2) Prepare for and pass Level 3 Competency Case Study (Jan-Feb, Year 3 *or* appreciably later) Recommended timing of Level 3 Competency Case Study submission is highly dependent on the volume and pace at which the student is able to apply, develop, and hone their skills in their personal clinical practice. We recommend applying your skills across several cases before selecting a notable, complex patient or client case that gives you generous opportunity to showcase your expertise. Successful completion will require your research into targeted functional medicine interconnectedness that goes beyond the scope of the standard Level 1 and Level 2 content. The AFMC Recertification exam will be first offered in the exam window of July 2021. To maintain the AFMC certification credential, practitioners must achieve Recertification during the exam window three years after their initial certification (or in one of the two exam windows beforehand). Certified students will receive email reminders about Recertification opportunities in the year prior to expiration. All current AFMC certified practitioners as of March 2019 must recertify by March 2022.
AFMC Logistics 13 An optional process. AFMC exam windows are offered each year in October, February, and July. All three certification levels are available during every AFMC exam window. All pre-requisites must be satisfied before Registration for a given certification level. Registration for each AFMC exam window opens about 8 weeks in advance, and eligible students will be notified via email. Registration will be open for two calendar weeks. © Purpose Inc. Each level features a Review course and a guided six-week study plan to aid student preparation. Cost of all certification Levels is included as part of the Core 101 and Advanced 202 semesters tuition as long as the Level 1 examination is completed within 18 months of Core 101 semester kick-off and the Level 2 examination is completed within 18 months of Advanced 202 semester kick-off. Delayed completion or required repeat attempts incur a $300 fee per examination window. There is no additional cost for the Level 3 Competency Case Study initial attempt and no required completion timing. Required repeat attempts incur a $300 fee per examination window. Level 1 and Level 2 and Recertification examinations are timed, two-hour, open-book, online exams. Successful completion requires a passing score of 85% or higher. One additional attempt is permitted within each examination window if the first attempt is not successful. Level 3 Competency Case Study submissions are graded on a pass/fail basis. If a case presentation is insufficient to meet all standards, then a separate, different case and assessment must be presented during a future examination window. Exam Integrity Clause 14 To ensure a high-integrity and authentic examination process for each student, it is imperative that every student commit to disclosing no details whatsoever about the AFMC examinations (including, but not limited to, the number of questions, content, scope, question sourcing, recommendations on what to study, course codes, etc.) to any other person, including peers, friends, or colleagues. This commitment goes beyond current students, as others you know may indeed be future students. © Purpose Inc. By entering your current legal first and last name and today's date below, you agree and commit to uphold the integrity of this examination process by following this protocol fully. If you do not enter your name and today's date accurately or at all, please note that we must discard your exam submission without refund and remove you from both current and any future certification eligibility. Please be aware that any discovered exception (no matter how small) to the above protocol (in the sole judgement of the AFMC certifying board) will result in the removal of both parties from eligibility to earn the AFMC certification at any time. Parties will be given a one-time opportunity to speak for their actions and clarify any misunderstanding, and the AFMC board's decision will be final. By submitting this examination for certification consideration, you agree to be subject to this protocol in order to maintain the integrity of your future AFMC certification. Prerequisite Completion (Courses) 15 Required Course Completion and Self-test submission Students complete and submit a course Self-test online at a timing of their choosing when they feel prepared to do so after thoroughly experiencing the course. An Answer Key to the Self-test will be emailed to the student after completion. Note that the Self-test and Answers Keys are part of the content scope considered in the appropriate exam © Purpose Inc.
Prerequisite Completion (OCCs) 16 Required Open Coaching Calls Required for both Level 1 and Level 2 exam registration beginning in August 2019. This requirement is waived for successfully completed Level 1 and Level 2 certifications prior to this time. Must be All Levels Open Coaching Calls (not Core 101). May be experienced Live or via Recordings. Remember six months of archives are always housed on the All Levels Open Coaching Calls course page in your student account. During each Open Coaching Call, a three-number code will be announced. © Purpose Inc. Each student must register participation in each Open Coaching Call and successfully enter the correct code in order to receive credit for completing that Call. Links to register participation will be available on the All Levels Open Coaching Call course page as well as the webinar and audio download pages for each recording.

Learn from the Roots Up and Stay Open 20 A key SAFM value is deep, thorough, repetitive learning and practice. You will not find any “FM for Dummies” or quickie “If this, Do that” exhaustive tables in any of our programs. Hold yourself accountable for thorough learning. The devil IS in the detail. Make the time to become truly knowledgeable and practiced before you start using what you learn (e.g. Help someone with a thyroid issue *after* you’ve completed the Thyroid/Adrenal class (multiple reviews), reviewed all the Q&A posts, and practiced with © Purpose Inc. multiple cases). The WS Forum is not “Alexa” (or Siri or Cortana). Avoid falling into the trap of looking for or relying on quickie answers/solutions shared online. The goal is to support each other in really learning the foundational material (not just “the right answer”). Purposefully avoid the trap of black-and-white thinking. There is always more than one (usually dozens) reason why a certain situation (or lab value) exists. A little knowledge can be a dangerous thing. Depending on how you use it Keep Beginners’ Mind. Especially as you start to feel confident and savvy! But hang in there with content that feels too advanced for you. Absorb what you can. Let it fuel your learning. Stay curious. Breathe. Be patient. Seek to be authentically confident and expert. Let the learning take time.
Know and Honor Your Scope 21 All SAFM students must consider and honor your own scope of practice in making choices about how they will apply their learning in support of patients and clients. We have a diverse melting pot of students, including prior training, certifications, degrees, licenses, expertise areas, modalities, experience levels, country/state of operation, © Purpose Inc. preferences, confidence. Do You.
