1) B OTOX®

“The One and Only” Botox has been approved by Health Canada for esthetic improvement of dynamic facial lines in upper
face since 2001. Botox relaxes the injected muscles for a period of 3 – 6 months. Some improvement can be seen in 3-5 days
but the full effect takes 2 weeks. It has been observed that with consistent, regular treatments, the results get better and
also last longer. In general, the side effects and complications of treatment are minimal, rare and always temporary.

Xeomin is a Botox alternative that has been approved by Health Canada since 2012 for cosmetic treatment of dynamic
wrinkles in face. Xeomin causes partial or complete relaxation of the injected muscle in face. Some results can be seen in 2-
3 days but the full effect is achieved in approximately 2 weeks. The results last on average 3 months, depending on the
number of injected units and also on the strength of the treated muscles. Side effects and complications of treatment are
the same as with Botox: rare, minimal and always temporary.

3) DERMAL FILLERS
A) RADIESSE HAND REJUVENATION: Consult with Dr. Lenka Kucerova and Shelby Holmes RPN
Radiesse is calcium based dermal filler that restores lost volume in the back of the hands. It has been approved by Health
Canada since 2005. It is injected in several tiny injections under the skin of the backs of hands that have been numbed with
topical numbing cream for painless or almost painless experience. Our patients report 0-2 out of 10 pain level with
injections of Radiesse. The results are immediate but they continue to improve over a period of 5 months or longer as the
tiny biodegradable calcium-based microspheres that are also found naturally in our bones and teeth are gradually replaced
by newly produced natural collagen fibres in our skin. There is very little to no down time of mild swelling and possibly
bruising, that resolves over few days. It is recommended to repeat the treatment in 3-5 months, especially with more
advanced volume loss in hands. It is one of the most rewarding procedures!

Hyaluronic acid based Dermal fillers are sterile clear gels consisting of non-animal hyaluronic acid, that are
injected under the skin, to correct loss of volume in cheeks, to soften some facial lines, wrinkles and folds, for lip
enhancement and for reshaping of jawline and chin. The results can last 6-18 months depending on the area of
the face, the volume that was injected and also on the type of the filler (lip results last usually 6 months). Touch
ups and follow up treatments help sustain the desired degree of correction.
Juvederm Ultra™ $400/syringe (Regular $450/syringe) unlimited quantity
For soft lines in face or for touch ups of nasolabial folds, marionette lines, for subtle lip enhancement or for fine
lines around lips. It is recommended to purchase at least two syringes. They will be banked for future use if they
are not used in one session.

For your first treatment or repeat treatment of deeper nasolabial folds and marionette lines. Usually two syringes

As all these products are of the same value, so it really does not matter which one you purchase. They can be
mixed and matched to get you the desired results. Voluma is for cheeks, Volux is for contouring of jawline and
chin, Volite is for fine horizontal lines on neck (so called necklace lines), Volbella is for fine lines around the lips
(so called bar code lines) and for frown lines and Volift is for lips. These products are manufactured using slightly
different patented technology and they last longer (up to 18 months). Usually at least two syringes are needed to
correct the problem to your satisfaction. If we don’t use your prepaid products in the first treatment, we can use
them later on for touch ups or for treatment of other areas of face or neck.

Belkyra® is injectable solution of a substance called deoxycholic acid, which is approved by Health Canada for
cosmetic improvement of submental fullness, commonly called “double chin”, in adults from 18 to 65 years of age.
Double chin is often caused by genetic predisposition (it runs in the family), aging and changes in weight. Overall,
68% of adult Canadians are bothered by the fullness under their chin that is sometimes apparent only from their
profile and makes them look older and heavier than they are. The same natural substance that is in Belkyra is also
found in our digestive tract where it breaks down dietary fat. Belkyra is injected in several tiny injections to the
double chin area. There will be a slight swelling and tenderness in the area for several days after. Minor bruising is
easily covered by makeup. Belkyra will cause the under-chin fat cells to diminish gradually over the course of 3-4
weeks following the injections. The fat cells are dissolved and gone permanently! On average, people need two or

Latisse is a clear liquid that is used for rejuvenation of eyelashes to increase their length, thickness and darkness. One drop
of Latisse applied daily on a small brush to the base of eyelashes will show the results in 6-10 weeks. Younger women
usually see the results even sooner. While majority of users have no side effects, initially, about 4% of users may
experience itchy eyelids. The itch goes away within a short time.

This program was first used by Dr. Simeons in 1954 for treatment of obesity. It consists of a low-
calorie diet combined with injections of hCG (human Chorionic Gonadotropin hormone) and B

preserving the healthy subcutaneous fat and lean muscle. It does not dehydrate the body like many
other diets. It speeds up the metabolism in a healthy way.

TCA CROSS is the chemical reconstruction of skin scars (CROSS) using Trichloroacetic acid (TCA).

Dr. Lenka Kucerova
Dr. Lenka is our Medical Director. She has undergone extensive training, preceptorships and certifications in many non-surgical
cosmetic medicine procedures (in Santa Monica, Beverley Hills, New York, Boca Raton, Phoenix, Laguna Beach California, Toronto,
Las Vegas, Miami, Banff Alberta, Dublin Ireland, Prague Czech Republic and London England). She is also certified through
CoolSculpting University and has achieved Double Diamond status with Allergan Inc. She has been injecting cosmetic Botox,
therapeutic Botox for treatment of migraines, Xeomin and dermal fillers for facial and hand rejuvenation for over 15 years, after she
