Summer 2018 The Renal Messenger Page 1 From the Editors Desk... Dear members and supporters, welcome to 2018, I wish you all a safe and happy year ahead. You will see from the front cover that World Kidney Day is on Thursday 8th March this year and this day is shared with International Women’s Day hence the international logo. There is nothing formal planned for this day as the Society has planned to “hit the ground running” with our fundraising efforts to boost our patient support programmes that continue every day of the year! Kidney Health New Zealand have provided good information on Pages 5 and 6. Firstly we kick off with a Cake Stall in the Hospital foyer on Friday 23 February (see pages 2 and 3 for the finer details). Following that we are holding our “Annual Ethel and Bethel Bingo Night” on Friday 13th April—many of you will know that this is a wonderful night of fun and laughter, with prizes thrown in! We will be selling Entertainment Books again this year, so get in touch if you would like a copy reserved. The digital version has been gaining popularity, so all you have to do is purchase via the Entertainment Book website and nominate the Society as the recipient of the donation portion of the cost. I am still old school and like to browse through the book. Special thanks to Sharon (page 10) for “Life with Freddie” - we love reading about personal journeys and would love to receive more! Just before sending this edition to print, I attended the funeral of a long term member who received a deceased donor kidney later in life and it was with her for 15 years. The transplant made so many more things possible for this wonderful kind lady, who left a lasting legacy in Christchurch. I have a supply of Organ Donation NZ posters and information in the office if anyone would like some to distribute to their local library, medical centre, etc, we need to encourage people to “have that conversation” so more lives are enhanced in this way. Summer 2018 The Renal Messenger Page 3 Special thanks to MY FOOD BAG for donating the above Baking Box for our upcoming cake stall, this will be raffled on the day and Viv or Lois will be around the HDTC, Acute Dialysis and Ward 14 prior to the day, so you can ask them about purchasing a raffle ticket. Also special thanks to HARCOURTS GRENADIER (Beckenham Office) for coming to the rescue! We had a tentative promise from a large organisation to help us out with the cake stall but they were unable to help when they realised the date was set, so we now have the support of this Harcourts office and we are very excited to see what goodies they make for us on the day. Of course all of the Committee members, volunteers and some of the Nephrology Staff and members of the Society bake, but it is always wonderful to have a large selection of goodies on offer. Please get in touch if you are able to help us out too, everything helps! Summer 2018 The Renal Messenger Page 5 Chronic Kidney Disease in New Zealand What is Chronic Kidney Disease (CKD)? CKD is a progressive loss in kidney function over a period of months or years. Each of your kidneys has about a million tiny filters, called nephrons. If nephrons are damaged, they stop working. For a while, healthy nephrons can take on the extra work. But if the damage continues, more and more nephrons shut down. After a certain point, the nephrons that are left cannot filter your blood well enough to keep you healthy. When kidney function falls below a certain point, it is called kidney failure. Kidney failure affects your whole body, and can make you feel very ill. Untreated kidney failure can be life-threatening. The number of people in New Zealand with CKD is currently unknown, although based on overseas populations an estimate of 7–10% of the population would seem reasonable. It has also been estimated that one in three people in the general population are at risk of developing CKD. The burden of CKD on New Zealand communities is increasing and the number of people requiring dialysis has almost doubled since 2000; in 2016 there were reported to be 2678 people undergoing dialysis and 1694 people who had a successful kidney transplant. Approximately half of all people in New Zealand requiring dialysis have diabetes as a primary cause of their condition. Most patients with stable CKD can be fully managed in primary care, particularly patients with stable stage 3 CKD or those patients aged over 75 years with early and stable stage 4 CKD. The most important aspects of CKD management are: Controlling blood pressure; and if the patient has diabetes, Controlling blood glucose
The Renal Messenger Page 6 Summer 2018 Patients with stable CKD (stage 3 – 4) have a five-year cardiovascular risk > 15%, if they do not have diabetes, which increases to > 20% if diabetes is also present. These patients need appropriate cardiovascular disease management and it is important that additional medicines, e.g. statins and aspirin, are initiated according to cardiovascular guidelines to reduce cardiovascular risk. Complementary community-based care strategies involving nurse-led teams have been shown to improve outcomes in patients with moderate CKD who are at high-risk of progressing to kidney failure People with end-stage renal disease commonly have a progressive fall in GFR, marked proteinuria and hypertension. Early recognition and management, and appropriate referral can prevent end-stage renal disease in some cases and greatly improve the outcome for others.2 Being able to detect CKD early allows: Early protective intervention to reduce progression towards end-stage renal disease (CKD stage 5) Monitoring and treatment of cardiovascular risk factors to reduce cardiovascular disease Monitoring and treatment of complications. Who is at risk of CKD? Aged over 60 years Hypertension Cardiovascular disease Diabetes Smoking Family history of kidney disease Māori, Pacific, South Asian origin History of Acute Kidney Injury Obese What you should not forget: Early chronic kidney disease has no signs or symptoms. Chronic kidney disease usually does not go away. Kidney disease can be treated. The earlier you know you have it, the better your chances of receiving effective treatment. Blood and urine tests are used to check for kidney disease. Kidney disease can progress to kidney failure.
Summer 2018 The Renal Messenger Page 7 ACKNOWLEDGEMENT AND THANKS TO: Special thanks to Karen Faalilo from the Property Transfer Office—not only for the continued sponsor- ship our newsletters, but also for the wonderful Christmas donation made to the Society. Rodger Curragh for the donation to the Christmas party, along with boxes of biscuits for everyone involved in the Christchurch Hospital Nephrology Department and the Society. $300 from the Murray Wakelin Family Trust—this was in addition to funeral donations previously received. This was due to the Funeral Celebrant requesting his fee to be given to the Society. To everyone who puts a little extra with their membership subscription, this is definitely not expected, but helps the Society enormously.

Summer 2018 The Renal Messenger Page 10 LIFE WITH FREDDIE Just over ten years ago my life changed for the better. I had endured 2 years on both kinds of dialysis—not very successfully either. I was given the ultimate message that I didn’t want to accept—I needed a kidney urgently. One day, whilst on the dialysis machine in the Hospital, one of the Nephrologists burst in and told the nurses not to let me leave the building! A deceased donor kidney had become available, but because I had just had lunch I had to wait until later that day. I was taken off the machine in due course and placed in Ward 14, where I was scanned. I remember going down to theatre and Professor Justin Roake was there ready. He asked how I felt and I said “scared and happy at the same time”. I was back on Ward 14 about midnight on a Friday and “Freddie” didn’t kick in until the following Wednesday. I named him that, as he is my friend. Not to say life has been a picnic for me. I have had pneumonia twice, CMV twice, and had my gallbladder removed with over 50 gallstones. One major regret I have is that Dad never found out, he would have been happy for me. I have a wonderful GP and a great team of kidney doctors. I guess what I am trying to say is “never give up hope”. Sharon Maree Skilling Special thanks to Sharon for sharing this with us all. Jo
