    •   $10 per person entry fee for Majors Division, Cash only.
    •   The teams will be built by a blind draw for both Minors and Majors.
    •   First Place of Majors will receive 70% of all funds collected
    •   Second Place of Majors will receive 30% of all funds collected

    Any participant and/or his/her guardian, in consideration for the City of Seven Hills through its volunteer
    Home Days committee providing facilities, instruction, transportation and supervision in the activity for
    which he has registered does hereby:
    1. Agree to assume all risks and responsibilities of possible damage or injury involved through participation
    in said activity. I understand I am to furnish my own insurance in case of injury.
    2. Request permission to participate in the activity with the full knowledge that the said activity could result
    in damage or injury to me.
    3. Agree to indemnify and hold harmless the City, its department and agents from liability for personal
    injury or property damage resulting from my participation in said activity.
    5. Waive the right to dispute all proper charges once he/she/they have registered and/or participated in the
