 Belk cares about our associates’ physical, mental, and financial wellbeing, and we’re committed to providing high-quality benefit programs
 to give you and your family peace of mind. Open Enrollment for 2022 benefits is happening from October 18 – November 1, 2021, and
 this is your annual opportunity to review or update your benefit choices for the coming year. In 2022, we’re pleased to offer the same great
 medical, dental, and vision coverage – plus a variety of other benefits – without increases to premium rates or deductibles. In addition,
 Belk will continue to help you save for the future by matching your contributions to your 401(k) account and Health Savings Account (HSA).

 Open Enrollment is a good opportunity to review how you’ve used your benefits in the past and learn what’s new for 2022. That way, you
 can decide if you want to make adjustments to your coverage. If your current benefit elections are working well for you, that’s great – your
 current elections will automatically carry over to 2022. You only need to take action during Open Enrollment if you want to change or add to
 your coverage, or if you want to contribute to your Health Savings Account (HSA) or Dependent Care Flexible Spending Account (FSA).

2022 Open Enrollment is October 18 – November 1, 2021.

STEPS FOR ENROLLING
You decide what’s best for you and your family in the coming year with Belk's benefit programs. By using this guide and your online resources, you’ll be
ready when it’s time to enroll.

This year, your benefit elections from 2021 will carry over to 2022 – your coverage will not change – unless you make changes through Workday during
Open Enrollment. The exceptions are Health Savings Account (HSA) and Dependent Care Flexible Spending Account (FSA) contributions; you need to
re-enroll for these programs each year. To change or enroll in benefits, especially if you want to participate in the HSA or Dependent Care FSA in
2022, you must take action in Workday.

Your Steps to Enroll
1. Review the different medical plans, insurance options and supplemental benefits in this Guide.
2. Make plans to attend our Virtual Benefits Fair to learn about your 2022 benefit choices at virtualfairhub.com/belk beginning October 4, 2021.
3. To enroll or change your benefits, log in to Workday beginning October 18 where you’ll find an Open Enrollment task in your Inbox:
  ●   Click Enroll Now and make your elections before 11:59 pm EST on November 1, 2021.
  ●   Click Submit to complete your enrollment.
  ●   Look for a confirmation statement once you have submitted your enrollment elections, and make sure to print and/or save it for your records.
Your benefit elections will take effect on January 1, 2022.

Be sure to make your elections from October 18 - November 1, 2021. If you need help enrolling, contact HR Shared Services at 1-800-588-3700 or email

If you and/or your dependents have Medicare, or will become eligible for Medicare in the next 12 months, a Federal law gives you more choices about your
prescription drug coverage. Please see page 32 for more details.

Do I Need to Enroll?
This year, most of your current benefit elections will automatically carry over to 2022. The only benefits
that won’t carry over are Health Savings Account (HSA) and Dependent Care Flexible Spending                  NEW TO BELK?

Who You Can Cover                                                               Making Changes to Your Benefits During the Year
Who you cover with your benefits is an important decision.                      The benefits you choose during Open Enrollment remain in effect for the entire

                                     HOW THE HEALTH SAVINGS ACCOUNT WORKS
                                     The health savings account (HSA) is a smart way to make your money work for you. Plus, Belk helps you save for future healthcare expenses by matching your
                                     contributions. When you contribute to your HSA, Belk will match your pre-tax contributions dollar for dollar to a maximum amount: $400 annually for individual
                                     coverage and $800 annually for family coverage. Optum, our HSA administrator, will provide you with a debit card so you can pay for health care expenses at most
                                     pharmacies and physicians' offices.

                                              WHAT HAPPENS WHEN YOU USE A NON-NETWORK PHARMACY?

                                              When you visit a non-network pharmacy to fill a prescription, you pay the full cost
                                              of your prescription upfront. Then, you must file a claim with Express Scripts for
                                              reimbursement. Once your claim is processed, you will receive reimbursement for
                                              the amount you paid minus the appropriate coinsurance.

Managing Maintenance Prescriptions

90-DAY SUPPLY
With Express Scripts, you have two ways to get up to a three-month supply of your maintenance medicines (those drugs you take regularly for ongoing
conditions). You can choose home delivery from the Express Scripts Pharmacy, or fill the prescription at any CVS pharmacy location.

If you have prescriptions with open refills through the CVS mail-order pharmacy program, your prescription will be transferred automatically to the Express
Scripts Pharmacy (except for controlled substances and compound medications).

The Express Scripts Pharmacy mail-order program has several advantages. When you use the Express Scripts Pharmacy, you are automatically using an
in-network pharmacy, so there are no claim forms to file for reimbursement. In addition, you can submit and receive your prescriptions from home.

You can receive two 30-day courtesy fills at ANY participating network pharmacy. On the third fill, you must
obtain a prescription for a 90-day supply and that prescription must be filled at a participating CVS retail
location or through the Express Scripts Pharmacy. If filled at the wrong pharmacy or wrong day’s supply,
you will be responsible for paying 100% of the prescription cost.

SPECIALTY MEDICATIONS
Accredo is an Express Scripts specialty pharmacy. Accredo provides medicine for patients with serious,
chronic conditions like cancer and hepatitis C. Accredo offers specialized teams of pharmacists, nurses

                                     DEPENDENT CARE FLEXIBLE SPENDING ACCOUNT
                                     A Dependent Care Flexible Spending Account (FSA) is a tax advantaged savings account that can be used to pay for eligible child or elder care expenses.
                                     Optum is our Dependent Care FSA administrator. When you enroll in the Dependent Care FSA, your contributions are deducted from your paycheck on
                                     a pre-tax basis, so you pay less in taxes. Note that this account does not include a debit card; instead, you can use funds in your account to reimburse
