PBA 05/11 Preserved benefit Age claim form and information leaflet Before completing this benefit application form, you are advised to read the PSS Product Disclosure Statement at www.pss.gov.au or call 1300 000 377. The attached benefit application form should be completed by preserved benefit members of the Public Sector Superannuation (PSS) Scheme who are claiming payment of their preserved benefit having both reached age 55 and retired from the workforce.
