10 St Vincent’s Hospital Melbourne Welcome messages Mr Michael Hii Pre SET Supervisor 2020 was tough... not in the least for surgical training. Whilst it is highly likely we will all be involved in continuing to manage the existing pandemic during 2021 we’ve also a real need to continue to improve surgical training as we go forward (perhaps more so). This will be year 5 of our refreshed training program and I feel we now have a solid foundation to build upon. Impressively, despite what happened in 2020 the leadership group/ Pre SET committee were able to make some key improvements from 2019 and we want to go forward in this style again. The formal education program will continue, centred on weekly lectures, GSSE preparation and the registrar ready course. I think the weekly sessions are slowly becoming part of the ‘culture’ and attendance is really improving (and becoming an expectation). The online platforms worked well in improving attendance and obviously capacity for those offsite or finishing late to also attend and so we are really keen to continue a hybrid version of teaching to allow both onsite and offsite trainees maximum exposure. For 2021 we really want to broaden SET application practice as a key part of the curriculum. In the past we’ve focused this on those trainees who were successful in obtaining an interview; but in order to increase these numbers we need to expand this to earlier stage trainees. From 2021 we’ll look to have a more inclusive application training program which attempts to develop trainees at earlier stages in their career. There are a few other big steps forward as part of the plans for 2021. I’m super excited to announce that Ms. Nicole Winter will be joining the team as the Deputy Pre SET supervisor and will help drive this. Nicole will first of all be orchestrating a biannual interview process which will allow us to touch base with the PreSET group individually and have a clearer idea of their individual goals; secondly she will directly supervise/ enact implementation of the master roster giving it a more personal - relevant - clinician touch. Nicole will therefore be instrumental in addressing two of the most often noted issues with the Pre SET positions, namely rostering and communication / engagement. Mr Lih Ming Wong, has also spearheaded the development of the St Vincent’s Surgical Academy which will also commence in 2021. This will provide a parallel pathway for more direct senior clinician involvement in training, education and mentorship and will work terrifically with the existing Pre SET curriculum. The initial focus will be on the mentorship program. Our key measures will continue to be education attendance, and increasing our numbers of successful SET applicants in all streams. Finally, I’d like to congratulate Henry Badgery on his appointment as Chief Surgical Resident and of course, also thank Alison Browning and her Pre SET committee for the amazing work they did in one of the most difficult training years in memory.
11 Pre SET Surgical Training Manual 2021 Welcome to the St Vincent’s Hospital Melbourne Pre-SET Surgical Training Program. It is a privilege to commence as your Deputy Pre-SET Supervisor in 2021 and help mould the path for future surgeons. I recently returned to Melbourne and joined St Vincent’s Hospital Melbourne as a Consultant Upper GI Surgeon. For me, it was a bit of a homecoming – I started off as a medical student at the St Vincent’s Clinical School where Prof Beswick taught me a great deal and made me realise that I am a surgeon and not a physician. I spent my junior doctor years learning what medical school could never prepare you for, then somehow successfully commenced SET training– I did not receive a first-round offer. What we will continue to build in the Pre-SET space is what I would have liked as a St Vincent’s Pre-SET resident all those years ago. I would like to congratulate Mr Hii and the previous Pre-SET committees for establishing a fantastic foundations program and continuing the St Vincent’s culture of excellence in education. For 2021, we hope to Ms Nicole Winter further integrate these elements, connect with you and Deputy Pre SET supervisor individualise your training experience. For the first time in 2021 we are introducing a biannual interview process as part of your Pre-SET Pathway. A surgical junior doctor’s career path is rapidly changing and becoming more and more competitive and complex. We are here to individually connect with you, to understand your goals, direct you to the resources you need to reach these goals, and provide a bespoke framework where you can take control of these years and navigate through to a successful SET application. As tough as 2020 was, we must embrace the positives too. 2020 has enabled us to rapidly advance new platforms to connect, communicate and collaborate. 2021 will be the year we use this to reconnect with all our junior doctors - those onsite, on external rotations, rural rotations, relieving rotations, part-time training jobs, and the list goes on. 2021 is your year to take control of your training, engage with your colleagues, seek out your mentors and succeed.

We continue to work on improving the success rates of SET applications, with further advancement in research; interview training and improved rostering to fulfil the RACS requirements. Henry Badgery has been appointed as Chief Surgical Resident. Alison and her team continue to advance the formalised teaching sessions. Surgical training at St Vincent's Hospital Melbourne continues to go from strength to strength, and we look forward to welcoming you to be part of this program. Prof Peter Choong Professor of Surgery Welcome to the Department of Surgery at St. Vincent’s Hospital. This is a major hub of the University of Melbourne surgical network. The Department of Surgery offers higher degree research opportunities for many specialty disciplines including Honours, Masters and Doctorate level research. The Melbourne Medical School now offers guidance about the Academic Clinician pathway and how this can be developed through your resident and registrar journey. The University of Melbourne Department of Surgery is a broad network of surgical departments situated at 6 major sites around Melbourne, namely St. Vincent’s Hospital, Royal Eye and Ear Hospital, Royal Melbourne Hospital, Western and Sunshine Hospital, Austin and Repatriation Hospital, Northern Hospital. These departments drive research that covers the majority of surgical specialties. Themes include but are not limited to bioengineering and medical devices, tissue engineering, basic biology, cancer, outcomes and prognostic factors, epidemiology and hospital services research, surgical technique and health economics. Research in the Department of Surgery emphasises a collaborative multidisciplinary approach that combines quantitative and qualitative aspects. Collaborative cross campus research is encouraged and can be utilised to meet SET training requirements. 13 Pre SET Surgical Training Manual 2021 This year has been a challenging year across the world, with significant changes to the way we all live and work secondary to the COVID-19 pandemic. Despite this, the St Vincent’s Melbourne Pre-SET Surgical Society (SVM PSSS) has had a successful fourth year, with the consolidation and introduction of a number of programs. Education has become a strength of our organisation this year. With the move to online platforms, weekly HMO teaching, GSSE course and medical student teaching have all had outstanding years, with excellent attendance and programs. We have introduced an interview practice program that has allowed our more senior pre-SET doctors the ability to connect and practice with each other. We were also able to adjust the Registrar Ready program to the beginning of the year, allowing us to provide this program at a time where it was of greatest utility. All committee members and contributors to education have generously offered their time to support these initiatives, and I am greatly appreciative of their Dr Alison Browning Chief Surgical Resident (2020) efforts. The work of this committee would not be possible without a committed team of junior doctors and supervision from Mr Hii, our pre-SET supervisor. I would like to acknowledge everyone’s efforts, resilience and flexibility in this challenging year. As my time at St Vincent’s comes to a close, I reflect on the opportunities for leadership and career development this hospital has afforded me. It has been a privilege to take on the role of Chief Resident and to act as an advocate for the pre-SET doctors, and I look forward to seeing the progression of the pre-SET program into the future. I wish you all the best in your coming careers, whatever they may hold, and look forward to working with you in the future.
14 St Vincent’s Hospital Melbourne Dr Henry Badgery Chief Surgical Resident (2021) As the incoming chief surgical resident, I would like to warmly welcome all those coming to work with us at St Vincent’s Hospital Melbourne in 2021, both new and returning. The St Vincent’s Melbourne PreSET Surgical Society (SVM PSSS) is an increasingly important part of the fabric of St Vincent’s and is unique within Victoria. It is a great privilege to be leading the committee in 2021. The goals of the SVM PSSS are to support pre-vocational doctors in being accepted onto their desired surgical training program, to nurture the culture and camaraderie among St Vincent’s doctors and to provide support, education, training and professional guidance. Acceptance onto a surgical training program is an increasingly competitive process and surgical residents will commonly spend many years as Pre-SET doctors. It is vital that these years are as rich and productive as possible. The SVM PSSS has been proactive in establishing programs to aid professional development of our pre-vocational surgical doctors through education, research. In 2021, we will be introducing the pre-SET pathway, where all junior surgical staff will be provided with one-on- one consultation with senior surgical staff to ensure they are on track with their SET applications plus providing an opportunity to discuss ways in which their application can be improved. The SVM PSSS is always looking for ways to improve the experience and opportunities provided to our junior doctors. We always welcome feedback and fresh ideas to improve our programs. Furthermore, I encourage you to get in touch with myself or anyone on the committee should you wish to raise any issues or concerns. 16 St Vincent’s Hospital Melbourne St Vincent’s Surgical Academy T he St Vincent's Academy of Surgeons commences in 2021. Developed by Mr Lih Ming Wong, it brings together a group of like minded surgeons across multiple specialities, interested in teaching, research and mentorship. The academy aims to assist with coordination of surgical education across the campus, with particular focus on junior surgical staff, interns and medical students. This will involve sub-specialty tutorials organised through the Pre-SET society. There will also be an emphasis on surgical career guidance and development of appropriate mentoring groups, across the spectrum from student to surgeon. This will commence with a careers night, tentatively planned for Feb 25th, 2021. For anyone interested in any form of surgical research, the academy will have surgeons who can guide and encourage your endeavours. 25 Pre SET Surgical Training Manual 2021 1 Pre-SET Wednesday Weekly Education Series The weekly HMO teaching sessions aim to educate pre-SET trainees across various surgical specialities, anaesthetics and perioperative medicine to foster well rounded, knowledgeable pre-SET trainees. Topics will cover the investigation and management of common, as well as rare but serious surgical conditions not to be missed. The sessions are run by St Vincent’s consultants and fellows in a combination of didactic and interactive case-based discussion format. The 2021 teaching sessions will be held in-person where appropriate as well as through Zoom, an online platform to ensure those off-site are able to attend. The weekly HMO teaching sessions aim to prepare residents to become competent registrars and it is an expectation that our pre-SET surgical HMOs attend as many sessions as able. 27 Pre SET Surgical Training Manual 2021 2 Surgical Essentials for Residents Program The Surgical Essentials for Residents (SERP) Program is a series of interactive practical workshops focusing on fundamental surgical skills across a variety of surgical specialties. Topics include laparoscopy and chest tube insertion. Those who attend a minimum of 4 x 1.5 hour sessions will be provided a formal certificate of completion. Due to limited space in labs and the need to provide practical resources, registration and commitment to this program is required and there exists a waiting list for each session. 28 St Vincent’s Hospital Melbourne 3 ‘Registrar Ready’ Program Following the success of the 2020 program, Registrar Ready Day 2021 will continue to run as a single all day event on Saturday, February 6th. The purpose of this program is to offer new registrars and residents that may be stepping up into more senior roles the opportunity to practise key clinical skills and upskill in non-technical domains. All sessions are taught by senior SET registrars, fellows, and/or consultants. The program for 2021 is as follows: • How to Manage a Team • Suturing • Surgical Dressings • Surgical Draping • Perioperative Management of the Surgical Patient • Informed Consent • Keeping your head above water Frequency: Once yearly Location: St Vincent’s Clinical School, 23 Regent Street, Fitzroy Time: 08:30-17:30 4 General Surgical Sciences Exam Preparation Course The Generic Surgical Sciences Exam (GSSE) is a prerequisite exam for all applications to SET training. Its primary components consist of foundational knowledge in anatomy, physiology and pathology. With few courses available to guide study, pre-SET doctors have historically found the physiology and pathology components of the exam most challenging to prepare for. The St Vincent’s Pre-SET Surgical Society has designed a two-day intensive education program targeted at preparing candidates for these most difficult components of the exam. Lectures are focused on high-yield exam topics, and taught by experienced lecturers who are familiar with the GSSE exam structure and style of questions. Over the years of running this course, we have refined our course material and design, as well as attracted participants from all around Australia. In recognition of its value, our course is now in its early stages of attaining Royal Australasian College of Surgeons accreditation. 2020 has been the 6th consecutive year running this course at St Vincent’s Hospital. This year, with the unique challenges presented by the Covid-19 pandemic, we have had to think outside the box and find new and innovative ways to continue to deliver high-yield exam material in an interactive and engaging way. This has led to the successful online delivery of the course in 2020 enabling pre-SET doctors from interstate as well as overseas to attend. In 2021, we will continue to deliver both face-to-face and online attendance options. This course is aimed at pre-SET doctors sitting the upcoming GSSE. However, given the importance of these foundational topics in all specialty examinations, we strongly encourage medical students, interns, and residents pursuing critical care and medical specialties to also attend. 29 Pre SET Surgical Training Manual 2021 5 Wet Skills Laboratory Over the last 4 years, surgical trainees at St Vincent’s Hospital have been invited to participate in live-animal, advanced laparoscopic workshops. These skills labs have been run at the University of Melbourne. A select group of 6 trainees participate in this small group, intensive workshop in laparoscopic surgery. The program will run twice in 2021 with workshops tentatively planned for 27th March and 31st July. In an era of increasingly complex surgery, the workshop has provided trainees a vital opportunity to learn and practice advanced laparoscopic skills prior to putting them into practice in the real world. Despite advances in technology, laparoscopic training simulators, even high-cost “virtual-reality” trainers, do not sufficiently simulate operating on real tissues. This year we have run four workshops, and this has given our trainees the unprecedented opportunity to acquire advanced laparoscopic skills whilst operating on real, living tissue in a porcine model. They have had the opportunity to practice their laparoscopic skills including dissection, suturing and stapling, and perform operations such as laparoscopic cholecystectomy, small bowel resection and anterior resection. Mr Eugene Ong Colorectal Surgeon On each course, 2 trainees have been allocated per station, and each station has had a proctor. This has meant our trainees have an intensive workshop and have been able to accomplish and learn more than on other comparable courses. In addition to acquiring technical skills, thanks to our industry partners, the trainees have been exposed to the latest surgical devices, including advanced electrosurgical and stapling devices. The industry representatives have also attended these labs to educate the trainees in the theory and practical usage of these devices. The workshops have been extremely well received by the participants and will continue to be a highlight in the surgical training St Vincent’s Hospital. The workshops have been generously sponsored by Medtronic/Covidien, and would not be possible if it were not for their ongoing support and enthusiasm for surgical training.
30 St Vincent’s Hospital Melbourne 6 SET Semi-Structured Interview Practice The St Vincent’s Pre-SET society provides semi-structured SET interview practice for applicants Interested trainees who are intending to apply for SET in 2021 will have the opportunity to register to be part of this program. Fortnightly interview questions and practise partnerships will be e-mailed to participants, to aid with regular practise and rotational experience and exposure to different craft groups and skill sets. Formal interview practice evenings will also be held, closer to the interview dates. We will ask surgeons from St Vincent’s Hospital to participate. The aim will be to practice an interview in a semi-authentic environment, ideally with consultants who are not familiar to the trainee. We also encourage surgical residents to participate in these practice evenings to provide feedback and to help familiarise themselves with the interview process. We will continue to develop a question bank prior to this to maximise the accuracy of the simulation. Further details will be provided closer to the date. 7 Surgical Olympics 2021 will mark the inaugural Surgical Olympics at St Vincent’s Hospital Melbourne. Inspired by programs such as the Gold Scalpel, the Surgical Olympics will be an opportunity for pre-SET trainees to demonstrate the practical skills that they have acquired and compete with each other for a prize. This event will take place in December. Further information will be provided in due course. 8 Surgical Panel The annual Surgical Panel is an information evening held each year with invited speakers from all levels of surgical training including Unaccredited Registrars, SET trainees, Fellows and Consultants. The aim of this session is to discuss what a career in Surgery looks like at each level, some of the benefits and challenges throughout a surgical career and highlight key components of the SET application process. The night ends with an informal question and answer session. Medical students, interns and Pre-SET trainees are encouraged to attend this valuable session. 32 St Vincent’s Hospital Melbourne T he Doctor of Medicine course at St Vincent’s Clinical School, The University of Melbourne is a 4-year graduate program. Students are placed in the hospital setting full-time for the last three years of the course with a one-day placement in year 1. The Clinical School utilizes various teaching platforms and we encourage teaching from consultants to registrar/residents. It remains the premier teaching hospital at the Melbourne Medical School, The University of Melbourne. There will be opportunities in 2021 for a select number of HMO surgical residents/registrars to further their skills as tutors and teachers. The Clinical School has an active surgical student’s society and through the student leadership group, Mr Naveed Alam (surgical mentor) and Mr Michael Hii – pre-SET supervisor they run annually an innovative surgical education program which sits along with the core teaching programs. The formation of the resident’s society along with the appointment of the chief surgical resident has allowed for strong links between the student’s and resident’s society. The program in 2019 was well received by the medical students with the support of the Student Teaching Coordinator on the resident’s committee. Resident and registrars will assist in teaching in the following • surgical lab sessions and simulation programs – resident and registrar led • seminar program – registrar involvement with core presentations overseen by consultant content experts • program support – skills training support as required from the surgical students' society In addition to below, a buddy mentor system will be rolled out to support senior students with an interest in surgery. 33 Pre SET Surgical Training Manual 2021 Student Teaching Coordinator The Pre-SET Surgical Society has provided a supplementary teaching program to Melbourne University Medical Students at St Vincent’s Hospital since 2018. We work closely with the St Vincent’s Clinical School and the St Vincent’s The teaching program faced great challenges in Clinical School Surgical Student Society. We 2020. 34 St Vincent’s Hospital Melbourne PreSET Pathway In 2021, we have introduced a compulsory biannual interview program as checkpoints in your pathway. This begins with Pre-SET Entry Interview for all HMO 3 surgical residents and all unaccredited general surgery registrars. This will be an opportunity for you to tell us a bit about yourself, where you are along your path to SET applications, any issues or concerns you would like assistance with, and any suggestions for the year and going forward. In preparation for this interview, an online form is required before your session. The aim of this program is to ensure all St Vincent’s pre-SET doctors have a clear plan in preparation for SET applications in meeting prerequisites, identifying and contacting referees, improving CVs and preparing for interviews. A mid-year Pre-SET Redirect Interview will be offered to all trainees and conducted by request from you. A junior doctor’s career path is becoming more complex and competitive. 36 St Vincent’s Hospital Melbourne Research has always been at the forefront of improving patient outcomes, yet it can be daunting to think about venturing down this path. When and where should you begin, Research how much time and effort can you spare, will your work become published? What begins to sound like an insurmountable task should rather be an exciting journey. As Coordinator the Pre-SET Committee Research Officer my role for you is two-fold: 1) I act as a point of call to help answer general queries and direct you towards the appropriate contacts to get involved in research 2) I act as an advocate for surgical research and facilitate events and opportunities throughout the year. Many of you reading this may choose a more solo attack to get involved in research, and so with the wise words of my predecessor, the best place to start is by emailing consultants in your desired surgical field and speaking to registrars on your surgical rotations. Research The unit runs an extensive prospective database which currently contains short, medium and long-term information for over one thousand patients. There are numerous Opportunities possibilities within this for rapid turnover projects or more extensive articles. Alongside this, there are numerous prospective trials currently running including randomised controlled trials which are open for collaboration from the junior medical staff. The Upper GI and HPB unit employ a full-time research coordinator to assist in running projects, ethics applications, data collection, other logistics and writing. There will be at least fortnightly research meetings held Monday at 12:30pm. This meeting DEPARTMENT OF HEPATOBILIARY AND is chaired by Mr Michael Hii and attended by most of the consulting surgeons, as well UPPER GI SURGERY as our full-time research assistant, Lynn Chong, one Masters Student and 3 MDRP students from the University of Melbourne. The research group welcomes and encourages contributions from junior medical staff and offers a wide range of projects, ranging from case reports all the way through to randomised trials. For junior medical staff looking to complete higher degrees, appropriate supervision can be arranged (along with part-time work) for those interested in foregut research. 37 Pre SET Surgical Training Manual 2021 Department of Orthopaedics The Department of Orthopaedics is the oldest dedicated orthopaedic department in Australia and is an academic division of the University of Melbourne Department of Surgery. It has a strong tradition in academic and professional leadership with 3 of its previous heads of department having higher degrees (Tom King MD, Harry Crock MD, Peter Choong MD), 4 surgeons being National Director of Orthopaedic Training (Brendan Dooley, Jonathan Rush, Kevin King, Peter Choong) and 7 previous members of staff being President of the Australian Orthopaedic Association (Tom King, Kevin King, Brendan Dooley, Jonathan Rush, John Batten, John Owen, Peter Choong). The current Chair of the AOA Research Committee and Co-Chair of the Scientific Advisory Committee of the Victorian Orthopaedic Trust is Professor Peter Choong. The Department of Orthopaedics pursues research that is drawn together by the Advanced Limb Reconstruction Programme. This includes clinical and hard science (basic and engineering) research. The clinical research themes are divided into (i) Orthopaedic Oncology which leverages a bone and soft tissue database to investigate the behaviour and outcomes of a wide variety of tumours as well as the outcomes after modern multidisciplinary treatment; (ii) Arthritis Surgery research which leverages a unique high fidelity joint replacement registry to explore the drivers of patient-related outcomes, health economics of joint replacement surgery and the effectiveness and efficiency of clinical and institutional processes related to joint replacement surgery. The basic science and engineering science themes are divided into (i) Tissue engineering of cartilage and skeletal muscle to investigate novel ways to treat osteochondral and neuromuscular injuries using smart materials, stem cells and 3D bioprinting (ii) Advanced fabrication of implants using latest technologies in 3D manufacturing in collaboration with the School of Aerospace and Advanced Manufacturing Engineering RMIT, (iii) Development of Limb Robotics in conjunction with the Melbourne School of Engineering which is focused on developing space-age neuroprostheses, (iv) Development of smart devices for hand delivery of bio-scaffolds and cells at the time of surgery in conjunction with the Intelligent Polymer Research Institute, University of Wollongong, (v) Development of new methods for rapid expansion of cell cultures using nanotechnology in conjunction with Swinburne University of Technology, (vi) Development of Bone Substitutes for reconstructing skeletal defects in conjunction with the Queensland University of Technology, (vii) Development of robotic-assisted surgery in orthopaedics and orthopaedic oncology in collaboration with the Stryker Corporation. The Orthopaedic Laboratories on 3rd Floor Healey Wing is the focus for the basic research themes. The newly inaugurated Advanced Biofabrication Facility on the 1st Floor, Department of Surgery, Clinical Sciences Building unites the stakeholders of the Advanced Limb Reconstruction Programme via platforms of technology that include bioreactors, molecular biology, stem cell biology, advanced bioprinting. The NHMRC Centre for Research Excellence into Joint Replacement Surgery is situated on the 2nd Floor, Department of Surgery, Clinical Sciences Building and brings together researchers in the field of arthritis surgery research. There are ample opportunities for undergraduate students (Biomedicine, Science), postgraduate students (Doctor of Medicine), residents, registrars and surgeons to participate in vibrant and highly successful research under the supervision and mentorship of driven and professional researchers.
