Two ways to reach a healthy weight You’re receiving this booklet if your Blue Care Network Qualification Form shows that your body mass index is 30 or more. To earn enhanced benefits in Healthy Blue Living, sign up for our Weight Watchers or Walkingspree program in the first 120 days of your benefit year. We’ll cover the cost as long as you participate, although some restrictions apply. From the date you enroll, you’ll need to participate for the rest of your benefit year or until we receive an updated qualification form showing your BMI below 30. If you or your covered spouse chooses not to participate, everyone on your contract will have standard benefits for the rest of your benefit year. You’ll have the same great Healthy Blue Living plan, but with higher out-of-pocket costs. The software isn’t compatible with Windows RT®, Windows ME®, Windows 2000®, Windows 98®, Windows XP® or older Windows systems. 4
Option 1: Weight Watchers For more than 40 years, Weight Watchers has helped millions of people around the world lose weight. Blue Care Network offers this program to members who have a BMI of 30 or more to help them become healthier and work toward earning enhanced benefits. What do I need to do to meet the Weight Watchers requirement for enhanced Healthy Blue Living benefits? Attend 11 out of 13 weekly meetings per session. Participate until your BMI drops below 30 and we have an updated qualification form from your doctor. Automatic renewal If you were in Healthy Blue Living’s Weight Watchers program in your previous benefit year and your qualification form shows that your BMI is still 30 or more, we’ll automatically re-enroll you. There’s no need to re-select the program. This applies to members who participated in Weight Watchers for the whole benefit year and met the requirements. We’ll start the re-enrollment process as soon as we receive your new qualification form. BCN won’t reimburse you if you attend before we re‑enroll you. How do I enroll in Weight Watchers? The home or work ZIP code you provide determines which Weight Watchers group you’ll join. Enrollment options vary slightly based on the group, but both offer telephone registration.
