Reviewing and, where necessary, revising the client treatment plan to address ongoing problems k. Developing level of care change, discharge, and relapse prevention plans with clients to ensure movement to the appropriate levels of care l. When necessary, reporting to, and contacting, coordinating, and consulting with referral sources, (i.e., courts, schools, employers, DCPP, etc.) m. Participating in treatment team meetings and attending supervision session n. Providing active case consultation o. Work hours are flexible but include late afternoon and evening hours. Will provide trauma-informed individual supportive counseling sessions and facilitate weekly support and education group(s) for survivors of domestic violence. Will provide occasional presentations on domestic violence and 180 services to community groups and professionals.
