Harnessing choice architecture to improve medical care
19 Apr 2021
Introduction People live busy complex lives where most decisions need to be made quickly.
As a consequence, people tend to prefer simple rather than expanded choice sets, easy
alternatives that require no complex tradeoffs and benign options that avoid major moral
quandaries
Choice architecture in physician-patient communication: a mixed-
methods assessments of physicians competency
19 Apr 2021
Background Clinicians’ use of choice architecture, or how they present options,
systematically influences the choices made by patients and their surrogate decision makers.
However, clinicians may incompletely understand this influence. Objective To assess
physicians’ abilities to predict how common choice frames influence people’s choices
Reporting incidents involving the use of advanced medical technologies
by nurses in home care: a cross-sectional survey and an analysis of
registration data
19 Apr 2021
Background Advanced medical technologies (AMTs), such as respiratory support or suction
devices, are increasingly used in home settings and incidents may well result in patient harm.
Information about risks and incidents can contribute to improved patient safety, provided that
those are reported and analysed systematically
Variation in tonsillectomy cost and revisit rates: analysis of
administrative and billing data from US childrens hospitals
19 Apr 2021
Background Tonsillectomy is one of the most common and cumulatively expensive surgical
procedures in children. We determined if substantial variation in resource use, as measured
by standardised costs, exists across hospitals for performing tonsillectomy and if higher
resource use is associated with better quality of care, as measured by revisits to hospital

Promise and perils of patient decision aids for reducing low-value care
19 Apr 2021
Recognition of excessive waste and harm in health systems has prompted efforts to reduce
low-value care, which has been defined as ‘use of an intervention where evidence suggests it
confers no or very little benefit on patients, or risk of harm exceeds likely benefit, or, more
broadly, the added costs of the intervention do not provide proportional added benefits’
Its time to consider national culture when designing team training
initiatives in healthcare
19 Apr 2021
Introduction The cultivation of a workplace culture that promotes patient safety is critical to
the delivery of healthcare around the world. Since the 1999 Institution of Medicine report To
Err is Human there has been growing focus on healthcare safety.1 Breakdowns in
communication and teamwork are often cited as root causes of preventable medical error in
hospital and malpractice claims data

Digital imaging for reading of direct rapid antibiotic susceptibility tests
from positive blood cultures
25 Apr 2021
Abstract Delaying effective antibiotic therapy is a major cause of sepsis-associated mortality.
The EUCAST rapid antibiotic susceptibility test (RAST) is performed from positive blood
cultures to provide rapid results. Disc diffusion tests inoculated with positive blood culture
broth are read at 4, 6, and 8 h and interpreted against species and time-specific criteria
Clinical characteristics and drug susceptibility patterns of
Corynebacterium species in bacteremic patients with hematological
disorders
24 Apr 2021
Abstract The aim of this study was to clarify the clinical and microbiological characteristics of
Corynebacterium bacteremia in hematological patients. We retrospectively reviewed the
medical records of patients with Corynebacterium bacteremia from April 2013 to June 2018
Hsp16.3 of mycobacterium tuberculosis in exosomes as a biomarker of
tuberculosis
24 Apr 2021
Abstract Exosomes are selectively packaged cell-derived vesicles that contain a rich cargo of
nucleic acids and proteins. The small heat shock protein, Hsp16.3, is an important capsule
protein produced by Mycobacterium tuberculosis (MTB). Exploring the distribution of Hsp16.3
in exosomes is valuable to tuberculosis biomarker development. Our results showed that

Haemophilus influenzae one day in Denmark: prevalence, circulating
clones, and dismal resistance to aminopenicillins
23 Apr 2021
Abstract Haemophilus influenzae is a common cause of mucosal infections that warrants
accurate surveillance. We aimed to assess the prevalence of the species in clinical
specimens, and characterise population structure and resistance to aminopenicillins by whole
genome sequencing
Seasonal variations in blood culture numbers and time to positivity and
potential impact of reducing incubation periods
23 Apr 2021
Abstract Patients with viral respiratory infections often present symptoms compatible with
bloodstream infections. Consequently, the winter period commonly associated with epidemic
respiratory illnesses shows an increase in the number of blood cultures (BC) and to
occasional saturation of automated BC systems
Comparative activity of plazomicin against extended-spectrum
cephalosporin-resistant Escherichia coli clinical isolates (2012–2017) in
relation to phylogenetic background, sequence type 131 subclones, bla
CTX-M genotype, and resistance to comparator agents
23 Apr 2021
Abstract Extended-spectrum cephalosporin-resistant Escherichia coli (ESCREC) are a
growing threat. Leading ESCREC lineages include sequence type ST131, especially its
(blaCTX-M-15-associated) H30Rx subclone and (blaCTX-M-27-associated) C1-M27 subset
within the H30R1 subclone
Prevalence and characteristics of persistent symptoms after non-severe
COVID-19: a prospective cohort study
23 Apr 2021
Abstract We performed a prospective cohort study of 311 outpatients with non-severe
COVID-19 (187 women, median age 39 years). Of the 214 (68.8%) who completed the 6-
week follow-up questionnaire, 115 (53.7%) had recovered. Others mostly reported dyspnea
(n = 86, 40.2%), weight loss (n = 83, 38.8%), sleep disorders (n = 68, 31.8%), and anxiety (n
= 56, 26.2%)
Comparison of teicoplanin versus vancomycin in combination with
piperacillin-tazobactam or meropenem for the risk of acute kidney injury
22 Apr 2021
Abstract We compared the rates of acute kidney injury (AKI), 7-day and 30-day mortalities,
and resolution of AKI at discharge in combination therapies involving either teicoplanin (TEI)
or vancomycin (VAN) with piperacillin-tazobactam (TZP) or meropenem (MER)
Semi-mechanistic PK/PD modelling of meropenem and sulbactam
combination against carbapenem-resistant strains of Acinetobacter
baumannii
22 Apr 2021
Abstract Due to limited treatment options for carbapenem-resistant Acinetobacter baumannii
(CR-AB) infections, antibiotic combinations are commonly used. In this study, we explored
the potential efficacy of meropenem-sulbactam combination (MEM/SUL) against CR-AB. The
checkerboard method was used to screen for synergistic activity of MEM/SUL against 50
clinical CR-AB isolates
Application of metagenomic next-generation sequencing in the
diagnosis and treatment guidance of Pneumocystis jirovecii pneumonia
in renal transplant recipients
21 Apr 2021
Abstract Pneumocystis jirovecii pneumonia (PJP) is difficult to be diagnosed, so this study
explored if PJP could be diagnosed by metagenomic next-generation sequencing (mNGS)
and if mNGS could guide the therapy of PJP. mNGS was successfully diagnosed 13 out of
14 PJP recipients with 11 through peripheral blood samples, verified by PCR. Ten non-PJP
recipients were enrolled as the control group
