Sports climbing in therapy Climbing addresses - social - mental and - physical well-being = enormous therapeutic potential!
Multiple Sclerosis (MS) Facts • Progressive, chronic, neurological disease • Not curable yet • Mainly young adults (onset between 20 and 40 years) • Worldwide 2.5 million, in Germany > 220.000 • People with MS are less physically active than healthy adults (Motl et al. 2005)
Study hypotheses Pilot study 2006 + RCT 2009 (TKMS) Assessment of the impact of sports climbing on psychosocial factors and motor function in Multiple Sclerosis (MS) In 27 MS-patients sports climbing (top rope, 2 h/week, 6 months) a) can reduce fatigue b) can increase quality of life and self-efficacy c) can improve balance and walking-ability IG (n=12) Treatment sports climbing pre-test 6 months post-test CG (n=15) no additional treatment RCT = randomised controlled trial IG/CG = Intervention/Controlgroup
Results TKMS Intensity of training • all participants finished the climbing- selfperceifed physical activity programm after 6 month (drop-out 0%) = „moderate-exhausting“ • selfperceifed physical activity (mean 14,3 ±0,6) • sign. decrease of fatigue (WEIMuS p=0.011) Borg-Skala Name: Fatigue (n=24) Datum: below cut off-value 6 easy p = 0.011 7 sehr, sehr leicht after intervention 8 9 sehr leicht 10 36.0 11 recht leicht 26.0 27.0 12 13 etwas anstrengender 17.5 14 15 anstrengend 16 17 sehr anstrengend 18 19 sehr, sehr anstrengend exhausting 20 cut off-value: > 32 = fatigue Kern, 2014

Inclusive Climbing: Conclusion First evidence for climbing effects indoor in patients with MS MS =„disease of 1000 faces“ => heterogenity Sports climbing is feasable, individual, highly motivating and a good additional training TKMS: Significant improvement in fatigue, cognitive function, self-efficacy and balance strong social network climbing outdoor? Requirements Outdoor Outdoor possible, but… … weelchair accessable area … elaborate transport … special trained trainer … more trainers … adjusted terrain / rock / inclination … special needs for special disorders: MS weather condition (Uthoff-Phenomenia) restrooms (dysfunctional bladder) over/underestimating own skills
