1.7 Intervention insurance

With our ‘Interventieverzekering’ intervention
insurance, you can avoid unexpected costs for
interventions aimed at preventing or limiting
sickness absence and getting employees on long-
term sick leave back to work faster. Some
examples are:

All reimbursements made under this insurance are
eligible as an exemption under the work-related
expenses scheme. With our contracted healthcare
providers, you can be sure that your employees
will get the help they need quickly and receive
high-quality healthcare. But that is not all the ‘CZ
Interventieverzekering’ covers all your employees,
even if they are not insured with CZ. For 2022, the
annual premium is € 23.75 per employee.

1.8 Additional discount based on payment

The group health insurance we offer your company includes general insurance, additional insurance packages
and dental insurance policies. However, you can rightfully expect more from a health insurer that goes further.

2.1    General insurance

At CZ your employees can choose from three general insurance policies: two in-kind insurance policies and a
refund insurance policy. We accept everyone without medical selection.

2.2    Additional insurance packages

Your employees can take out additional insurance for healthcare that is not covered by the general insurance
policy. Our group additional insurance packages provide reimbursement for things such as physiotherapy,
alternative treatment methods, contraceptives and sports medicine-related advice. The higher the premium,
the higher the reimbursements.

CZ also has additional insurance packages that have been put together especially to suit your employees’
stage of life.

Your employees interested in having good dental cover can opt to take out an additional dental insurance
package that provides more comprehensive reimbursements.

This premium offer is based on the compulsory deductible of €385,- and an additional voluntary deductible of €0,- for the
general insurance policies. All mentioned premiums and the contents of the packages are valid until December 31, 2022.

For the ‘Uitgebreide Tandarts Collectief’ dental insurance package, we will ask your employee to provide a
dentist’s statement, except where your employee had equivalent dental cover with his or her previous insurer.
For orthodontic care, a waiting period of one year applies under both dental packages.

2.3    Healthcare in Belgium and Germany

We want employees to enjoy trouble-free access to healthcare across the border region. With this in mind, CZ

policyholders and their family members to get healthcare in Belgium and Germany.

Good healthcare is important, which is why we support your employees in their healthcare choices. We will
explain the various options and their differences, and advise them on the healthcare that is right for their
situation. And your employees are always welcome to turn to us with their health-related questions.

This implementation form is part of the ‘Agreement regarding group health insurance’
entered into by CZ and the INTERMEDIARY (hereinafter referred to as: ‘the Agreement’).
In entering into the Agreement, the INTERMEDIARY is representing the interests of
employees of the INTERMEDIARY's members. In particular, the INTERMEDIARY has
negotiated the option for its members to take part in the Agreement after completing and
submitting this implementation form, so that the participating members’ employees can
benefit from the group advantages agreed by CZ and the INTERMEDIARY.

In order to participate in the Agreement as a member of the INTERMEDIARY, you need to
complete this implementation form, sign it and send it to the INTERMEDIARY. The
provisions of the Agreement will then apply to you in their entirety. You can request a copy
of the Agreement from the INTERMEDIARY. In signing this implementation form, you
declare that you have read the Agreement and that you agree to its contents.

If you have 1-4 employees, you will be registered under the group entity number of the
SME policy.
If you have 5-149 employees, you can register as soon as you have received the group
entity number.

The details you provide on this sheet must be accurate and truthful. If, at any time, it
emerges that you have entered any details inaccurately, whether intentionally or otherwise,
CZ will be entitled to end your participation in the Agreement, in writing and with immediate
effect, and to recover any losses or damage from you.’
