EMERGENCY PREPAREDNESS OUTBREAK PLAN The Emergency Preparedness Outbreak Plan is a section of the overall SCC Infection Control Policy and Framework as outlined below. This p Emergency Preparedness Outbreak Plan will activate when there is clear evidence of a threat to consumer/resident, employee’s and extended stakeholders. 1. BACKGROUND What is coronavirus (COVID-19) Coronaviruses are a group of viruses belonging to the family of Coronaviridae, which infect both animals and humans. Human coronaviruses can cause mild disease similar to a common cold, while others cause more severe disease (such as MERS - Middle East Respiratory Syndrome and SARS – Severe Acute Respiratory Syndrome). A new coronavirus that previously has not been identified in humans emerged in Wuhan, China in December 2019. Symptoms Symptoms can range from mild illness to pneumonia. Some people will recover easily, and others may get very sick very quickly. People with coronavirus may experience: • fever • flu-like symptoms such as coughing, sore throat and fatigue • shortness of breath All documents are to be treated as SCC confidential. EMERGENCY PREPAREDNESS OUTBREAK PLAN In more severe cases, infection can cause pneumonia, severe acute respiratory syndrome and sometimes death. Who is most at risk of a serious illness? Some people who are infected may not get sick at all, some will get mild symptoms from which they will recover easily, and others may become very ill, very quickly. From previous experience with other coronaviruses, the people at most risk of serious infection are: • people with compromised immune systems, such as people with cancer • Elderly people • Aboriginal and Torres Strait Islander people • Very young children and babies, and • People with diagnosed chronic medical conditions. Most cases of coronavirus (COVID-19) stated in Wuhan City in Hubei Province, China and have now spread throughout the world. Health authorities have confirmed cases of coronavirus in Australia and update their website daily with the latest information and outbreak alert. See link below. https://www.health.gov.au/health-topics/novel-coronavirus-2019-ncov#what-is-coronavirus- covid19 If you do not have internet access to the Australian Department Website, it is suggested to watch the news as they provide updates from the department. How it spreads There is evidence that the virus spreads from person-to-person. The virus is most likely spread through: • Close contact with an infectious person • Contact with droplets from an infected person’s cough or sneeze • Touching objects or surfaces (like doorknobs or tables) that have cough or sneeze droplets from an infected person, and then touching your mouth or face All documents are to be treated as SCC confidential. EMERGENCY PREPAREDNESS OUTBREAK PLAN Prevention Surgical masks in the community are only helpful in preventing people who have coronavirus disease from spreading it to others. If you are well, you do not need to wear a surgical mask as there is little evidence supporting the widespread use of surgical masks in healthy people to prevent transmission in public. Everyone should practice good hygiene to protect against infections. Good hygiene includes: • Washing your hands often with soap and water • Using a tissue and cover your mouth when you cough or sneeze with your arm or elbow, not your hands • Avoiding close contact with others, such as touching There are additional resources attached for more advice on how to use additional protective measures against coronavirus from the World Health Organization, The Australian Health Department uploaded to the SCC intranet. Are there any restrictions on health and residential aged care workers attending work? Health and residential aged care workers with direct contact with consumers either in residential aged care or Home Care are more likely to be exposed to COVID-19. Early detection of COVID-19 in health care workers is important to protect the vulnerable patients they treat in clinical practice. Health care workers should not go to work if they have: • symptoms of an influenza like illness, as per usual practice; OR • returned from any country overseas and have fever OR symptoms of acute respiratory infection (e.g. shortness of breath, cough, sore throat). These health care workers should be isolated and tested for COVID-19; OR • have been in close contact with a confirmed case of COVID-19 (either in the community or at work) within the past 14 days and have not been protected with appropriate PPE; or • returned from a higher risk country within the past 14 days. Go to www.health.gov.au/covid19-travellers for the list of at-risk countries and quarantine requirements. If employee’s have a confirmed diagnosis of the COVID-19 they: • Must not come to work • Seek medical advice and inform their site of the diagnosis • Isolate themselves on the GP or department’s directive to prevent further spreading to other people. EMERGENCY PREPAREDNESS OUTBREAK PLAN Diagnosis (consumers and staff) If an employee or consumer become unwell and think they might have symptoms of coronavirus, they need to seek medical advice. The nurse in the Aged Care Facility needs to contact the GP immediately and follow the suspected outbreak plan for any respiratory outbreak. Staff need to use the ISBAR tool to inform the GP of the consumers symptoms, travel history and any recent close contact with someone who has coronavirus or might have travelled with or near someone with the virus. If you are a staff employee and your need to leave home to see your doctor: • Call the GP first to inform them you are coming • Advise the GP of your current symptoms and; • Wear a surgical mask (if you have one) to protect others. If an employee or consumers have serious symptoms such as difficulty breathing, call 000 immediately for urgent medical help. There is no treatment for coronavirus at this stage, but medical care can treat most of the symptoms. Antibiotics do not work on viruses and there are several research studies trying to create a vaccine. Visitor and vendor information: Ongoing promoting and communication for awareness on COVID-19 2. ACTION PLAN Southern Cross Care will follow the COVID-19 Flowchart for Management in RACF in Australia – Appendix 1 from the coronavirus-covid-19-guidelines-for-outbreaks-in- residential-care-facilities, issued on 13 March 2020. EMERGENCY PREPAREDNESS OUTBREAK PLAN 3. How to isolate: In the Aged Care Facility: Staff need to ensure the Contact and Droplet precautions are adhered to and isolation procedures are applied. Action precautionary outbreak procedures as follows: • Follow the COVID-19 Flowchart for Management in RACF in Australia (See above) • Signage – Precautions • PPE • Documentation (Health department Line listing) • Inform CEO, EMT, Area Managers, Quality and Risk Manager, Catering Manager, Environmental Services Manager and CI and Training Coordinator. • Communicate with consumers and staff in the facility on what is expected from them • Communicate with families, Public Guardian, volunteers, visitors and contractors • Contact employees to establish their availability and your resources for the outbreak • Inform NURSLINE • Order additional stock where needed. The GP would need to be immediately informed to assess and request the necessary test, as per Tasmanian Health Guideline. The Tasmanian Infection and Outbreak authorities need to be informed of any outbreaks. The consumer might be transferred to hospital depending on their condition and if there are available beds. This will be a discussion between the facility, GP and Health Department. The facility might be placed under lock down and full isolation procedures must be followed with directive from the Tasmanian Health Departments. Independent Living Units: The Village Coordinator needs to inform the residents occupying the units to be mindful of the symptoms and to adhere to the following to prevent spreading the disease: • Should they feel unwell to contact the Village Coordinator so that she or he is aware of their illness. • Should they feel extremely ill to contact “000” and if only feeling mildly ill to contact GP or the National Help line. • During their illness not to go to public places, such as communal areas such as clubhouse, shopping centres, etc. If possible, ask other people to get food and other necessities for you and leave them at your front door. EMERGENCY PREPAREDNESS OUTBREAK PLAN • During this period, whilst they are unwell, for the wellbeing of others, to refrain from having visitors in their home. • Whilst they are unwell, please do not visit any of our Nursing Homes for Assistance • Residents do not need to wear a mask in their home. • Suggest to resident to cancel any domestic assistance for e.g. cleaners • If they need to leave home to seek medical attention, wear a surgical mask (if they have one) to protect others against possible infections. ILU coordinator must ensure their employees have: • The necessary PPE available to protect them against a possibly ill resident; PPE (surgical mask, hand sanitizer, gloves etc. • Inform CEO, EMT, Area Managers, Quality and Risk Manager, Catering Manager, Environmental Services Manager and CI and Training Coordinator if there is a possible employee exposure. • Communicate with resident and their family, Public Guardian, volunteers, visitors and contractors if applicable. • Contact employees to establish their availability and your resources for the outbreak (Admin, gardener, maintenance etc.) Home Care: Home care staff need to provide information to the consumer about risk, isolation and follow- up. Employee’s visiting a consumer at home and suspect there might be a possible COVID-19 virus infection need to inform their team leader immediately. Home Care Team Leaders need to update care plans and follow-up to ensure scheduling care to an infected consumer is reviewed and managed. • Home Care Team Leaders must ensure their employees have the necessary PPE available to protect them against a possibly ill consumer; PPE (surgical mask, hand sanitizer, gloves etc. • Inform CEO, EMT, Area Managers, Quality and Risk Manager, Catering Manager, Environmental Services Manager and CI and Training Coordinator if there is a possible employee exposure. • Communicate with consumers and their family, Public Guardian, volunteers, visitors and contractors if applicable. • Contact employees to establish their availability and your resources for the outbreak All documents are to be treated as SCC confidential. Rights, Roles and Responsibilities of Health Workers, including Workplace Health and Safety Health workers are at the front line of any outbreak response and as such are exposed to hazards that put them at risk of infection with an outbreak pathogen (in this case COVID-19). Hazards include: • Pathogen exposure • Long working hours • Psychological distress • Fatigue • Occupational burnout • Stigma • Physical and psychological violence. This document highlights the rights and responsibilities of health workers, including specific measures needed to protect Workplace Health and Safety 5. The Responsibility of the CEO, Executive Managers and Support Staff includes: General: Assume overall responsibility to ensure that all necessary preventive and protective measures are taken to minimise Workplace Health and Safety risks as outlined in the Governance Framework: • providing information, instruction and training on Workplace Health and Safety, including but not limited to: • Refresher training on infection prevention and control (IPC) • Use, putting on, taking off and disposal of personal protective equipment (PPE) • Provide adequate IPC and PPE supplies (masks, gloves, goggles, gowns, hand sanitizer, soap and water, cleaning supplies) in sufficient quantity to healthcare or other staff caring for suspected or confirmed consumers/residents. • Implementation of Workplace Health and Safety management systems to identify hazards and assess risks to health and safety • Finalise arrangements for seasonal flu vaccinations • Regularly communicate with aged care consumers and families • Review infection prevention and control (IPC) measures • Zero-tolerance policies towards workplace violence and harassment. • Familiarize personnel with technical updates on COVID-19 • Provide appropriate tools to assess, triage, test and treat consumers/residents • Share infection prevention and control information with consumers, residents, families, volunteers and other stakeholders. EMERGENCY PREPAREDNESS OUTBREAK PLAN • Provide a blame-free environment for workers to report on incidents, such as exposures to blood or bodily fluids from the respiratory system or to cases of violence, and to adopt measures for immediate follow-up, including support to victims. • Advise workers on self-assessment, symptom reporting and staying home when ill • Maintain appropriate working hours with breaks • Not be required to return to a work situation where there is continuing or serious danger to life or health, until the employer has taken any necessary remedial action. • Allow workers to exercise the right to remove themselves from a work situation that they have reasonable justification to believe presents an imminent and serious danger to their life or health. When a health worker exercises this right, they shall be protected from any undue consequences. – please refer and discuss with the Executive Manager People and Culture for guidance. • Honour the right to compensation, rehabilitation and curative services if infected with COVID-19 following exposure in the workplace. This would be considered occupational exposure and resulting illness would be considered an occupational disease, • Enable co-operation between management and workers and/or their representatives. • SCC Leave support plan during the COVID-19 Outbreak. • Provide access to mental health and counselling resources. • Support for staff is the Southern Cross Care Employee Assistance Programs for both SCC and Mary’s Grange. EMERGENCY PREPAREDNESS OUTBREAK PLAN • Request information from visitors regarding who they are visiting/recent travel • Displays at all RACF entry points • Letter communication out to all family, visitors, contractors, clients, consumers, staff etc • Minimise wherever possible staff working across facilities • Service tracking registers to be completed for all staff who are in high risk categories i.e have travelled overseas/have been requested to self-isolate • Log book to be completed by all staff who work across multiple sites have reason to visit sites • Student Placements will continue to be permitted but ensure observed hand hygiene competency and briefing on COVID – 19 on orientation to facility. • Ensure any visitors for official purpose i.e contractors, union representatives, have discussion and requested to perform hand hygiene • Weekly update to staff via the intranet/email • Weekly EMT briefing to monitor issues/consider need for escalation Level 2 – Where community-based infections occur/at the advice of the Federal or State Department of Health • Cancel all volunteers attending facility • Cancel all external group outings that are facility arranged • Suspend Student Placements • Restrict all non-essential visitors • Implement additional cleaning hours at RACFs • Restrict all staff movement across facilities • Staff not to attend any external provider training at statewide level • Review all large activities which involve external groups and multiple family/visitors e.g. ANZAC day service so that these are smaller, in house functions only • Second daily update to staff via the intranet/email • Second daily EMT briefing to monitor issues/consider need for escalation Level 3 – As per Level 2 indicators OR when advised by the Federal or State Department of Health • Cease all internal training /move online if possible • Contractors – urgent repair only and to be escorted by maintenance hand hygiene observed • No new admissions to facility permanent and respite • Cease external providers entering sites for ordering or delivery purposes e.g. Bunzl, ecolabel, pharmacy – implement internal strategies for stock review and ordering and deliveries at designated meeting point • Cease all large group activities where an external provider is involved e.g. concerts All documents are to be treated as SCC confidential. Shortage of Staff Action Plan Southern Cross Care (Tas) Inc has employees working across several sites. It is the responsibility of Managers that during an outbreak staff working across sties be reviewed to minimise risks. When staff are working within a facility that has an identified outbreak, they must not continue to work at any other site during this period. Roster clerks need to act now to ensure that all staff availability is up to date to ensure access to staff where required. Managers to review staffing levels and commence recruitment processes where necessary. If there is a potential risk that an employee will incur a loss of income due to the working restriction they are to Inform the Executive Manager People & Culture and Area Manager to ensure that this does not occur. Staffing strategies for lockdown situation • Ensure staff movement is contained. If this leads to services being compromised by short staffing – consult Area Manager for advice • Where quick action needs to occur to allow a staff member who is available to work at another site – this should be escalated directly to the Executive Manager People and Culture for review and approval • Additional support to be provided to ensure staff have access to clean clothes, food and nutrition and rest onsite if unable to leave 8. The responsibility of all SCC employees any categories: • Follow established Workplace Health and Safety procedures, • Avoid exposing others to health and safety risks and • Participate in employer-provided Workplace Health and Safety training. • Use provided protocols to assess, triage and treat consumers/residents. • Treat consumers/residents with respect, compassion and dignity. • Maintain consumer/resident confidentiality • Follow established public health reporting procedures of suspected and confirmed cases • Provide or reinforce accurate infection prevention and control and public health information, including too concerned people who have neither symptoms nor risk. • Put on, use, take off and dispose of personal protective equipment properly. • Undertake the self-disclose and self-isolation measures and report any symptoms to managers, if they occur. • Advise management if they are experiencing signs of undue stress or mental health challenges that require support interventions; and • Report to their immediate supervisor any situation which they have reasonable justification to believe presents an imminent and serious danger to life or health. 10. Communications: Southern Cross Care EMT will draft letters to: • All staff with ongoing updates in and around the workplace • ILU residents • Home Care consumers • Volunteers, families and contractors accessing any SCC sites The EMT may at any time call an emergency meeting and will in the interim meet at weekly or intervals depending on the emergency situation. The matter will be discussed at all site meetings, EMT meetings, Clinical Governance, WHS meetings and information will be escalated to the SCC Board for review and overall governance. EMERGENCY PREPAREDNESS OUTBREAK PLAN 11. Catering Action Plan If a site is compromised that is co-located with a production kitchen, the kitchen will no longer produced food for distribution. The outbreak would affect Glenara Lakes Main Kitchen, Fairway Rise Main Kitchen and Mary’s Grange Main Kitchen. In such an event food will be sourced from the following - Glenara Lakes Main Kitchen: • Launceston General Hospital Fairway Rise and Mary’s Grange: • Cambridge Kitchen who provide meals for the Royal Hobart Hospital. (Distribution of staff) This will be arranged and coordinated by the Catering Manager Statewide. Kitchens and servery to follow all cleaning recommendations from Eco lab and Bug Control (SCC Intranet - Helpful links) 12. Cleaning/Laundry Action Plan Ecolab: https://ecolab.widencollective.com/portals/y8ryvco0/KitchenProcedures-COVID-19 Cleaning procedure recommendations for kitchen/serveries: Chemical use recommendations: Urgent training for catering staff will be coordinated by the Catering Manager and other key catering supervisors in conjunction and with support from the Quality and Risk Manager. Emergency Stock Action Plan SCC will source stock to ensure there is adequate PPE, and other resources to manage an outbreak and sustain optimal infection control measures Should our regular providers not have stock, SCC will source other providers for additional stock where possible. Facilities need to inform the Area Managers or other direct managers should they receive any communication that other clinical stock items are low, such as dressing materials etc. 14. Vendor Alert: SCC need to inform their vendors if there is an outbreak to minimise contact and source additional resources. Vendors would need to cease form entering a RACF unless necessary.
