PA Board of Accountancy Allegheny County Earl C. Aussenberg, certificate and license no. CA013473L, of Pittsburgh, Allegheny County, was ordered to pay a civil penalty of $5,000 because he engaged in public accounting during a period in which he did not hold a current license. (10/02/18) Aussenberg Bernson & Perilman PC, license no. AF001280L, of Pittsburgh, Allegheny County, was ordered to pay a civil penalty of $5,000 and costs of investigation of $427.16 because it held itself out as being composed of certified public accountants while its license was lapsed. (10/02/18) Berks County Douglas G Werley, certificate and license no. CA033475L, of Blandon, Berks County, voluntarily surrendered his certificate and license to engage in the practice as a certified public accountant, because he pled guilty, pled nolo contendere, or was found guilty of a felony under any Federal or State law or the laws of any foreign jurisdiction and convicted of a felony offense under the Criminal History Record Information Act. (10/02/18) Bucks County Judith Anne Baldasari, certificate and license no. CA017827L, of Doylestown, Bucks County, was required to pay a civil penalty of $2,500, because she failed to timely complete a peer review for the renewal period January 1, 2016, to December 31, 2017, that was required as a condition of renewal. (10/02/18) Chester County John Klett, of West Chester, Chester County, was denied certification as a certified public accountant based upon being convicted of numerous felony and misdemeanor offenses. (05/01/18) Delaware County Ian Patrick Kauffman, certificate and license no. CA056431, of Media, Delaware County, was required to pay a civil penalty of $2,500 and costs of investigation of $504.57, because he engaged in the practice of public accounting during a period in which he did not hold a current license, failed to timely complete a peer review for the renewal period January 1, 2016, to December 31, 2017, that was a required condition of renewal. (10/02/18) Lehigh County McPhillips CPA & Company LLC, license no. AF056593, of Coopersburg, Lehigh County, had a public reprimand placed on its disciplinary record with the Board, paid a civil penalty of $2,000 and costs of investigation in the amount of $246.68 because it operated as an accounting firm in the Commonwealth while not currently licensed and registered under the law. (10/02/18) Philadelphia County Howard Charles Lapensohn, CPA, license no. CA022971L, of Philadelphia, Philadelphia County, was suspended for failing to pay a previously imposed civil penalty. PA Architects Licensure Board Out of State Michael William Jones, of Elmira, New York, was granted a probationary PA license for no less than two years based on committing fraud, misstatements or misrepresentation of facts on an application. (05/30/18) PA Board of Auctioneer Examiners Erie County Howard J. Hammond IV, license no. AU003978, of Union City, Erie County, was suspended for failing to pay a previously imposed civil penalty. (06/19/18) Philadelphia County Piedmont Auctions, license no. AY002172, of Philadelphia, Philadelphia County, was suspended for failing to pay a previously imposed civil penalty. (06 /19/18) Westmoreland County Daniel Kenneth Pletcher, license no. AU005239, of Latrobe, Westmoreland County, was revoked and a $97,500 civil penalty imposed because of his felony and misdemeanor convictions. (05/21/18) Out of State Lincoln W. Briscoe, PA license no. AU003684R, of Miami, Florida, was revoked and assessed a civil penalty of $1000 for engaging in the practice of auctioneering when he was not licensed. (07/10/18) PA Board of Barber Examiners Allegheny County Michael A. Brisco, d/b/a Good Fellaz Hair Inc, LLC Shop, license no. BO432789, of Pittsburgh, Allegheny County, was suspended for failing to pay a previously imposed civil penalty. (08/22/18) Huntingdon County Vincent M. Pahula, license no. BL047506L, of Huntingdon, Huntingdon County, was suspended for at least five years immediately stayed in favor of probation, based upon his felony conviction. (07/25/18) Lancaster County John Rodriguez d/b/a New York Barber Shop, license no. BO432980, of Lancaster, Lancaster County, was issued a public reprimand and assessed a $500 civil penalty, based upon operating an establishment without the supervision of a designated manager barber or other designated licensee and failed to meet minimum maintenance and sanitation requirements. DS022583L, of Coral Springs, Florida, was automatically suspended based on his felony convictions under the Drug Act. PA Board of Funeral Directors Monroe County Joseph A. Fantasia, license no. FD139272, of Stroudsburg, Monroe County, was revoked and a $10,000 civil penalty imposed for his practice of fraud or deceit in obtaining a license and for violating a statute in another state concerning the profession of funeral directing (05/18/2018). Philadelphia County Vince N. Baker, F.D., license nos. FD013127L and FS013127L, last known of Philadelphia, Philadelphia County, was suspended, immediately stayed in favor of probation for no less than one year, and a $2,000 civil penalty imposed for his practice of the funeral profession on a lapsed and expired license (05/18/18). Baker Funeral Home, Ltd., license no. FF000110L, of Philadelphia, Philadelphia County, was suspended, immediately stayed in favor of probation for no less than one year, and a $4,000 civil penalty imposed for its practice of the funeral profession on a lapsed and expired license (05/18/18). PA Board of Landscape Architects No disciplinary actions taken. PA Board of Massage Therapy Berks County Cynthia Lynne Arnold, license no. MSG012154, of Reading, Berks County, was suspended for a minimum of three years retroactive to April 10, 2018, based on findings she violated the terms of a previous board order. (07/02/18) Bucks County Amanda Herlihy, of Bensalem, Bucks County, was granted a license to practice massage therapy by examination, said license immediately placed on probation, based on applicant’s crimes of moral turpitude. (02/28/18) Erie County Monica Abbate, of Erie, Erie County, was issued a $4,300 civil penalty, assessed a $375.25 cost of investigation fee and ordered to cease and desist, based upon employing four unlicensed individuals to provide massage therapy services. (07/03/18) Philadelphia County Ann G. Rosenblum, of Philadelphia, Philadelphia County, was denied licensure based on her conviction of a crime of moral turpitude, her inability to practice with reasonable skill and safety, failure to disclose pending criminal charges and questionable moral character. (07/25/18) Wayne County Candace L. Frisbie, license no. MSG005468, of Honesdale, Wayne County, was placed on probation for three years subject to the terms of the Board order based on her felony conviction. (06/25/18) Page 8 of 29
PA Board of Medicine Allegheny County Ted. R. Evans, license no. YM003346L, of Pittsburgh, Allegheny County, was indefinitely suspended retroactive to April 9, 2018, based upon being unable to practice with reasonable skill and safety to patients by reason of illness, addiction to hallucinogenic, narcotic or other drugs or alcohol and failure to submit to a mental and physical examination. (08/06/18) Mark R. Foster, license no. MD023955E, of Bridgeville, Allegheny County, had his license to practice as a medical physician and surgeon temporarily suspended, pending a hearing, on the grounds that his continued practice within the Commonwealth may be an immediate and clear danger to the public health and safety. (10/18/2018) Bucks County David Lawall, license nos. PO000060 and OH000105, of Newtown, Bucks County, had a public reprimand placed on his permanent disciplinary record with the Board and paid a civil penalty of $1500, because he practiced as an orthotist/prosthetist from January 5, 2015, until June 20, 2016, while his licenses were lapsed and expired. (10/23/18) Carbon County Teresa Elizabeth Hubbard, license no. YM012986, of Jim Thorpe, Carbon County, had her license to practice as a respiratory therapist indefinitely suspended for no less than three years retroactive to February 8, 2018, for violating a previous Board order. (05/23/18) Dana Joyce Morgan-Buck, license no. BH003352, of Palmerton, Carbon County, was indefinitely suspended for no less than three years retroactive to April 17, 2018, because she violated terms and conditions of a previous Board order. (07/05/18) Crawford County Brennan Phillip Wetherbee, license no. YM014921, of Cochranton, Crawford County, was indefinitely suspended based on being unable to practice with reasonable skill and safety by reason of illness and made misleading, deceptive, untrue or fraudulent representations in obtaining a license. (06/15/18) Delaware County Edward Sannoh Forson, license no. BH001051, of Collingdale, Delaware County, was indefinitely suspended and assessed a $1000 civil penalty based on receiving ARD in the disposition of felony charges. (06/07/18) Jose Luis Boggio, license no. MD427979, of Drexel Hill, Delaware County, will be ordered to pay an assessment fee in the amount of $3,000 towards costs of investigation; and he shall attend and successfully complete at least ten (10) hours of remedial education dedicated on the topic of medical record keeping and two (2) hours of remedial education related to the topic of the proper prescribing of Benzodiazepines. (10/23/18) Erie County Raymond W. McAllister, license no. MD066253L, of Erie, Erie County, was indefinitely suspended for a minimum period of sixty (60) days and upon reinstatement, his license shall be indefinitely suspended for no less than three (3) years, such suspension to be immediately stayed in favor of no less than three (3) Page 9 of 29
years of probation under the Disciplinary Monitoring Unit, subject to the terms and conditions of the Board; and he shall attend and successfully complete the “Professional Boundaries and Ethics Course” during the period of active suspension, because he violated the law by engaging in sexual behavior with a current patient. (10/23/18) Franklin County Tunde Tolani Tijani, license no. MD056431L, of Greencastle, Franklin County, was assessed a $7,930.73 cost of investigation based on engaging in immoral conduct. (05/23/18) Lackawanna County Steven Joseph Kravitz, license no. MA003033L, of Archbald, Lackawanna County, was indefinitely suspended for no less than three (3) years, such suspension to be immediately stayed in favor of no less than three (3) years of probation, unless that period of probation is extended or modified for cause by mutual agreement of the Respondent and the Professional Health Monitoring Unit, Disciplinary Monitoring Unit subject to the terms and conditions of the Board, because he is unable to practice the profession with reasonable skill and safety to patients by reason of illness or addiction to drugs or alcohol. (10/23/18) Montgomery County Nalini Mehta, license no. MD020206E, of Lansdale, Montgomery County, was assessed a $1,000 civil penalty and was required to submit evidence of having completed all missing continuing education within 6 months based on Dr. Mehta’s failure to complete required education during the applicable biennial reporting period and for making misleading, deceptive, untrue or fraudulent representations in obtaining licensure. (06/27/18) Snyder County Ahmed Kneifati, license no. MD037158L, of Selinsgrove, Snyder County, was placed on indefinite probation based on disciplinary action taken by another state. (06/06/18) Westmoreland County Nabil Jabbour, license no. MD028428E, of Greensburg, Westmoreland County, was temporarily suspended, pending a hearing, on the grounds that his continued practice within the Commonwealth may be an immediate and clear danger to the public health and safety. (10/09/2018) Milad Ishak Shaker, license no. MD437512, of Mount Pleasant, Westmoreland County, was temporarily suspended, pending a hearing, on the grounds that his continued practice within the Commonwealth may be an immediate and clear danger to the public health and safety. (10/09/2018) Susan B. Reamer, license no. YM002114L, of Irwin, Westmoreland County, was indefinitely suspended retroactive to December 6, 2017, and assessed a $227.50 cost of investigation based on her failure to comply with a Board Order to submit for an evaluation. (05/22/18) York County William Scott Wolfe, license no. MD417360, of York, York County, paid a civil penalty of $1,000 and costs of investigation of $1,114.60; was suspended for a period of one year; and shall attend and successfully complete remedial education, because he engaged in unprofessional conduct, is guilty of immoral or unprofessional conduct, and acted outside the scope of his license, in that he engaged in sexual behavior with a patient. Out of State Roger A. Browne, of Margate, Florida, was denied PA licensure based on a felony conviction. (08/06/18) Scharazard Lee Gray, PA license no. MD427277, of Pettibone, North Dakota, was reprimanded and assessed a $500 civil penalty based on disciplinary action taken by the proper licensing authority of another state. (06/25/18) Brent Edward Clark, PA license no. MD042270L, of Morgantown, West Virginia, was automatically suspended based on his felony convictions under the Drug Act. (07/12/18) George Edmond Smith, PA license no. MD034972E, of Sulpher, Louisiana, was indefinitely suspended for no less than three years retroactive to October 31, 2017, because he violated terms and conditions of a previous Board order. (06/22/18) Kwadwo Gyarteng-Dakwa, PA license no. MD439277, of Greensboro, North Carolina, was placed on probation for no less than six months based on disciplinary action taken by other states. (06/19/18) Ivan Dario Baraque, of West Nyack, New York, was granted a PA license to practice medicine and surgery, such license being immediately indefinitely and actively suspended based on his felony conviction. (06/27/18) Jeremy Theodore Reed, PA license no. MD439377, of Harker Heights, Texas, was indefinitely suspended for no less than three years, such suspension shall be immediately stayed in favor of no less than three years or probation, subject to the Board's terms and conditions, because he was unable to practice the profession with reasonable skill and safety to patients by reason of illness, addiction to hallucinogenic, narcotic or other drugs or alcohol, or mental incompetence (10/23/18) Bonnie L. Chen, PA license no. MD422582, of Livingston, New Jersey, was ordered to permanently and voluntarily surrender her license because she had a license or other authorization to practice the profession disciplined by the proper licensing authority of another state. (10/23/18) Brian Roy Altman, PA license no. MD427153, of Woodstock, Virginia, was publicly reprimanded because he had a license or other authorization to practice the profession suspended, revoked, refused or otherwise disciplined by the proper licensing authority of another state, territory, possession or country, or a branch of the Federal Government. (10/23/18) Navigation Commission for the Delaware River and its Navigable Tributaries No disciplinary actions taken. Secretary of the Commonwealth – Notaries Public Blair County Erin M. Stitt, notary commission no. 1161013, of Tyrone, Blair County, is suspended for 12 months, stayed in favor of probation, and required to attend six hours of approved notary education and pay a civil penalty of $500, based on findings that she failed to know through personal knowledge or have satisfactory evidence that the person appearing before her was the person described in and who was Page 11 of 29
executing the instrument and failed to be familiar with the duties of a notary public. (10/24/18) Dauphin County Michelle Ann Gehret, notary commission no. 1283400, of Harrisburg, Dauphin County, agreed to the suspension of her notary commission for twelve months, such suspension immediately stayed in favor of probation, to successfully complete a six-hour notary practice and procedure course, and to pay a civil penalty in the amount of $750 and probation assessment fees in the amount of $600 because she failed to know through personal knowledge or have satisfactory evidence that the person appearing before her was the person described in and who was executing the instrument; failed to maintain an accurate notary register by failing to record a notarization; failed to maintain an accurate notary register by failing to record the character of the act and amount of the fee collected; failed to be familiar with the duties and responsibilities of a notary public; and failed to be of good character, integrity and ability.
