PA Board of Accountancy Lancaster County Dwayne Alan Sollenberger, license no. CA052164, of Manheim, Lancaster County, was suspended for failing to pay a previously imposed civil penalty. (01/19/21) Douglas Patrick Barton, CPA, license no. CA024866L, of Brownstown, Lancaster County, was reinstated to probation for a minimum of one year, based on compliance with a previous board order. (02/25/21) Montgomery County Brian Scot Block, certificate and license no. CA035423L of Jenkintown, Montgomery County was revoked, because he was found guilty, pled guilty, or pled nolo-contendere to any felony, any crime an element of which is dishonesty or fraud, and/or any Federal or state revenue law or the revenue law of foreign jurisdiction; he had his right to practice before any Federal or State governmental agency suspended or revoked; and he has been convicted of a felony. (1/20/21) Philadelphia County KPMG LLP, license no. AF000373L of Philadelphia, Philadelphia County, was levied a civil penalty of $5,000; because they engaged in conduct that brings the profession of public accounting into disrepute or that lowers the public esteem for the profession; and they engaged in unprofessional conduct. (1/20/21) Out of State Brandon Sprankle, license no. CA0043364 of San Jose, CA, was actively suspended for three years; and was levied a civil penalty of $1,500; because he had his license to practice before any Federal or State governmental agency suspended or revoked. PA Board of Barber Examiners Cumberland County Jose Gonzalez of Mechanicsburg, Cumberland County, had his application to sit for the barber licensure examination granted and, upon successful completion of the examination, his barber license shall be granted and then immediately placed on probation for two years based on his criminal conviction. (2/11/21) Zhaquan McGhee of Mechanicsburg, Cumberland County, had his application to sit for the barber licensure examination granted and, upon successful completion of the examination, his barber license shall be granted and then immediately placed on probation for two years based on his criminal conviction. (2/11/2021) Montgomery County Keonna Cox of Norristown, Montgomery County, had her application to sit for the barber licensure examination granted and, upon successful completion of the examination, her barber license shall be granted and then immediately placed on probation for two years based on her criminal conviction. (2/ 24/2021) Northampton County Flow Factory, license no. BO433141 of Easton, Northampton County, was levied a civil penalty of $2,000; costs of investigation of $53.84; and must cease and desist from employing unlicensed individuals in the practice of barber activity, and their license will be publicly reprimanded, because he aided and abetted unlicensed barber practice activity. (12/14/20) PA Board of Certified Real Estate Appraisers Blair County Sharon K. Schultz, license no. BA000262L of Altoona, Blair County, was levied a civil penalty of $250; because she failed to complete at least 28 class room hours of continuing education including the 7-hour National USPAP Update Course, and at least two hours on the Act, regulations and policies of the Board prior to the beginning of the biennial renewal period as required by the Board Regulations. RL138971 of Doylestown, Bucks County, was levied a civil penalty in the amount of $1,750; because she failed to complete at least 28 class room hours of continuing education including the 7-hour National USPAP Update Course, and at least two hours on the Act, regulations and policies of the Board prior to the beginning of the biennial renewal period as required by the Board Regulations. LCO004167 of Greensburg, Westmoreland County, was indefinitely and actively suspended until such time the board grants the reinstatement of his license; after the reinstatement of his license, his license shall be placed on probation for no less than one year, and a reprimand was placed on his disciplinary record with the board, because he was convicted of a felony or a crime of moral turpitude, or received probation without verdict, disposition in lieu of trail or an accelerated rehabilitative disposition in the disposition of felony charges in the courts of this Commonwealth, the United States or any other state, territory, possession of the United States. CT176525, of Pittsburgh, Allegheny County, was suspended for failing to pay a previously imposed civil penalty, said suspension having been stayed on February 10, 2021 based on entering into a payment plan. Johns, license number CO274147, of Kittanning, Armstrong County, was placed on probation based upon a felony conviction. (01/05/21) Bucks County Antoine Lovelace t/d/b/a Lace’d of Langhorne, Bucks County, license number CB125122, was levied a civil penalty of $1,000 and costs in the amount of $192.26 for failing to have the owner or designated person in charge on the salon premises. (01/05/21) Jennifer L. Winter, license no. CT018924L, of Quakertown, Bucks County, was suspended for failing to pay a previously imposed civil penalty. (01/26/21) Delaware County Cynthia Dawn Alvarez, license no. CO259990, of Upper Darby, Delaware County, was suspended for failing to pay a previously imposed civil penalty. (01/12/21) Andrew G Dorton, license no. LCO004104 of Havertown, Delaware County, was suspended for no less than three years, with such suspension immediately stayed in favor of no less than three years of probation, because he possessed used, or distributed a controlled substance, was convicted for driving after imbibing alcohol or utilizing drugs, engaged in fraud or deceit by failing to disclose the conviction, and is unable to practice as a crane operator with reasonable skill and safety by reason of mental or physical illness, condition, or physiological or psychological dependence upon alcohol, hallucinogenic or narcotic drugs, or other drugs which tend to impair judgment or coordination. Kristin Michelle Zimmerman, license number CL191363, of Marietta, Lancaster County, was indefinitely suspended, with the suspension immediately stayed in favor of probation to run concurrent with the probation terms of her criminal convictions in the Court of Common Pleas of Lancaster County and the Court of Common Pleas of Lebanon County because she was convicted of 2 felony offenses. (01/14/21) Mifflin County Ginger Morrison, license number CO261503 of Lewistown, Mifflin County, had her cosmetologist license suspended and stayed in favor of probation to run concurrent with the probation terms of her criminal convictions in the Court of Common Pleas of Mifflin County and the Court of Common Pleas of Dauphin County because she was convicted of 2 felony offenses. PA Board of Dentistry Allegheny County Gaeteano A Catone, license no. DS015862L of Monroeville, Allegheny County, permanently voluntarily surrendered his license, because he is unable to practice with reasonable skill and safety to patients by reason of illness, and he failed to demonstrate the qualifications or standards for a license. (11/20/20) Beaver County Frank J. Makoczy, license no. DS025031L of Beaver, Beaver County, permanently voluntarily surrendered his license, because he failed to follow current infection-control recommendations issued by the CDC, and committed an act of gross negligence, malpractice or incompetence or repeated acts of negligence, malpractice, or negligence. (11/20/20) Blair County Deininger & Rupe Dental Laboratory, Altoona, Blair County, was levied a civil penalty of $3,000; costs of investigation of $1,000; must cease and desists from aiding and abetting its employees in performing any dental activity which requires an authorization to practice the profession, and must make this limitation known publicly at its facility and on its website, because it aided and abetted unlicensed technicians in performing dental prosthodontic services without appropriate prescriptions from licensed dentists. (1/15/21) Bucks County M Yazan Aljraki, certificate no. DF004312 and radiology permit no. 006003 of Jamison, Bucks County, voluntarily surrendered his license, because he engaged in unprofessional conduct and practiced dentistry without a license. Butler County Amy Renae Seybert, license no. DH012736L of Butler, Butler County, had her license indefinitely suspended and a public reprimand placed on her disciplinary record with the Board, because she violated a lawful disciplinary order of the Board. (11/20/20) Cambria County Thomas John Bach, license no. DS026287L of Johnstown, Cambria County, was suspended for no less than three years, retroactive to July 19, 2019, immediately stayed in favor of probation, because he is unable to practice dentistry with reasonable skill and safety to patients by reason of illness, drunkenness, excessive use of controlled substances, chemicals or any other type of material, or as the result of any mental or physical condition, and was convicted of a misdemeanor. (11/20/20) Crawford County Elizabeth Renee Doubet, certificate no. DF003910 of Meadville, Crawford County, was indefinitely suspended for no less than three years, because she is unable to practice as an expanded function dental assistant with reasonable skill and safety to patients by reason of illness, drunkenness, excessive use of controlled substances, chemicals, or any other type of material, or as the result of any mental or physical condition. (1/15/21) Delaware County Maria Helena Barboza, license no. DS025344L, permit no. DP025344A, of Chadds Ford, Delaware County, was placed on probation for a period concurrent with her criminal probation and a public reprimand was placed on her permanent record with the Board, because she was convicted of any misdemeanor under the Drug Act, admitted guilt to a crime or misdemeanor involving moral turpitude, and failed to provide written notice to the Board of her pleas and sentencing. (11/20/20) Philadelphia County Avraham A Weiner, license no. DS039877, permit no. DN002813, of Philadelphia, Philadelphia County, was levied a civil penalty of $4,000; costs of investigation of $2,815; must complete 12 hours of remedial education on the topic of dental diagnosis and five hours on the topic of dental recordkeeping, and a reprimand was placed on his permanent record with the Board, because he engaged in unprofessional conduct. (11/20/20) Erik D Williams, license no. DS027201L, permit no. DN027201A, of Philadelphia, Philadelphia County, was levied a civil penalty of $5,000; costs of investigation of $6,442; and must complete remedial education pertaining to recordkeeping and documentation, because he committed unprofessional conduct by failing to properly prepare, maintain, and retain patient records. (11/20/20) York County Biju Cyriac, license no. DS035702 of York, York County, was levied a civil penalty of $10,000; costs of investigation of $2,724.21; and a reprimand was placed on his permanent record with the Board, because Page 9 of 29
he engaged in unprofessional conduct by failing to possess a current Board-issued permit prior to administering conscious sedation in his dental office. (11/20/20) Out of State Rochelle Lee Fuls, license no. DH068598 of Phillipsburg, NJ voluntarily surrendered her license for at least five years, because she continued to practice as a dental hygienist after having failed to renew her license. (11/20/20) Gerald Awadzi, license no. DS040262 of Leesburg, VA was placed on probation until his license is restored to unrestricted status in Maryland, levied a civil penalty of $2,500; and a reprimand will be placed on his permanent record with the Board, because he had a license to practice dentistry suspended or had other disciplinary action imposed by the proper licensing authority of another state. (11/20/20) PA Board of Professional Engineers, Land Surveyors and Geologists No disciplinary actions taken. PA Board of Funeral Directors Chester County Elnora Brickus Chikaka, F.D. and BKG/Brickus Funeral Home & Cremation, Inc., license numbers FD014593L, FS014593L and FR001083L, of Coatesville, Chester County, were ordered to pay a civil penalty of $12,768.10, based upon engaging in gross incompetency, negligence, and misconduct in carrying on of the profession. (01/07/21) Clearfield County William C Williams Jr, license nos. FD012462L & FS012462L of Curwensville, Clearfield County and David E. Franson, II, license nos. FD138247 & FS015325 of Clearfield, Clearfield County, were jointly and severally levied a civil penalty in the amount of $1,000; and costs of investigation of $434.80; because they both failed to obtain written authorization for cremation from the coroner of the county in which the decedent passed away in; and by engaging in gross incompetency, negligence or misconduct in the carrying on of the profession by requesting a county coroner to backdate the date of cremation authorization to hide the fact that cremation already occurred without coroner approval. (2/18/21) Lackawanna County Michael P. Glinsky, license nos. FD010711L & FS016107, of Olyphant, Lackawanna County, and Michael P. Glinsky Funeral Home, Inc. license no. FR000591L, of Olyphant, Lackawanna County, jointly and severally levied a civil penalty of $1,500; and costs of investigation in the amount of $697.44; because Michael P. Glinsky practiced on a lapsed and expired license, Michael P. Glinsky Funeral Home, Inc. practiced on a lapsed and expired license, and Michael P. Glinsky Funeral Home, Inc. failed to have a licensed funeral director, in good standing with the Board, assigned as a permanent supervisor. (12/11/20) Vincent Frank Maletta, license nos. FD014179L & FS015379, of Olyphant, Lackawanna County, had his licenses to practice as a funeral director and funeral supervisor in the Commonwealth of Pennsylvania suspended for a period of two years, immediately stayed in favor of probation, was levied a civil penalty Page 10 of 29
of $2,000; and costs of investigation in the amount of $599.36; because he attempted to practice under cover of a license which had been revoked or suspended and practiced as a funeral director in this Commonwealth with an expired and lapsed license. (12/11/20) Lebanon County Rothermel-Finkenbinder Funeral Home & Crematory, Inc., license no. FR013850, of Palmyra, Lebanon County, was levied a civil penalty of $1,000; and costs of investigation in the amount of $763.69; because it violated the law by furnishing embalming, other services or merchandise without having obtained written permission from a family member or other person authorized by law to make funeral arrangements for the deceased. (12/11/20) Lehigh County David J. Boyko, license no. FD012811L of Allentown, Lehigh County, was placed on probation for at least three years, and must attend the EDRS Webinar offered by Department of Health, Bureau of Health Statistics and Registries, because he engaged in gross incompetence and negligence in the carrying on of the profession. (2/18/21) PA Board of Landscape Architects No disciplinary actions taken. PA Board of Massage Therapy Allegheny County Darius G. Battles, L.M.T., license no. MSG009782, of McKees Rocks, Allegheny County, had his license to practice as a massage therapist temporarily suspended, pending a hearing, on the grounds that his continued practice within the Commonwealth may be an immediate and clear danger to the public health and safety. (12/07/20) Cambria County Shawn Patrick Marker, license no. MSG000372 of Johnstown, Cambria County, permanently voluntarily surrendered his license to practice as a massage therapist in the Commonwealth of Pennsylvania, because he engaged in sexual harassment, sexual impropriety, sexual violation or sexual abuse while providing massage therapy services to a client, and failed to use safe and functional coverage/draping practices during the practice of massage therapy when a client was disrobed and he engaged in misconduct in carrying out the practice of massage therapy while providing massage therapy services to a client. (2/16/21) Centre County Aiyan Liu d/b/a Yanzi Spa aka Clover Spa (Unlicensed) of State College, Centre County, was levied a civil penalty of $1,000; costs of investigation of $562.17; and must cease and desist from employing unlicensed personnel to provide massage therapy to customers at Yanzi Spa, Clover Spa, or any other entity/operated by her, because she employed an unlicensed individual to provide massage therapy services. Cumberland County Matthew John Perrotta, L.M.T., license no. MSG011118, of Camp Hill, Cumberland County, had his license suspended based on his failure to abide by the terms of his probationary license. (2/26/21) Philadelphia County Carmen Sieleckie, a/k/a Carmen Trefon, d/b/a Russian Massage Spa, a/k/a Reflexology Center, no license, of Warminster, Philadelphia County, was levied a civil penalty $1,000; costs of investigation of $205.34; and must cease and desist from engaging in the practice of massage therapy or holding herself out as a massage therapist in the Commonwealth, unless properly licensed by the Board, because she engaged in the practice of massage therapy or offered to engage in the practice of massage therapy on January 31, 2020, without licensed as massage therapist. (11/04/20) Out of State Joseph P. McLaughlin (unlicensed), of Bellmawr, NJ, was levied a civil penalty of $1,000; costs of investigation of $157.12; and must cease and desist from engaging in the practice of massage therapy or holding himself out as a massage therapist in the Commonwealth, unless properly licensed by the Board, because he provided massage therapy services to a client at her home in Philadelphia, PA, and utilized in connection with describing services provided by his business, Novabodyworxs, the word “massage”, which implied directly or indirectly that massage therapy services were being provided by the business even though no Pennsylvania licensed massage therapists were employed by the business. (2/16/21) PA Board of Medicine Allegheny County Dinakar Golla, license no. MD424542 of Pittsburgh, Allegheny County, was levied a civil penalty of $1,000; costs of investigation of $2,018.54; and received a reprimand on his disciplinary record, because he submitted incorrect Continuing Medical Education credits in conjunction with an application to be recredentialed and provide medical services. (2/3/21) David Harold Averbach, license no. MD063133L of Pittsburgh, Allegheny County, permanently voluntarily surrendered his license, because he had a license or other authorization to practice the profession disciplined by the proper licensing authority of another state. (2/3/21) Cambria County Richard Joseph Green, license no. MD029508E of Johnstown, Cambria County, permanently voluntarily surrendered his license, because he prescribed controlled substances to an individual outside of the minimum standards for prescribing controlled substance which was a direct cause of the individual’s death. Scott M. Camazine, license no. MD055750L of Boalsburg, Centre County, was ordered to have his practice monitored for two years, and must complete eight hours of continuing education in opioid treatment, because he violated Board regulations with regard to prescribing, administering and dispensing controlled substances to patients. (12/15/20) Dauphin County Brett Bernard Ernst, license no.
