INTRODUCTION The following information is provided to give the candidate a better understanding of the Probationary Firefighter EMT or Paramedic position, the elements involved in the selection process, and the recruit training program utilized by the Miramar Fire- Recue Department. 4
POSITION DESCRIPTION Responds to emergency fire, medical, hazardous material, auto accidents and other emergencies. Performs fire suppression duties to protect the lives and property of those in need of assistance. Responds to emergency medical calls, provide basic, intermediate, and advanced life support treatment to patients as well as transportation to appropriate medical facilities. Assesses the condition of patients and makes decisions regarding proper courses of action. Performs such duties as CPR, cardiac monitoring, medication calculation, and administration, immobilization of body, wound treatment, control of bleeding, monitoring, and relaying of vital signs, oxygen and IV therapy administration, sterile suctioning, intubation, defibrillation, ECG monitoring and the management of cardiac arrest and severe trauma patients. Performs extrication of victims from wreckage as necessary. Maintains radio communications with dispatch center, hospital personnel and other public safety agencies to receive and to transmit instructions involving patient care. Determines the need for back-up assistance. Completes required records and forms documenting patient treatment. Assists with the inspection and maintenance of emergency vehicles and the performing of daily supply and equipment inventory; restocks equipment in preparation for subsequent medical responses and shifts. Participates in general fire prevention, public education, and public relations efforts, including making presentations to various groups. Participates in employee training programs; provides leadership and instruction to subordinate fire personnel and/or trainees as assigned. Performs all duties in accordance with applicable licenses, certifications, policies, codes, statutes, laws and regulations, and standards of quality and safety. Keeps abreast of new technology, methods and regulations for fire prevention and suppression operations; attends meetings, conferences and continuing education classes as required and/or appropriate. Remains on call 24 hours per day for emergencies. Prepares and/or processes maintenance records, accident/incident reports, EMS medical reports, exposure reports, inventory records, patient billing reports, vehicle daily check-off, work orders, student evaluations, etc. Operates a vehicle, rescue unit, support unit, aerial truck and fire engine, and operates/uses all fire suppression equipment, hand and power tools, generator, gas detector, self-contained breathing apparatus, rescue equipment and protective gear, extrication devices, life-saving / life supporting equipment, medical instruments and supplies. Operates a computer, two-way radio, telephone, etc. Interacts and communicates with the immediate supervisor, other department supervisors and employees, other City personnel, other fire/rescue/EMS departments, mechanics, school personnel and students, community organizations, patients, and the general public. 5
APPLICATION PROCESS Applications for Miramar Fire Rescue Firefighter/Paramedic are only accepted when testing dates have been established. This can be accomplished by visiting https://www.governmentjobs.com/careers/miramar, to complete the online application. Print a copy of the application to bring with your document/certification, on the IN-PERSON ONLY Application Acceptance Dates (see page 8 - Hiring Process). Actual testing dates will be determined by the department’s need to maintain an active eligibility list. Each application is screened for the requirements, as published in the position announcement. Qualified candidates will be notified of the next phase of the hiring process via email or by phone. MINIMUM APPLICATION REQUIREMENTS • Be a minimum of 18 years of age • Be a U.S. Citizen or Legal Resident • High School Diploma, GED or equivalent from an accredited institution • Current and valid State of Florida Driver License • Current State of Florida Firefighter Certificate of Compliance • Current State of Florida EMT Certificate or Paramedic Certificate • Current State of Florida Paramedic Certificate or if EMT, be in the last semester of Paramedic course (must obtain the State or National certificate within 12 months), of date of hire • Current BLS/ACLS Certification • Booklet Acknowledgement Receipt (see link for the following document) *Please note: City of Miramar Fire-Rescue Policies prohibit the employment of any person who has tattoos to the head, face, neck, or hands or any visible body art, branding scars or tattoos, that depict, describe, or refer to intolerance of any protected class, or in any form of discrimination. An applicant not achieving the minimum passing score will 8
be eliminated from the hiring process. Those candidates who do not pass the written examination will be notified. • Failing to attend the written examination will be cause for disqualification from the hiring selection process. • No applicant will be permitted to enter the testing room after commencement of the examination. Any applicant leaving the room after the written examination has commenced will not be allowed re-entry. • Candidates will arrive to the exam with two #2 sharpened pencils. No calculators, scratch paper, or other aids will be permitted to be used by any applicant. • Any applicant caught cheating will be removed from the testing room and their test will not be scored. They shall also forfeit any future employment opportunities to test with the Miramar Fire Rescue Department. Suggested Study Material: IFSTA 6th Edition and Nancy Caroline Emergency Care in the Streets - 8th edition PHYSICAL ABILITY TEST (PAT) Friday, March 4, 2022 Saturday, March 5, 2022 (If needed) Schedules to be advised The candidate will be required to complete a Miramar Fire Rescue sponsored physical ability test at the Broward Fire Academy to validate the applicant’s current physical condition at the time of the test. The Miramar Fire Rescue Department will provide the necessary equipment at the testing site. The PAT test is explained in detail at the end of this document. Refer to pages 14-17. APPLICANTS WITH PHYSICAL OR MEDICAL CONDITIONS THAT MAY JEOPARDIZE THEM DURING TESTING SHOULD: 1. CONTACT THEIR PHYSICIAN BEFORE TAKING THE PHYSICAL ABILITY TEST TO DETERMINE IF IT IS ADVISABLE TO TAKE THIS TEST. 2. This test should be considered strenuous physical activity and as such carries a risk of injury, not limited to strained muscles, scrapes, scratches and/or bruises. FIRE AND EMS - SKILLS VALIDATION This section will consist of various skills that a State Certified Firefighter and Paramedic have previously been taught. Some examples of Fire Skills may include Ladder Climb, Ladder Raise, Tool and Appliance identification, and Search and Rescue. Some examples of EMS Skills may include ECG interpretation, Intravenous set up, Drug calculations, and safe patient handling. These skills will be a pass/fail evaluation; failure of any component will result in disqualification of the hiring process. ORAL INTERVIEW Wednesday, March 9, 2022 Thursday, March 10, 2022 (If needed) Schedules to be advised The oral interview will occur with a panel comprised of Miramar Fire Rescue Department members of varying ranks and Human Resources. 9
• Be yourself and be honest when communicating. • Make eye contact and be sincere. • Use recent work experiences to help answer relative questions. If you do not have a great deal of work history, draw upon personal life experiences that are relevant. • Think about things that will help you stand out among the other candidates. • Research the City of Miramar and the Miramar Fire-Rescue Department. Familiarize yourself with the job responsibilities of a firefighter as well as the philosophies and culture of fire departments and station life in general. • Practice public speaking and have mock interview sessions to improve skill development as well as confidence. • All applicants must pass the interview with a minimum of 70% to continue in the process. Keep in mind, the dates noted for the Hiring Process. If needed, dates are subject to change. 10
CANDIDATE ELIGIBILITY When positions become available, the Fire Department will select preferred candidates from the rankings on the Miramar Fire Rescue eligibility roster. Scoring Matrix: Written Exam will be worth 50% of the total score (must pass with a 70% to continue in the process) Miramar Fire Rescue Sponsored Physical Ability (Pass/Fail, must pass to continue in the process) Fire and EMS Skills Validation (Pass/Fail, must pass to continue in the process) The Oral Interview will be worth 50% of the total score (must pass with a 70% to be placed on the eligibility roster) Placement will be based on cumulative score CONDITIONAL JOB OFFER Selected candidates will be given a “Conditional Job Offer”. Candidates will be required to successfully complete a Personal History Questionnaire (PHQ) and other pre-employment requirements consisting of: criminal history background check; health/physical assessment (NFPA 1582 medical screening); voice stress analysis; psychological evaluation; driving record review; and a drug and alcohol test. PERSONAL HISTORY QUESTIONAIRE (PHQ) Candidates will receive the PHQ and will be required to be completed as instructed by the Miramar Fire-Rescue Department. A time frame will be given for completion. CRIMINAL HISTORY BACKGROUND CHECK Candidates will receive a background check including a motor vehicle driving record review. An outside agency will conduct this check and forward the results to the Miramar Fire-Rescue Department for dissemination to move the candidate forward. HEALTH/PHYSICAL ASSESSMET - MEDICAL EXAM Candidates will receive a medical exam in compliance with NFPA 1582 (see page 13) is the last step in the hiring process. Candidates will be scheduled to attend a medical exam by the department’s physician. A determination of fit for duty will be given to the Miramar Fire- Rescue Department. VOICE STRESS ANALYSIS Candidates will receive a Voice Stress Analysis conducted by Miramar Police Department. A report will be prepared and forwarded to the Miramar Fire-Rescue Department with a recommendation for the candidate’s eligibility to hire. PSYCHOLOGICAL EVALUATION Candidates will receive a psychological evaluation by the City’s contracted psychologist. The evaluation results will be compared to firefighter statistics from across the country. A report will be prepared and forwarded to the Miramar Fire-Rescue Department with a recommendation for the candidate’s eligibility to hire. DRIVING RECORD REVIEW Comprehensive review of the candidate’s driving history up to the last 7 years. If the candidate was licensed in another state, those records will be requested up to 7 years based on the conditional letter of employment. 11
DRUG AND ALCOHOL TEST This testing will consist of a 14-panel urine drug and alcohol screening and a breathalyzer alcohol test performed at a City of Miramar contracted medical facility. All drug and alcohol testing must be passed in accordance with Miramar Fire Rescue Administrative Policies and Operations Manual 200.02. NOTIFICATION OF HIRE The Miramar Fire-Rescue Department will then notify the candidate(s) who successfully completes the above steps; whereas an offer of employment will be made. 12
NFPA 1582: Standard on Comprehensive Occupational Medical Program for Fire Departments To be eligible for employment as a firefighter, you must meet the medical requirements of the current edition of NFPA 1582, the Standard on Comprehensive Occupational Medical Program for Fire Departments. The following table contains information extracted from Chapter 6: Medical evaluation of Candidates, addresses some of the applicable conditions that might interfere with one’s ability to safely perform essential job tasks. Please review and ensure that these conditions do not apply to you. Any condition preventing use of firefighter protective clothing and SCBA Active General tuberculosis or other air-transmissible infectious disease Any disease or injury that cause, or may cause syncope or altered level of consciousness Far visual acuity less than 20/40, corrected with contact lenses or glasses or far visual acuity less than 20/100 uncorrected Eyes & Vision Color perception – monochromatic vision resulting in inability to use imaging devices such as thermal imaging cameras Any eye condition that results in the candidate not being able to safely perform one or more of the essential job tasks Chronic vertigo or impaired balance Hearing loss in the unaided better ear greater than 40 dB at 500 Hz, 1000 Hz, 2000 Hz and 3000 Ears & Hearing Hz Any ear condition or hearing impairment that results in the candidate not being able to safely perform one or more of the essential job tasks Coronary artery disease, including history of myocardial infarction, angina pectoris, coronary artery bypass surgery, coronary angioplasty, and similar procedures Cardiomyopathy or congestive heart failure, including signs or symptoms of compromised left or Heart right ventricular function, including dyspnea, S3 gallop, peripheral edema, enlarged ventricle, abnormal ejection fraction, and/or inability to increase cardiac output with exercise Any cardiac condition that results in the candidate not being able to safely perform one or more of the essential job tasks Scoliosis of thoracic or lumbar spine with angle greater than or equal to 40 degrees History of spinal surgery involving fusion of two or more vertebrae, or rods that are still in place Spine & Axial Skeleton Any spinal or skeletal condition that results in the candidate not being able to safely perform one or more of the essential job tasks All epileptic conditions including simple partial, complex partial, generalized, and psychomotor seizure disorders – a candidate with epileptic conditions shall have had complete control during the Neurological previous 5 years Disorders Hemiparalysis or paralysis of a limb Any neurological condition that results in the candidate not being able to safely perform one or more of the essential job tasks 13
RECRUIT TRAINING PROGRAM EXPECTATIONS The Miramar Fire-Rescue Recruit Training Program is a rigorous program, designed to teach and reinforce skills needed to perform at the level necessary for the Firefighting and Emergency Medical Service Profession. Part of this program is our Physical Training (PT) regiment. When accepted into the Recruit Training Program (RTP), you will be expected to participate fully in the PT program. The PT program includes: Stretching and Calisthenics: 5 - 7 exercises with stretching Aerobics: Running between 2 and 4 miles several days per week and various other aerobic exercises to include swimming Strength Conditioning: Strenuous aerobic weight training several days each week Endurance: Completing the Firefighter Combat Challenge weekly Failure to successfully participate in our PT program will result in being dismissed from employment. In preparing for RTP, it is suggested that you initiate a PT program that includes the above mentioned (or similar) physical activities. It is also suggested that you include a nutritional routine that will ensure optimum energy levels during your training. Keep in mind, Miramar Fire-Rescue Department’s PT program begins the first week of RTP and continues throughout the entire program. DO NOT wait to begin working out until the day you arrive. You may risk injury and/or possible failure due to the inability to meet physical job requirements. Examples of disqualification from the program: • Two (2) unexcused tardiness • One (1) AWOL per department policy • Missing more than 2 days of training • Failure of individual PO’s after 3 attempts • Conduct/Behavior Maintaining physical fitness is a PRIORITY and is the candidate’s personal responsibility. 14
BROWARD FIRE ACADEMY PHYSICAL ABILITY TEST (PAT) - IN DETAIL IMPORTANT IMFORMATION: • Print the County-Wide Firefighter Physical Ability Test – Part I, II and Water Survive Exercise Plan • Completed Physician Statement • The Physician Statement MUST be signed by a doctor • Keep a copy of the Physician Statement for your records • The County-Wide Firefighter Physical Ability Test is valid for one (1) year IMPORTANT REMINDERS: • The County-Wide Physical Ability Test starts at 7:00 a.m. • Applicants MUST have a valid photo ID to test • Wear appropriate clothing • Long pants. tee shirt and towel REQUIRED for swim portion of test • Change of clothing REQUIRED for physical ability portion of test • Bring a towel • Bring food and water • Applicants MUST have completed Physician Statement and Hold Harmless Agreement to take the County-Wide Firefighter Physical Ability Test 15
The Broward Fire Academy Physical Test is based upon a comprehensive job task analysis; this determined task that are deemed critical, ones performed frequently by firefighters. Part I - The On-Target Combat Task Tests - was developed by Paul O. Davis, PhD and Charles Dotson, PhD of ARA/Human Factors, Inc. This validated job-simulation test is used to objectively measure firefighter performance. The tasks are performed sequentially, and accurately reflect the metabolic demand imposed on a firefighter during actual emergency situations, ARA/Human Factors, Inc. have monitored heart rate responses and pulmonary requirements for these and other firefighter activities. Not all fires are the same, and it is not possible to anticipate every emergency scenario. However, a firefighter who can complete the tasks listed below within the recommended time frame possess the level of fitness necessary to meet most emergency demands. Part I - The On-Target Combat Task Test - is done wearing a 22-pound weighted vest. The events are done sequentially and must be completed within 7 minutes. The clock does not stop until all five events are completed. However, a candidate should pace his/herself and no running is allowed at any time during the entire test. Part Il consists of four separate exercises that will test a candidate's ability to perform as a firefighter. This ability test is designed so a candidate must walk at all times. Automatic failure will occur if: 1. The 7-minute time limit is exceeded. At this point the clock will stop and the test is completed by the individual. 2. Equipment is abused by unnecessarily dropping a nozzle, hose or sledgehammer on the ground. All equipment shall be properly set on the ground. 3. Failure to comply with specific instructions given by the staff of the Broward Fire Academy. PART I TASK 1- HIGH-RISE STAIR-CLIMB EVOLUTION Start behind the red line. The high-rise hose pack shall be placed on the left shoulder. The free right hand is to be used on the handrail for balance only. NOTE: The handrail cannot be used 10 pull yourself up or pivot around landings. Contact must be made with every step. Do not skip a step ascending or descending. The hose pack must be deposited on the fifth- floor landing at the spot marked with an X. TASK 2-HOIST EVOLUTION Walk through the door, exit through door out onto the balcony. Stand behind the red line; use a hand-over-hand method to pull the rolled hose to the top of the railing. Walk forward and lift the hose roll over the railing and place it on the floor. NOTE: Do not place your feet on the concrete edge that is painted red. Return to the fifth-floor landing and place the high-rise hose pack on the left shoulder. Descend the stairs using the handrail for balance only. NOTE: Contact must be made with every step. Do not skip a step or run at any time. Once at the base of the stairs, place the high-rise hose pack on the ground and proceed to the Forcible Entry Evolution. 16
TASK 3-FORCIBLE ENTRY EVOLUTION Position both feet on the diamond plate so your toes are even with the weighted slide. Bend forward so you can always see the end of the slide. With ten-pound dead-blower hammer, using short hard strokes, drive the slide five (5) feet until it clears the numbered markings. NOTE: The body must be always be kept in the bent forward position and the eyes on the target. The hammer head cannot be raised higher than the 10 0'clock position, and the hands must not move on the hammer handle. (Hammer handle must not slide through your hands) Carefully set the hammer down and proceed to the Hose Advance Evolution. TASK 4-HOSE ADVANCE EVOLUTION Pick up the nozzle and place it over either shoulder and drag a charged 1.75-inch hose line straight forward 75 feet. The evolution is finished when the nozzle completely crosses the line between the cones. If you should slip, get up and continue. NOTE: Hands should be placed on the hose or coupling, not the nozzle and the nozzle should reach the waist. Place the nozzle on the ground and walk around the cones to the Victim Rescue Evolution. Automatic failure will occur if the nozzle is dropped. TASK 5-VICTIM RESCUE EVOLUTION Grab the handles of the harness of the 175-pound victim and drag it, walking backwards only, 100-feet to the marked finish line. This evolution and Part I of the test are completed when the victim completely clears the finish line. NOTE: If you should slip, get up and continue. PART II TASK I-LADDER EXERCISE Stand behind the cone; pick up a 24' extension ladder and using a low shoulder carry walk 50 feet to the marked area. Place the ladder on the ground, walk to the 50-foot Bangor ladder where you will don a safety harness and be attached to a safety rope. Climb to the top, put in a leg lock with either leg and lean back. Descend the ladder where you remove the safety harness. NOTE: Proper technique must be employed for the carry and placement of the 24-foot ladder and for the climbing of the 50- foot ladder. Automatic failure will occur if: 1. A fear of heights is exhibited or stops on the ladder are made without authorization. 2. Running, skipping rungs, or disobeying instructions from test administrators is observed. 3. Climbing with both hands and/or both feet on the same rung. 4. 'The exercise is not properly completed. TASK 2-EXTRICATION EXERCISE Using the appropriate handles, pick up the Hurst Tool Spreader. Walk to the wall and place the tips into each of the four holes indicated. Keep the tips parallel with the red marks. Return the tool to the point of origin. Automatic failure will occur if: 1. The body is used for support. 2. Instructions for the test administrators are not observed. 3. The exercise is not properly completed. 17
TASK 3-CONFINED SPACE CRAWL EXERCISE Crawl through an unobstructed portion of the darkened maze. Follow the instructor to the "start" and crawl through the maze using a left-hand pattern. "Stop" when told. Follow the instructor out. No time limit. NOTE: A hard hat and gloves are to be worn for this exercise. Automatic failure will occur if: 1. Unauthorized stops are made. 2. Claustrophobic tendencies or a fear of the dark is exhibited. TASK 4-WATER SURVIVAL EXERCISE Jump off platform one meter above the surface of the water. Tread water for 3 minutes. At the instructor's signal, using any stroke, swim 82' or 25 meters in a timely fashion as determined by the Fire Academy instructor. Exit the pool. Note: A pair of long pants and tee shirt are to be worn for this exercise. If anyone is a poor swimmer, please notify an instructor so you can be placed near the edge. Automatic failure will occur if: 1. The exercise is not properly complete. SWIM TRAINING PLAN The Miramar Fire-Rescue Department has partnered with the City of Miramar Swim Instructors to develop a plan to evaluate Fire-Rescue candidate’s ability to successfully complete the Fire-Rescue Departments entry swim test (Task 4 – Water Survival Exercise).
