BASIC PLAN COVERAGE - SAMPLE CONTRACT Only items specifically listed under our Basic Coverage below are covered under this Service Contract. Any reference herein to items “not covered” by this Service Contract is for clarification purposes only, and shall not be deemed as a comprehensive list. NOTE: Additional coverage options and plans are available for purchase. See page “9” for additional options. INTERIOR PLUMBING SYSTEM THIS SERVICE CONTRACT COVERS Mechanical parts and components that affect the operation of the following items: Natural Gas or Electric Water Heaters up to 50 gallons Leaks and Breaks of Interior Waste, Gas, Water, Vent Lines and Drains Circulating Hot Water Pump Drain Line Stoppages Interior Plumbing Valves: Shower/Tub Valve, Diverter Valve, Ball Valve, Gate Valve Toilet Bowls, Tanks, Wax Ring Seals and Mechanisms (toilet replaced with white builder’s standard if necessary) Permanent Sump Pump - located within the perimeter of the foundation (ground water only) RECOMMENDED UPGRADE Select Elevated or Totally Elevated plan to include coverage for these additional items: Shower heads, shower arms, faucets (replaced with chrome builder’s standard if necessary), interior hose bibs, exterior hose bibs, pressure regulators, tankless water heater, whirlpool bath pump, and motor. EHW shall only be responsible for one sewer stoppage clearing for each main sewer line, secondary waste line, and/or toilet. EHW will clear stoppages which can be cleared with a standard sewer cable through an existing clean-out located in the home not requiring excavation. EHW will not clear a drain or sewer line by accessing a vent or removing toilets. EHW will not be responsible for costs associated with camera scoping a line to diagnose the issue. 4. EHW is not responsible for any damages due to back flow, or stoppages that are caused in whole or in part by damaged, broken, or collapsed lines that exist beyond the home’s foundation, stoppages that cannot be cleared with cable, stoppages caused by, or related to roots or the infiltration of other outside foreign objects, costs related to diagnosis of the stoppage, or the access or installation of a ground level clean out. 5. Limit of $800 maximum for the diagnosis and repair of Tankless, Direct Vent or Power Vent Water Heaters. 6. When replacement of Water Heater is required, customer will be responsible for code upgrade items including flues and vents, expansion tank, earthquake straps, gas/water flex lines, drip leg and any other code items determined by Service Contractor. See Orange Upgrade on page 10 for additional coverage for these items. 5
AIR CONDITIONING THIS SERVICE CONTRACT COVERS Mechanical parts and components that affect the operation of the following items: Primary Ducted Electric Central Air Conditioning (multiple units covered) - Condensing Unit - Evaporative Coils (including thermostatic expansion valves) - Compressor Thermostats (replaced with builder’s standard) *Evaporative Cooler Motor, Pump, Float Assembly, Pulleys, and Belts Air Source Heat Pumps Air Handling Unit Wiring Disconnect Boxes Breakers Accessible Ductwork Pre-Season Tune-Up RECOMMENDED UPGRADE Select Elevated or Totally Elevated plan to include coverage for these additional items: Elevated Plan: Freon recapture/recovery and recharge. Totally Elevated Plan: Freon recapture/recovery and recharge. HVAC Incompatibility Coverage (see page 11 for more details.) Note: *$500 Limit in the aggregate on Evaporative Cooler. Up to $20 per pound for refrigerant. This Service Contract only covers Air Conditioning Systems that are the main source of cooling for the primary living space of the home. EHW will cover up to $500 maximum for diagnosis, repair or replacement of accessible ductwork. Coverage provided for air conditioning units not exceeding five (5) tons per unit. CENTRAL HEATING SYSTEM THIS SERVICE CONTRACT COVERS Mechanical parts and components that affect the operation of the following items: Primary Natural Gas Furnace (multiple units covered) Air Source Heat Pumps Thermostat (replaced with builders standard) Electric Radiant Heater Accessible Ductwork Boiler Note: This Service Contract only covers Heating Systems that are the main source of heat for the primary living space of the home, with capacity not exceeding five (5) tons per unit. EHW will cover up to $500 maximum for diagnosis, repair or replacement of accessible ductwork. Coverage for diagnosis, access and repair or replacement of any radiant heat, boiler, is limited under this Service Contract to a maximum of $1,500 in the aggregate. 6
KITCHEN APPLIANCES THIS SERVICE CONTRACT COVERS Mechanical parts and components that affect the operation of the following items: Dishwasher - All parts and components affecting the heating or cleaning operation of the unit. Oven/Range/Cooktop - All parts and components affecting the heating or self cleaning operation of the unit, including hinges and seals. Microwave - All parts and components affecting the heating operation of the unit. Trash Compactor Instant Hot Water Dispenser Garbage Disposal RECOMMENDED UPGRADE Select Elevated or Totally Elevated plan to include coverage for these additional items: Kitchen Refrigerator: Mechanical parts and components that affect the cooling operation, including refrigerant recapture, recovery, and recharge; ice maker; beverage dispenser. Dishwasher: Racks, baskets and rollers. Oven/Range/Cooktop: Handles, knobs, dials, clocks, interior lining and rotisserie. Microwave: Clocks, shelves, interior lining. Trash Compactor: Removable buckets, lock and key assembly. Note: Maximum for diagnosis, repair or replacement of a Refrigerator is $2,000. Maximum for diagnosis, repair or replacement of a Oven/Range/Cooktop is $2,000. When replacement of an appliance is required, EHW will attempt to replace with equipment comparable in features, capacity, and efficiency but we are not obligated to match for dimensions, name brands and/or color/finish and will contribute up to $75 towards installation costs. GARAGE DOOR OPENER THIS SERVICE CONTRACT COVERS Mechanical parts and components that affect the operation of the following items: Garage Door Motors Capacitor Push Arm Receiver Unit Eye Sensors Carriage Switches RECOMMENDED UPGRADE Select Elevated or Totally Elevated plan to include coverage for these additional items: Rollers, Hinges, Springs, Remote Transmitters, and Key Pad. CENTRAL VACUUM MOTOR THIS SERVICE CONTRACT COVERS Central Vacuum Motor NO FAULT COVERAGE THIS SERVICE CONTRACT COVERS Breakdowns caused by unknown defects and conditions that predate the effective date of this contract such as 1.) insufficiently maintained systems, 2.) rust or corrosion, 3.) mismatched systems, and 4.) improper installation or repair so long as the mechanical failure was not known, was not caused by the current contract holder, and would not have been detectable by a state certified home inspection or a visual inspection and simple mechanical test. A visual inspection is an observation of the covered item to verify that it is structurally intact, without missing parts, and without damage or conditions that could cause imminent failure (i.e. corrosion, sediment build-up, mechanical damage, etc.). A simple mechanical test consists of operating the covered item and performing simple diagnostic tests to ensure that it operates properly and without irregular sounds, smoke, or other abnormal outcomes. A home inspection qualifies as a visual inspection and simple mechanical test. Note: The No Fault Coverage is intended to cover unknown pre-existing conditions caused by the previous owner. For purposes of determining whether a breakdown was caused by an unknown pre-existing condition, EHW will accept a licensed contractor’s written diagnosis or a state-certified home inspection as proof that the problem was unknown so long as the inspection was performed within 60 days of the effective date and so long as the contractor/inspector uses industry standard diagnosis guidelines. For example, HVAC industry standards suggest that a properly functioning A/C system should have a temperature differential split between 16 degrees and 22 degrees. A temperature split differential higher or lower than that standard should not be considered to be in “working condition” in the contractor’s/inspector’s written report. This coverage is not intended to take responsibility for normal maintenance and proper use of appliances and equipment by the contract holder. The contract holder must have all known pre-existing defects correctly repaired by a service professional and deliver to EHW any paid invoices evidencing the repairs before the defective item can be covered by this contract. This coverage also does not cover undersized systems. 8 4
RE-KEY SERVICE THIS SERVICE CONTRACT COVERS For the standard service call fee, EHW will re-key up to six keyholes, including deadbolts and provide up to four copies of the key. Note: EHW is not responsible for picking locks. Locks must be standard residential locks. We will not replace doors, doorknobs, or handles. Properties outside of these counties will be assessed an additional fee; Weber, Davis, Cache, Salt Lake, Utah, Summit, Wasatch, Tooele, Iron and Washington. PRE-SEASON TUNE-UPS FOR AIR CONDITIONING AND HEATING UNITS THIS SERVICE CONTRACT COVERS Air Conditioning Unit: For the standard service call fee, EHW will perform one basic A/C tune-up in accordance with local industry standards. EHW will calibrate thermostat, check freon levels and system pressures, EHW will also test temperature split, perform amp draw on condenser motor, evaporator motor and compressor, clean condenser coils, check condensate lines, clean or replace filters (must be supplied by owner), clean and tighten electrical connections, check contactors, test capacitors and safety switches. Heating Unit: For the standard service call fee, EHW will perform one basic heating tune-up in accordance with local industry standards. EHW will calibrate the thermostat, check heat operations, clean and tighten electrical connections, clean or replace filters (must be supplied by owner) inspect pilot system, test safety switches, clean burners, and test limit switches. Note: Tune-ups are covered for 1 unit. Any additional units inspected will require an additional $30 service call fee unit. Properties outside of these counties will be assessed an additional fee; Weber, Davis, Cache, Salt Lake, Utah, Summit, Wasatch, Tooele, Iron and Washington. If a contractor determines that a repair is needed while performing tune-up an additional service fee will be required to perform repair. AC tune-up may only be scheduled between March 1st and May 1st. Heating unit tune-up may only be scheduled between October 1st and December 1st. Homeowner is responsible for scheduling tune-ups online at ElevateHW.com. BUYER UPGRADE OPTIONS Homeowner may elect to receive additional protection and coverage on one or more of the following items by requesting the specific additional coverages and making additional applicable payment at closing of sale. Coverage is limited to buyer only. WASHER AND DRYER - $100 (PER SET) This option covers all parts and components that affect the operation. 9
KITCHEN REFRIGERATOR - $75 (BUILT IN OR FREE STANDING) This option covers all parts and components that affect the cooling operation, including refrigerant recapture, recovery and recharge, ice maker/dispenser, water dispenser, and auger motor. Note: Coverage is limited to a maximum of $2,000 in the aggregate per Service Contract. Repair or replacement of ice maker/dispenser, water dispenser, or auger motor will only be completed if parts are available. ADDITIONAL REFRIGERATION UNITS - $60 (AVAILABLE WITH KITCHEN REFRIGERATOR PURCHASE) This option covers a combined total of four appliances: wet bar refrigerator, additional refrigerator, wine refrigerator, free standing ice maker, and freestanding freezer. Free Standing Ice Maker: All parts and components are covered that affect the primary function of the ice maker and water dispenser. Note: Coverage available for up to four additional refrigeration systems and is limited to a maximum of $1,000 in the aggregate. SWIMMING POOL AND HOT TUB/SPA EQUIPMENT - $190 SALT WATER SWIMMING POOL AND HOT TUB - $350 This option provides coverage for above-ground and accessible working parts, and components of heating and filtration system as follows: heater, pool pump, motor, filter housing, filter timer, gaskets, blower, back flush valve, pool sweep motor and pump, above-ground plumbing and wiring. Additional Coverage For Salt Water Pools: Salt water control unit, salt cell, and flow sensor for the salt water chlorinator. Note: Swimming Pool and Hot Tub repairs will be limited to $1,000 per Service Contract for diagnosis, labor, parts, and/or materials. Salt Water Swimming Pool and Hot Tub repairs will be limited to $2,000 per Service Contract for diagnosis, labor, parts, and/or materials. ORANGE UPGRADE - $50 This option provides coverage for failures due to Mismatched Systems, Improper Installation, Permit Fees, Code Violations/Upgrades, Crane Costs, and Haul Away Service of failed appliances or systems, as set forth below. Code Violations/Upgrades: EHW will pay to correct code violations, if required, to effect repair or replacement under this service contract. Haul Away: EHW will pay the cost to remove a covered appliance, system, or component when we are replacing a covered system or appliance. Permits: EHW will cover the cost of obtaining local building permits when required to commence replacement of a covered item, EHW will not be responsible for replacement service if permits cannot be obtained. Mismatched System and Improper installation: EHW will repair or replace a covered item that was improperly installed, modified, or repaired prior to the Effective Date of this Service Contract or items pertaining to undersized cooling and heating systems relative to the square footage of area being cooled/heated. Crane: EHW will cover the cost of the use of cranes or other lifting equipment required for service of a covered rooftop air conditioner or heating unit. Note: Coverage under this option (Orange Upgrade Coverage) is limited to $250 in the aggregate to correct code violations/upgrades, improper installation and mismatched systems, permits, haul away, and crane costs. EHW may provide cash in lieu of repair/ service. WATER SOFTENER - $75 This option covers the mechanical parts & components, which affect the operation of the water softener. Note: $500 limit to diagnose, repair, or replace. 10
WELL PUMP - $100 (INCLUDES ONE PUMP PER SERVICE CONTRACT) This option covers all parts and components of well pump utilized exclusively for domestic use. SEPTIC TANK PUMPING - $35 (ONE TANK PER OPTION) This option covers a one time pumping per service contract if the stoppage is due to septic tank backup. SEPTIC SYSTEM - $75 (ONE TANK PER OPTION) This option covers the aerobic pump, jet pump, septic tank and line from house to septic, and sewage ejector pump. Note: Coverage for diagnosis, access, repair or replacement of sewer lines from house to septic tank, sewage ejector, aerobic and jet pumps, septic tank is limited to a maximum of $500 in the aggregate. ELEVATED PIPE LEAK COVERAGE - $100 This option covers concrete encased or underground pipe leaks that are located outside the foundation of the covered home, including water, gas, and drain lines that service the main home. Note: Coverage not available to condominiums and multi-unit buildings. EHW will not be responsible to restore landscaping as a result of accessing underground plumbing. Coverage is limited to $2,000 in the aggregate. BRAND FOR BRAND APPLIANCE UPGRADE - $200 This option provides additional coverage on all kitchen appliances including refrigerator (must have refrigerator coverage) by matching for brand and color of appliance where normally excluded otherwise. Note: Coverage is limited to $10,000 in the aggregate. SPRINKLER SYSTEM AND TIMER - $80 This option covers leaks and breaks of PVC lines (unless caused by freezing), sprinkler heads, bubbler heads, timers, gate valves, shut-off valves, solenoid, and other remotely activated control valves. Note: Sprinkler heads and system timer is replaced with builder’s standard when necessary. Covers systems with six (6) valves or less. Systems with more than six (6) valves please call for quote. HVAC EQUIPMENT INCOMPATIBILITY COVERAGE - $150 **Included in Totally Elevated Plan or must be purchased as an upgrade option When EHW replaces an R-22 condenser or air handling unit, if any replacement parts become incompatible with the existing equipment due to changes in the federally mandated refrigerant standards (R22 to R410A) EHW will pay to upgrade or modify the existing covered air conditioning equipment to maintain compatibility, and to meet the new federal standards. Note: The Federal Government is mandating the phase out of R-22 refrigerant (Freon) because of its ozone-depleting properties (see phaseoutfacts.org for more details). As a result of this federal mandate, air conditioning manufacturers have stopped producing air conditioning systems that utilize R-22 refrigerant. Instead, manufacturers are producing air conditioning systems that utilize a non-ozone depleting refrigerant called R-410a (Puron) for residential applications. The phase-out of R-22 refrigerant and R-22 air conditioning equipment will have a significant financial impact for homeowners who have R-22 air conditioning systems. If an R-22 condenser or air handler must be replaced, it must be replaced with R- 410a air conditioning equipment or components. This will cause existing R-22 air conditioning equipment to become incompatible with any new replacement equipment unless the existing equipment is upgraded. This coverage is meant to cover those upgrade costs due to incompatibility when EHW replaces an R-22 condenser or air handler. Buyer may purchase optional coverage up to 30-days after the effective date of standard coverage. Coverage will commence upon receipt of payment and will expire one year after the Effective Date of Standard Coverage. New Construction Plan: Optional coverage must be purchased within 30-days after close of sale and becomes effective one year after close of sale.
