• The time of year LBH benefit programs renew, and you can change your enrollment by: • Add or remove plans • Adding or removing dependents Annual • • Electing spending account contributions Reviewing all current benefit elections and ensuring the Enrollment: correct information is recorded for the new plan year • If you do not participate What is it? • You cannot change your benefits until the next annual enrollment period • UNLESS you experience a Qualified Life Event (QLE), such as marriage, birth of a child or loss of other qualified coverage • When is Annual Enrollment? • Monday, October 26, 2020 - Friday, November 6, 2020
2021 Annual Passive Enrollment • Allowing employees to continue benefits with Enrollment exception of: • Flexible Spending Accounts & Health Savings Accounts (Medical, Dependent and Limited Purpose) • Current Tobacco and Spousal Surcharges attestations will carry over • Action Required if: making changes to any plans, canceling a plan or newly enrolling in a plan • Any premium increases from 2020 to 2021 will automatically happen • New rate will be on your first paycheck in January 2021 • It’s a good idea to review your current benefit elections and beneficiary information to make sure they are still the best fit for you and your eligible dependents for 2021

Dependent Verification Process • All required documentation must be received by the Benefits Team as soon as possible. You can submit documentation via email at LBH_Benefits@lifebridgehealth.org. You can also fax your required documents to the ATTN: Benefits Team at 410-469- 5161. • Your dependent(s) will not have coverage until we receive the documentation. If documentation is not received within 30 days of your enrollment, your dependent will not be eligible for coverage until the next open enrollment period unless they experience a qualifying life event. Please reference your Benefits Guide for further details and list of required supporting documents. • If you have any questions regarding this process, please contact your site HR team or benefits at LBH_Benefits@lifebridgehealth.org.
