Aetna Health Promise The Aetna Health Promise is a collection of independent programs that emphasize a holistic approach to health through nutrition, activity, weight management, preventative screenings and more. Details for each program can be found throughout this newsletter and on pcsb.org/wellness. Aetna Health Promise Incentive Employees can earn a one-time payment for completing a certain amount of approved wellness activities. Details for the Aetna Health Promise Incentive can be found on page 3. Employees can earn this incentive by earning these credits between January 1, 2020 and December 31, 2020. The incentive will be added to employees’ paycheck in the first quarter of 2021. Employees and spouses insured through PCS can earn credits towards this incentive. Dependent children can not earn credits. Number of credits needed will depend on medical insurance coverage. Employees must be employed and have the PCS medical insurance at the time of payout to receive the incentive. Employees can view the incentives earned through their secure Aetna member website www.aetna.com. Details for where to see your credits can be found on page 8. Details for each program are provided throughout this booklet and can also be found on pcsb.org/wellness during the year. Earn an additional $50 gift card! Health Assessment + Routine Well Adult Exam = $50 gift card In addition to the $250 or $350 Aetna Health Promise Incentive, employees and dependent spouses can earn a $50 gift card each ($100 family max) for completing a Health Assessment and completing a Routine Well Adult Exam with your Primary Care Physician. Both activities will be tracked automatically. Once both activities have been processed, the member will receive an email within 45 days with instructions for how to redeem the gift card. If you do not receive the email within 45 days of when the activities are processed on Aetna.com, you can contact 877-922-4483 or support@AetnaRewards.com. 2

Health Assessment The Simple Steps Health Assessment is a health questionnaire provide by Aetna. Login to the secure Aetna member website- www.aetna.com then click ‘Stay Healthy’ and then ‘Stay Healthy’ again. Then you will click ‘Complete your Assessment’ under Health Programs. Follow the instructions to complete the assessment. Additional resources and online programs will be provided based on the results of the health assessment. Employee Credit: 1 credit Earn an additional $50 gift card for completing the Spouse Credit: 1 credit Health Assessment and a Routine Well Exam in 2020. For more details please refer to page 2. Biometric Screening (Metabolic Syndrome Screening) Available: January 1, 2020—November 1, 2020 Wellness screenings will be completed by Quest via on-site screening, at the Quest Patient Service Centers, or through a Physician Results Form. To register for a wellness screening, visit My.QuestForHealth.com. Enter the below registration key under 'Create Account.' Follow the instructions to confirm eligibility and create your account. If an employee has already created an account in 2019, they will log in to their account to schedule a screening at an On-Site Event, at a Quest Patient Service Center, or through a Physician Results Form. Employees must make an appointment on My.QuestForHealth.com to earn the credit. Employees insured through PCS can participate and earn 1 credit towards the Aetna Health Promise Incentive. Spouse are not eligible to earn this credit. Screenings will measure blood pressure, BMI, cholesterol, triglycerides, and blood sugar via finger stick (on -site screening) or venous blood draw (Quest Patient Service Center). Register on My.QuestForHealth.com Registration key: PCSB Unique ID: Aetna WID# (example: W123456789) 2 Board Family Spouse Unique ID: Aetna WID#S (example: W123456789S) Employee: 1 credit 2 Board Family Spouse: 1 credit Earn an additional incentive of Peerfit. Employees who complete the wellness screening through Quest will earn access to Peerfit. Peerfit is a national network of gyms and wellness studios (yoga, kickboxing, boot camp, etc.). PCS will provide 16 credits per month for up to 6 months to those who complete the wellness screening. Employees can use these credits to reserve and attend fitness classes (ex: yoga, kickboxing, cardio, etc). Employees will receive an email with registration instructions within a month following their screening. Credits will reset the first of every month. Employees will continue to have access to their account after the initial 6 month period. They can enter credit card information to purchase additional classes at a discount rate. Wish your favorite fitness studio was in the Peerfit Network? Nominate a studio at peer.fit/addstudio. 4
Health Screenings Employees and spouses insured through PCS can receive 1 credit for each of the following routine health screenings. Talk with your primary care physician to see if you are eligible for these screenings. • Routine Well Adult exam* *Earn an additional $50 gift card for completing the • Colorectal Screening Health Assessment and a Routine Well Exam in • Flu Shot 2020. For more details please refer to page 2. • Mammography • Skin Cancer exam - in a dermatologist’s office • Well Women exam • Dental Cleaning or Exam** - one credit max per year **Dental and vison exams will be tracked automatically if • Vision Exam** an employee has dental or vison insurance through PCS. If an employee/spouse has coverage outside of PCS, they must submit the Dental/Vision Credit Form found on Employee Credit: 1 credit per exam pcsb.org/wellness to wellnesscreditmailbox@aetna.com. Spouse Credit: 1 credit per exam Credits for routine health screenings will be automatically be tracked through claims. If members do not see their credits on their Aetna member website within 45 days, please email pcs.oconnellj@pcsb.org or pcs.deorseyg@pcsb.org. Aetna Get Active Virgin Pulse Powered by Virgin Pulse, Aetna Get Active is a quarterly team-based step challenge. All employees can participate regardless of insurance coverage and can invite family or friends to participate in these challenges. There will be 4 team challenges each year. The team challenges will be 6-8 weeks. Participants can also participate in individual healthy habit challenges throughout the year but will not earn a credit. To register: 1. Visit http://join.virginpulse.com/AetnaGetActive 2. Begin to enter 'Pinellas County Schools' into the search bar and select Pinellas County Schools. 3. Enter your information to sign up. You must enter in your PCS email using the domain @pcsb.org. 4. Follow the instructions to complete your account. 5. You will be able to invite friends and family once your account is created. Technical issues? Please call 833-525-5786. Four team step challenges each year. Employee Credit: 1 credit max for participating in one team challenge. Spouses (including 2 Board Family spouses) are not eligible for this credit. Employees must track steps during at least half of the challenge to earn the credit towards the wellness incentive. Individual healthy habit challenges or individual step challenges will not count towards the credit. PCS Wellness will reward the top three teams for each challenge and will also be randomly selecting participants during the challenge to win gift cards. 5
Back and Body Works Back and Body Works is a 4 week program that focuses on injury prevention and general health for your back and body. Learn how to lift, sit, stand, and do everyday activities properly to protect your back from painful and serious injury. This is an online program that will launch in spring 2020. Registration information will be sent out in Smart Start Newsletters, Risk Management updates, and posted on pcsb.org/wellness. For details, please visit pcsb.org/wellness. Employee: 1 credit 2 Board Family Spouse: 1 credit The Weight Is Over The Weight Is Over is an 8 week program that focuses on tools and techniques for how to manage and lose weight. This in-person class will track participants progress in weight, body fat percentage, BMI, and waist-to-hip ratio. If you are interested or to see if you are eligible for this program, please visit pcsb.org/wellness or contact Jessica O’Connell at 588-6134. Qualifications of a BMI of 25 and over may apply. Employee: 1 credit 2 Board Family Spouse: 1 credit Aetna Maternity Early risk identification, prevention and education can help families have healthy, full-term babies. The Aetna Maternity Program provides members with access to Aetna Maternity nurses and other resources during their pregnancy and after delivery. Members who begin the Aetna Maternity Program within the first 16 weeks of pregnancy will be eligible for 1 wellness credit, $50 gift card, and a Mayo Clinic Healthy Pregnancy book. To enter in this program call 1-800-272-3531 or log in to your member website at www.aetna.com and look under “Stay Healthy.” Employee Credit: 1 Spouse Credit: 1 Blood Pressure Program through the YMCA Together with the YMCA, PCS is offering a Blood Pressure Program to employees and dependent spouses. This is a 4 month program in person class that is open to all PCS Employees and spouses with PCS medical insurance. The YMCA's Blood Pressure Program helps participants reduce or manage their BP, increase awareness of triggers that may elevate BP, and learn the importance of nutrition and exercise. Diabetes CARE Program Members of the Diabetes CARE Program will receive 1 credit towards their wellness credit and receive diabetic testing supplies at a zero co-pay as long as requirements are met. To enter in the program call 727-588-6137. Visit the Diabetes CARE Page for more information and requirements of the program. Employee Credit: 1 Spouse Credit: 1 Kick Butt or AHEC Class PCS employees and spouses who are current tobacco users can earn a wellness credit by completing a Kick Butt Course or completing an AHEC community session. Kick Butt Employees who are current tobacco users can participate in the 6 week tobacco cessation program. This online program focuses on practical ways to reduce tobacco use to ultimately stop the habit forever. The program will consist of weekly webinars and 3 one-on-one coaching calls. For more information about this program, please call Gina DeOrsey at 727-588-6137. AHEC Quit Smoking Now (4– Week Class) Employees or spouses who are current tobacco users are eligible for FREE multi-session in person AHEC Classes. Visit pcsb.org/wellness for a complete list of Quit Smoking Now (4-Week Class) through AHEC. Employee Credit: 1 max Spouse Credit: 1 max Additional $150 for Benefit Eligible Employees for Tobacco Cessation Benefit eligible employees can earn $150 for completing either the Kick Butt program or the AHEC multi-session classes and remaining tobacco free for 6 months. Employees must call 588-6151 before starting the Kick Butts or AHEC class for details of the incentive. Where to check your Aetna Health Promise credits? Members will be able to track how many credits they have earned through www.aetna.com. Directions: 1. Members will login to www.aetna.com. 2. Click on the Stay Healthy tab. Then click the second link, Stay Healthy. 3. Click on the Incentive tab. 4. There will be two options: Wellness Package Incentive and 2019 Wellness Incentive Program. A. The Wellness Package Incentive will refer to the $50 gift card employees and their dependent spouse can earn by completing the Health Assessment and Routine Well Adult Exam (see page 2) B. The 2020 Wellness Incentive Program will refer to the 5 or 8 credits needed for the overall Aetna Health Promise Incentive. 5. If you click on 2020 Wellness Incentive Program, at the top of the page you will see the total points earned and all the programs will appear below. You will be able to click on each incentive programs to see the details of credits earned. Please note: Depending on the program, credits may take up 30 or 60 days to post. The Aetna continually works to enhance the function of the Aetna member website, therefore, the directions for checking credits may vary in the future. A. 2020 B. Missing Credits? Members can check how many credits they have through Aetna.com. If you are missing any, please email pcs.oconnellj@pcsb.org or pcs.deorseyg@pcsb.org with the activity, date of activity, and your date of birth. Please allow 30-60 days for credits to be posted before reaching out. 8
Health Assessment Directions 2. 1. Log on to 3. www.aetna.com. 2. Click on Stay Healthy. 3. Then click on the Stay Healthy link. 4. On the Health Programs page, you will click on Complete a Health Assessment 4. under Access your Health Dashboard. 5. You will then be directed to your Health Dashboard. You will then click on Launch My Health Assessment. 6. Next, you will click on Start Health 5. Assessment and then follow the directions to complete the health assessment. Once you complete your Health Assessment, you will receive a Health Score. You will 6. then see your credit for your health assessment under your incentives within 4 weeks. How to Find a Doctor 1. First, login to www.aetna.com 2. Click on Find Care & Pricing at the top of the page. 3. Then use the search bar to search a name, specialty, procedure or medication. Next to Search, use the Near field to enter your zip code or city. 4. When you begin to search, different options will begin to populate below. Click on the option that best represents what you are looking for. 5. This will then take you to a list of providers. Information about their location and phone number will be provided. Providers who are accepting new patients and who are in network will be identified. To schedule an appointment, call the desired provider directly. 9
