W H AT I S PA L L I AT I V E C A R E ? Palliative care supports you and those you care about to live with a life-limiting illness where death is inevitable. The Palliative Care Team recognises that from birth to death we all have physical, social, emotional, and spiritual needs that must be valued. Palliative support can happen anywhere – at home, in hospital, in hospice, or residential care. The Palliative Care Team focus their care on living well at the end of life and regard dying as a natural process. Palliative care values life and does not shorten it. It helps you understand what is happening and what to expect, enabling you to make decisions that are right for you and your family towards the end of life. 4
WHERE THE LANDSCAPE LIES E D U C AT I O N F O R PAT I E N T S A series of free, informal educational sessions for patients living with a life limiting illness. An opportunity to share thoughts with others in a unique but similar situation. • What you can expect at the end of life • Questions we have about dying • Open and honest conversations • What’s important to me right now? “Talking about what lies ahead and having questions explained so we could understand our situation and the treatment we could receive allayed any fears and concerns I had”. PA L L I AT I V E C A R E LINK NURSE PROGRAMME (PCLN) The Link Nurse Programme is a 5-day placement in a hospice/ palliative care setting for Hutt Valley nurses working in residential and primary care. Nurses are encouraged to develop innovative ideas that are relevant to their own practise. Much of the learning is case based with opportunity to debrief and reflect. We accept 4 placements at a time. The aims/outcomes of the programme are to: • Equip the link nurse to facilitate best practice palliative care in their unique clinical setting • To increase awareness and access to MDT resources for people receiving primary palliative care • To become more familiar with the philosophy and language of palliative care • Build confidence and resources in palliative care assessment and care planning. 17 - 21 MAY 27 SEPTEMBER - 1 OCTOBER There is an allowance of $200 per day available to cover backfill in your workplace. Palliative Care Lecture Series 2021 PA L L I AT I V E C A R E B R E A K FA S T LECTURES The lectures are delivered by teleconference on the first Thursday of every month from February – December. These sessions are aimed at all health professionals with an interest in palliative care. A light breakfast is provided. To assist Registered and Enrolled Nurses to update their knowledge, skills and competencies annually in the use of the Niki T34 syringe driver for symptom control in palliative care. F U N D A M E N TA L S O F PA L L I AT I V E C A R E This education resource has been specifically developed by Hospice NZ for all staff working in the aged residential sector, primary health, and anyone with an interest in end of life care. These seven workshops are at an introductory level. Ten learning sessions will be delivered over seven workshops. A Hospice NZ badge will be presented at completion of all seven workshops. This series can also be delivered at your workplace by arrangement either in single modules or grouped together. Dying, like birth, is a human event that everyone has to go through. Therefore, care of the dying is everyone’s business. Health professionals can ease and support this process knowing that it belongs with families in communities. • What is a meaningful death? • How do we encourage others to live without a cure? • What is needed and what helps people at end of life? SUPPORTIVE THERAPIES FOR PEOPLE WITH A LIFE-LIMITING ILLNESS This workshop will focus on ways we can support the patient and family through the last days of life using an integrative approach. Health workers strive to include the individuality of the person/patient and family, as well as the symptoms of an illness. • Nursing: An Art and Science • Practical Therapies • Connecting with people with reduced levels of consciousness • Rongoā / Maori medicine • Aromatherapy “Relaxed process; session covered talks, with content; clinical reflection, and practical activity as well as music - good mix...” WEDNESDAY 14 APRIL 9:00AM – 4:30PM COST: $170 This Workshop is a pre-requisite for the Supportive Nursing Therapies used in Palliative Care. Psychosocial impacts on the individual and their family/ whānau. • Supportive strategies for a person to support quality of life, a sense of self and connection with others • Assessment of psychosocial needs • Linking people and their families to supports available • When and how to refer on to specialist psychosocial therapy WEDNESDAY 23 JUNE 9:00AM – 4:00PM COST: $170 LIFE REVIEW WORKSHOP A one day workshop for those interested in being a biographer. The requirements are empathy, being able to relate to anybody, having a non-judgmental attitude and flexibility of time. • To explore and recognise the value of Life Review for each individual and their family • To provide the skills necessary to create a life story • Benefits of life review for the person living with a life limiting illness • The process: interviewing, reviewing, creating “The story is secondary in the process – it is what happens to our patient in the telling of their story that is the most important part”. B O O K T H I S D AT E CHANGINGMINDS POSITIVE END OF LIFE CARE CHANGING MINDS CONFERENCE F R I D AY 2 9 O C T O B E R 2 0 2 1 BRINGING THE ESSENCE OF PALLIATIVE CARE “BACK HOME” This day will focus on rediscovering the essence of palliative care in our local communities. We will look at ways to encourage our community groups and neighbourhoods to reclaim this ‘way of being’ so public perception about care of the dying is opened up and returned to where it belongs – with all of us. • Opening up Public ‘Perception’ to the essence of palliative care. • Expanding the ‘Essence’ out into our Communities. • ‘Regaining’ lost knowledge about the dying process. • Living links in ‘Action’. • ‘Uniting’ together to give effective and creative care. S P E C I A L D AT E S O F INTEREST HOSPICE AWARE NE S S W E E K MAY 2021 Hospice Awareness Week celebrates the services hospices provide throughout New Zealand. It addresses misconceptions that people may have around hospice care. Often, there is a mistaken belief that “hospice is just a place you go to die.” Hospice cares for people’s physical, emotional, psychological, social, cultural and spiritual needs. It helps people with any terminal or life limiting illness to live every moment in whatever way is important to them. DYIN G TO K NO W DAY 8 AUGUST 2021 Dying to Know Day is dedicated to bringing to life conversations and community actions around death, dying and bereavement. It stands for: • Community led conversations about end of life care, death and loss. • Death, dying and bereavement are social experiences best addressed through initiatives that involve the whole community. It is through family conversations over dinner, death cafes, school projects, art galleries, and workplaces that we can break down barriers and socialise the response to death. This can help build compassionate communities ready and able to respond to death and loss when it occurs. • Meaningful value-based end of life planning occurs at any age and regardless of our health status. • Planning ahead means we have the opportunity to communicate what we do and don’t want at the end of life. • Developing death literacy – the practical know how needed for end of life planning. 16 Respect
W ORL D HO SPIC E A ND PA L L I ATI V E C AR E DAY 10 OCTOBER 2021 World Hospice and Palliative Care Day is a unified day of action to celebrate and support hospice and palliative care around the world. It aims to: • Increase the availability of hospice and palliative care throughout the world by creating opportunities to speak out about the issues. • Raise awareness and understanding of the needs – medical, social, practical, spiritual – of people living with a life limiting illness and their families. • Raise funds to support and develop hospice and palliative care services around the world. 17 Partnership Compassion Learning

FOR PATIENTS AND FAMILIES These books can be found in the Britannia Lounge, Te Āio Sanctuary and the Tamariki Lounge at Te Omanga Hospice. • The collection has resources to help with the challenges of living with life-limiting conditions. Books can be borrowed for up to four weeks. Please complete a loan form before taking a book off premises. 18 Respect
