2020-2021 Ontario Graduate Scholarship (OGS)/ Queen Elizabeth II Graduate Scholarship in Science & Technology (QEII-GSST) Application Use this form to: • Apply for an OGS/QEII-GSST through the University of Waterloo Instructions: 1. The student should review the Application Instructions prior to completing this form. 2. The student will complete sections 1-10 (or 11 if applicable) then read and sign section 12. 3. The student will complete section 13 (checklist) before submitting the application. 4. The student will submit the complete application to their proposed department graduate coordinator. 5. The student will ensure that the following application components are also ordered/requested: a. Two OGS/QEII-GSST Academic Assessment Reports; b. OGS/QEII-GSST applicant transcript request, if applicable. If you submit a transcript request for any of the scholarships above but subsequently choose not to submit an actual application for it, then you must submit an OGS/QEII-GSST transcript request using Waterloo’s OGS/QEII-GSST applicant transcript request webform. Section 11: Special circumstances (only if applicable) Section 12: Collection, disclosure, consent, declaration and signature Collection and disclosure of information: The Ministry of Training, Colleges and Universities (MTCU) has provided the University of Waterloo with funding to administer the Ontario Graduate Scholarships (OGS) and the Queen Elizabeth II Graduate Scholarship in Science and Technology (QEII-GSST) competitions. As a condition of this funding, the university is required to make reports to the MTCU of your contact information, the amount of funding you receive, and information related to your program of study. This personal information will be used by the MTCU to administer and finance the OGS/QEII-GSST Program. If you apply for the Ontario Student Assistance Program (OSAP), this personal information will be used to update your OSAP application or award, including your declared income. Administration includes: public reporting on the administration and financing of the OGS Program and the QEII-GSST Program; monitoring and auditing your university or its authorized agents to ensure that they are administering the OGS Program and the QEII-GSST Program appropriately; conducting risk management, error management, audit and quality assessment activities; and conducting policy analysis, evaluation and research related to all aspects of student assistance. Financing includes: planning, arranging or providing funding for the OGS Program and the QEII-GSST Program. The Ministry administers the OGS Program and the QEII-GSST Program under the authority of the Ministry of Training, Colleges and Universities Act, R.S.O. 1990, c. M.19, as amended.
