This guide is intended to give a brief description of the PetSmart benefits program. Full details of the program are set out in the legal plan documents. In the event of any discrepancy, benefits will be paid according to the terms of the legal documents and government regulations. Manulife Financial will provide a more detailed description of these plans to all members. 2
Your Benefits At a Glance The table below contains a brief overview of your PetSmart benefits. For more details see pages 3 and 4 of this guide and then also refer to your Manulife Employee Benefits Booklet or access it online at http://benefits.petsmart.com under the Canada tab. If you have questions about your coverage, please call Manulife Customer Service at 1-800-268-6195; our group/plan number is 31049. Benefit Highlights Extended health care benefits Prescription drugs 90% reimbursement, dispensing fee cap of $12.00 (NEW! Increase from $8 to 12) • Hospital 100% reimbursement for semi-private room accommodation • Out-of-country emergency 100% reimbursement, to a $1 million maximum for associate and dependents • Professional services 80% reimbursement, to certain per calendar year maximums Dental care • Preventative services (Level 1) 90% reimbursement, to certain maximums • Major services and orthodontics 50% reimbursement, to certain maximums Vision care One eye exam per calendar year is covered at 100% up to $250 every 12 months (if under age 18) 100% (up to R&C maximum*) 100% up to $250 every 24 months (if over age 18) Basic life insurance Company-paid benefit equal to one times your annual base salary, to a maximum of $350,000 Optional Employee life insurance Available in units of $10,000 to a maximum of $200,000 Optional Dependent life insurance Available in flat amounts of $10,000/spouse and $5,000/child Accidental death and dismemberment (AD&D) Company-paid benefit equal to the amount of Basic Life coverage Short-term disability (full time salaried & hourly associates) • Payable for 17 weeks Company-paid benefit equal to 66.67% of your weekly earnings to the greater of $1,200 or the current maximum weekly benefit under the Employment Insurance Act Long-term disability (full time salaried & hourly associates) • Payable after the first 17 weeks 60% of your monthly insured benefit to a monthly maximum of $6,000; up to $7,500 if evidence of insurability approved. Mandatory participation and 100% employee paid premium. Savings and stock purchase plans • Group Registered Retirement Savings Plan You can contribute up to your CRA limits; PetSmart will (RRSP) match your contribution in the DPSP • Deferred Profit Sharing Plan (DPSP) PetSmart will match your contributions, up to 4% of your earnings. * R&C is the average rate or charge for identical or similar services by providers in the same geographic area. 3
Extended Health Care Benefits The PetSmart Extended Health Care Benefits plan covers services and supplies typically not covered under your provincial health care plan. Coverage highlights Maximum coverage Prescription Drugs The plan pays 90% of the cost of covered generic prescription drugs. None You pay the remaining 10%. Dispensing fee cap at $12.00/ prescription. Show your benefits card to the pharmacist before your prescription is Note that some drugs are not included filled. Dental Benefits The PetSmart Dental Plan pays for expenses up to the current year’s dental association fee guide limits. While you’re a member of the plan, you’ll be reimbursed for all eligible expenses, up to the annual, per- person maximum. Be sure to check if your dentist follows the current fee guide (and doesn’t charge more). Charges exceeding the current fee guide amount cannot be reimbursed. PERSONAL BENEFITS - OPTIONAL Personal Benefits Personal Benefits offer Life and Critical Illness coverage that’s affordable, portable and available to you, your spouse and your children. This is a stand-alone product and is not part of your PetSmart group benefits. Since you purchase this coverage directly from Manulife, it moves with you even if you leave PetSmart. Your Personal Benefit contract is between you and Manulife Financial. Personal Life Insurance This benefit provides additional coverage above and beyond your Basic Group Life Insurance and allows you to choose the amount of coverage that fits your circumstances. ➢ Up to $25,000 of term life insurance for you and your spouse without medical information. ➢ A maximum of $500,000 may be purchased by providing additional medical information. ➢ Child Life coverage is a flat amount of $20,000 per dependent child. Personal Critical Illness This benefit provides a lump sum payout if and when a critical illness strikes,and can provide income replacement or funds to cover healthcare costs when you need it most. Survival rates for critical illnesses have improved, but the risk of being affected by one remains high. ➢ Covers 22 medical conditions for you and your spouse. ➢ You can purchase up to $25,000 of coverage without providing medical information. ➢ A maximum of $150,000 may be purchased by providing medical information. ➢ Child Critical illness covers 15 childhood medical conditions and coverage is a flat amount of $10,000 per dependent child. Provincial Contact Information Your provincial health care plan provides basic health coverage. Typical items covered under the plan are ward hospital accommodation in Canadian hospitals and doctors’ fees. Where applicable, PetSmart pays the provincial premiums and payroll taxes associated with your provincial health care plan. Your PetSmart Extended Health Care benefits plan supplements your provincial health care plan, including reimbursement for prescription drugs. To be eligible for supplementary health insurance, you must be insured under the provincial hospital plan of your province of residency.
