STANDARIZATION OF IV CANNULATION PROCEDURE- A CHALLENGE Ms Loma Waghmare, Ex- Grade I (Nursing Education), Ruby Hall Clinic-Pune INTRODUCTION: Peripheral cannulation provides access for the purpose of intravenous (IV) hydration or feeding and the administration of medication. A peripheral cannula is a exible tube containing a needle (style), which may be inserted into a peripheral vein, usually those in the lower arm including metacarpal, cephalic and basilic vein. Cannulae may also be inserted in the veins of the lower extremities. Peripheral cannulation is simple procedure, but it tests the skill and the experience of nursing personnel. It is evidenced that specialised training, handling the peripheral catheters lead to reduce the risk of infection and rate of thrombophlebitis. Infusion experts provide good skilful insertion of IV cannula and maintenance. NEED OF THE STUDY: IV cannulation is the commonest and rst invasive procedure among hospitalised patients. It is however associated with risk and complications that can delay the prognosis and have an adverse impact on the clinical outcome. In our institute trained nurses perform the procedure of peripheral cannulation. Improper insertion and maintenance of peripheral cannula can cause grave consequences of extravasation and inltration. This in turn will affect the patient physical, psychological and nancial well being. It will also reect on the condence and reputation of the unit. Therefore accuracy in the insertion of the peripheral cannulation is vital. PROBLEM STATEMENT: A study to assess the effectiveness of hands-on-training in peripheral cannulation among the nurse in a tertiary care hospital. OBJECTIVES: 1. To assess the effectiveness of the hand- on- training in peripheral cannulation 2. To identify the common errors in the procedure occurs and take corrective action. 7
RESEARCH METHODOLOGY: Research Approach: Quantitative research Setting: Ruby Hall Clinic Pune-01, General Ward Sample Size: Phase one: 59 (49 were assessed) Phase two: 23 Inclusive Criteria: All nurses in general ward who were given hands- on training Exclusive Criteria: During the assessment of the procedure, unit in-charges and the nurses shifted to critical care were excluded. RESULTS: The analysis of the training assessment of the procedure of peripheral cannulation showed that the overall compliance to the procedure was 88.9% The area where compliance was lacking were a. Flushes the cannula using push- pause technique with 63.2% b. Cleans the area with disinfectant solution in circular motion with 44.8% c. Lowers the angle of the cannula with 42.85% d. Inserts the cannula into the vein at 15 to 30 degree angle & Checks for rst ashback, conrms it & Inserts further and withdraws stillette slightly for second ashback; conrms it with 10.2% each e. Keeps drape under the site of insertion & Positions the site, ties the tourniquet and wears gloves with 4.08% each PERCENTAGE OF NON COMPLIANCE IN PHASE 1 63.2 70 44.8 42.85 60 50 40 2.04 4.08 4.08 10.2 10.2 8.16 % PERCENTAGE 30 10.2 20 10 0 CLEANING OF THE SITE IN ANGLE OF INSERTON (15- INSERTION WITH PUSH- USE OF DRAPE LOWERING ANGLE CONFIRMS 2ND FLASH FLUSHING- PUSH PAUSE PROCEDURE EXPALATION CHECK 1 ST FLASH BACK GLOVING PULL TECHNIQUE TECHNIQUE CIRCULAR BACK 30 ) Fig 01: Percentage of non compliance in phase 1 8
The result of the re- demonstration training showed improvement with an overall compliance of 96.85% with an improvement in the gaps analysed in phase one The areas where improvement is needed were the following a. Cleans the area with disinfectant solution in circular motion 34.78% b. Flushes the cannula using push- pause technique with 8.6% c. Checks for rst ashback, conrms it ; writes the date and time of insertion, at the site & disposes waste in appropriate containers with4.38% each Fig 01: Percentage of non compliance in phase 1 CLINICAL IMPICATIONS: NURSING PRACTICE: - Regular periodic on the job demonstration and re demonstrations should be conducted. - Instilling condence in the staff nurses thus improving the skills in performing the procedure. ADMINISTRATION: - Regular supervision and auditing should be conducted to nd the area of non compliance. RESEARCH: - The study can be performed at a large scale. - Study can be performed more precisely taking in consideration the years of experience of the staff nurse. BIBLIOGRAPHY/ REFERENCE: 1. Economic impact of Advanced IV Kit on VAD securement in Hospital Settings: A quality improvement project Ms Jermin Roshini-CNO, Ms Jayashree-ICN, Ms Dallie- Nursing Educator (Fortis Cunningham Hospital) ABSTRACT: Hospitals are intended to heal the sick, but they are also sources of infection. Intravenous administration of uids, drugs, and nutrition is very common in hospitals. Although insertion of peripheral and central cannulae and subsequent IV therapy are usually well tolerated, complications that prolong hospitalization, and in some cases cause death, can arise on occasions. This article seeks to review this area and to outline good medical practice, their success depends on such aspects as careful training in reducing the complication and better patient care. AIM statement: To improve the compliance in IV Cannulation and to reduce Complication like PLABSI by implementing Advanced IV kit and PLABSI bundle in Vascular Access Devices. Background: Despite of having structure VAD Protocols, Noncompliance in practices have been observed among health care workers due to lack of product availability at the insertion time, lack of awareness and compliance. In order to improve knowledge level and to motivate health care workers to adhere to Best practices and thereby to reduce PLABSI by implementing innovative and best practices. Method: Ÿ Implementation of Advanced IV Kit in all departments Ÿ PLABSI Bundle. Ÿ Pre and Post Audit Audit ndings Pre and post analysis: Analysis of Quality Improvement project was done for a year 2016 April –March 2017(VIP SCORE 2 , 3 –Average of 7-8 numbers in a month ) and after implementation of Advanced IV Kit and PLABSI bundle post analysis from April 2017 –March 2018 (VIP SCORE 2 , 3 –Average of 1-2 numbers in a month. Results: Ø Retraining conducted to all health care workers special interests and involvement shown by Nurses. Created interest and involvement by adopting different teaching methods like group activities & demonstrations. Celebrated IV Nurses Day to bring awareness and emphasize on importance IV Therapy and complications. Gradual improvement has been observed in reduction of PLABSI and we are trying to sustain the results by continuing vigorous and meticulous surveillance activities by the team.
