Chronic care disease costs account for 42% of total direct medical care expendi- tures or $39B CDN annually, and with de- clining health of the average Canadian, is proven to be lifestyle related.1 The Chronic Disease Prevention Alliance of Can- ada estimates the debilitating nature of illness creates an indirect cost due to productivity loss amounting to $54.4B annually.1 JSC Lifestyle Man- agement Inc. is collaborating with TELUS in an activity involving up to 1000 individuals living in Windsor-Essex, Ontario. The focus of the study design uses Community Based Participatory Research (CBPR) to engage participants in different cohorts in the research process.2 In more traditional studies, major environmental and individual barri- ers were identified.3 Use of the TELUS PHR inherently overcomes some of the selected key barriers, such as client privacy, security, compliance with evolving standards, and availability of systems.3 TELUS provides a nationally recognized system compliant with Canada Health InfoWay security and privacy standards. The tenets of our model promote early adoption in a preventative health approach to enable employers/employees to focus more on their health strengths, before disease exists. Benefits from education through the use of the PHR and its value in early detec- tion and personal health management are expected, complementing treatments of pre-existing conditions. A simple health passport focused on evidence-based informa- tion accessed and implemented by participants, complements the TELUS PHR. One added advantage of the TELUS PHR is that it integrates with TELUS Health Space, a provider system/client care relationship that can optimize preventative future care. MAIN OBJECTIVES: Windsor-Essex community participants with a chronic illness such as Cancer, Diabetes, Autism receives supervised edu- cation and support for 12 months during this research activity leading to potential benefits such as: • Reduced long term costs of living with chronic disease • Improved adherence to required medication • Pain management • Stress management in living with a chronic disease • Improved overall perception of their quality of life 1 Mirola, M.
