Day & Boarding Student Permissions and Consent When processing this application, it may be necessary for St Philip’s College, Alice Springs to look at documents held by previous schools, health care professionals or other government agencies. This information will be collected, used and stored consistent with the Privacy Act 1988, NT of Australia Information Privacy Act 2002 & Privacy Amendment (Enhancing Privacy Protection) Act 2012. The consent of the owner of the information, while not always necessary, is appreciated and will speed up the assessment of the application. The College requests permission be given by the parent/s or guardian/s of a student to allow the principal or representative to contact, collect and record any relevant information (either orally or via documentary material or report) about the child. Student Enrolment Agreement Student Code of Conduct
Day and Boarding Student Enrolment Agreement (Parents) It is agreed as follows: Student Name: 1. Acceptance of Enrolment 1.1 The College agrees with the Parents to accept enrolment of the Student on the terms set ☐ Agree out in this Enrolment Agreement. 2. Responsibilities of the Parents 2.1 The Parents Agree: 2.1.1 to pay the Enrolment Fee of $100 at the time of signing this Enrolment Agreement and ☐ Agree then to pay the Acceptance Fee of $250 at such time as an Offer of a Place is given which they acknowledge is non-refundable in any circumstances. 2.1.2 to pay all tuition fees and charges levied by the College by the due dates in accordance ☐ Agree with the Schedule of Fees published by the College from time to time. 2.1.3 that the Student will comply with the Code of Student Conduct which may be amended ☐ Agree from time to time at the College’s absolute discretion either verbally or in writing. 2.1.4 that the Student and the Parents will comply with the College’s rules, regulations, policies ☐ Agree and procedures including those published in the Parent Handbook which may be amended from time to time at the College’s absolute discretion verbally or in writing. 2.2 Both Parents must sign this Enrolment Agreement unless a parent or guardian informs the ☐ Agree Principal in writing that he/she is solely responsible for the Student and for payment of the tuition fees and charges levied by the College. The Parents acknowledge that, unless otherwise agreed in writing with the Principal, each parent is jointly and severally liable for the payment of all tuition fees and charges levied by the College. 2.3 Should any tuition fees and charges not be paid by the due date then the College may ☐ Agree suspend or terminate the Student’s enrolment at the College’s absolute discretion. 2.4 That the College is entitled to charge a monthly administration and service fee, in such ☐ Agree amount as may be determined from time to time at the College’s absolute discretion, on any outstanding tuition fees and charges until those tuition fees and charges have been paid in full. 3. Withdrawal of Student from the College 3.1 Should the Parents wish to terminate the Student’s enrolment under this Enrolment ☐ Agree Agreement then at least a term’s notice in writing must be given to the Principal before the removal of a student from the College, otherwise a charge equivalent to a term’s fees will apply. 3.2 Where the Parents give a term’s notice to terminate the Student’s enrolment under this ☐ Agree Enrolment Agreement (as provided by Clause 3.1) and where the Student is a boarder, the Col- lege, in its absolute discretion, is entitled to charge that component of the tuition fees and charges relating to boarding for the balance of the academic year in which the notice of withdrawal takes effect. 3.3. A pro rata charge is made for new students entering the College for the first time after a ☐ Agree term has commenced. If a student leaves during a term without giving a term’s notice, no refund will be made for the remaining portion of that fee instalment. 3.4 A holding fee of 25% of the applicable Tuition Fees for the year level is required if students ☐ Agree take leave of absence from their studies at St Philip’s College. The amount charged is calculated according to the time on leave; it is applicable for leave covering a minimum of one term to a maximum of one year. One term’s notice in writing to the Registrar is required. 4. Change of Address 4.1 The Parents agree to immediately inform the College in writing of any change in their ☐ Agree postal address or telephone numbers. 5. The Parents agree to indemnify the College in respect of any costs or expenses which the College incurs as a result of the College taking action pursuant to this clause. 6. Student Information/special needs 6.1 The College has limited resources to provide for special needs of particular students. ☐ Agree Special needs include physical or intellectual disabilities, learning difficulties or learning support requirements and needs of a medical, psychological, health or dietary nature. The College does not warrant that it is able to provide for special needs and the parents acknowledge that the College cannot always meet a student’s existing, future or potential special needs. 6.2 The Parents warrant that they have informed the College of all special needs of the ☐ Agree Student which may be relevant to the education or welfare of the Student. If the Parents fail to inform the College of any special needs in relation to the Student then the College, in its absolute discretion, may refuse to proceed with enrolment of the Student or terminate the enrolment of the Student under this Enrolment Agreement. 6.3 The Parents agree that they will immediately inform the College should the special needs ☐ Agree of the Student change in any way from the date of signing this Agreement until termination of the Student’s enrolment at the College (as also mentioned on page 6 of this Enrolment document). 6.4 Where the parents inform the College of special needs of the Student, or where those ☐ Agree special needs change, then the College will discuss those special needs with the Parents. If the College considers that it cannot meet the special needs of the Student then the College, in its absolute discretion, may refuse to proceed with enrolment of the student or terminate the enrol- ment of the Student under this Enrolment Agreement. 6.5 If the College agrees to accept a Student for enrolment or to continue with the enrolment ☐ Agree of a Student after being informed of special needs in relation to the Student then the College will not be liable for any failure to provide for those special needs. 7. Discipline 7.1 The College’s disciplinary policies apply to conduct of the Student both inside and outside ☐ Agree the College and whether or not the conduct is connected to College activities. 7.2 The College may, in its absolute discretion, determine when conduct of the Student war- ☐ Agree rants discipline and may apply such discipline as the College, in its absolute discretion, considers appropriate having regard to the Code of Student Conduct and other College rules, regulations, policies and procedures in force from time to time and the conduct of the Student. 7.3 The Parents accept that the College’s disciplinary procedures include suspension from the ☐ Agree College and immediate termination of the Student’s enrolment under this Enrolment Agreement at the College’s absolute discretion. In the event of suspension or termination of enrolment under this Enrolment Agreement there will be no refund of any fees previously paid. 8. Loss of Property and Insurance 8.1 It is impossible for the College to implement and administer systems or processes which ☐ Agree will adequately protect the Student’s personal property against loss, damage or theft. 8.2 All personal property brought to the College or to a College activity by the Student is at ☐ Agree the sole risk of the Student and the Parents. The College accepts no liability for loss or damage to personal property of the Student however that may occur, and the College has no responsibility to implement and administer systems or processes which may minimise or avoid such loss or damage. 8.3 The College does not insure the personal property of Students or Parents which is brought ☐ Agree to the College or to College activities. No Warranty 11.1 The College does not warrant that it will achieve any particular outcome in respect of the ☐ Agree Student. 11.2 The College does not warrant that it can control behaviour or activities of other Students ☐ Agree or Parents. 12. Indemnity by the Parents 12.1 The Parents will indemnify the College, the Principal and the College’s other employees ☐ Agree and agents against: 12.1.1 any loss or damage suffered by the College, the Principal or the College’s other employees and agents, or 12.1.2 any claim against the College, the Principal or the College’s other employees and agents, arising out of a failure of the Student or the Parents to comply with the Code of Student Conduct and all other rules, regulations, policies and procedures of the College published in the Parent Handbook and which may be amended from time to time at the College’s absolute discre- tion either verbally or in writing. 13.
