Pakistan Children’s Heart Foundation Page 4 Version 6.0 PCHF-201904A Date: Apr 2019 Executive Summary Approximately 5 million children are born of whom almost 50,000 have congenital heart disease alone in Pakistan. Among these 5 Million more than 13,000+ need invasive surgery /intervention during the first year of their lives. However, in total 25000 born will be needing surgical/intervention procedure in their life span. Contrary to this only an estimated 5,000 surgeries are performed across the country and almost same cath procedures. As the numbers suggest there is an extensive queue of children waiting to access the needed health care while many will face sad ends before they can reach it. There is an enormous need for an institution where trained and qualified doctors can perform pediatric cardiac procedures keeping up with the best of International standards. Pakistan Children's Heart Foundation (PCHF) has four objectives; I. Awareness and Advocacy for Congenital Heart Defects (CHD) among public, health care professionals and policy makers, II. Capacity building of existing departments of private and public hospitals by organizing medical missions & provide onsite expertise to enable existing hospitals to develop/enhance CHD programs. III. To establish a131 bed not-for-profit Children’s Heart Hospital and Research Institute (CHHRI) in Lahore to serve as Centre of excellence. Misbah Ul Haq former captain Pakistan Cricket Team is heading this initiative. IV. Provide financial assistance to deserving CHD Families for treatment of heart surgeries/interventions through public donations and Zakat at different public as well as private hospitals across Pakistan CHHRI envisions the establishment of a 125-bed center of excellence dedicated to the treatment of children suffering with CHD (Congenital Heart Disease). This center will utilize the services of the specialists in the fields of pediatric cardiology and pediatric cardiac surgery, including the advocates of this project. The project will focus on objectives like awareness, advocacy of CHD, training and capacity building of existing hospitals on a larger scale. We have gathered together a diverse group of like-minded individuals to act as the management for this project. The board of directors and advisors comprises of a team of professionals who have excelled in their various fields of expertise. Together, we formed the company “Pakistan Children’s Heart Foundation” which is registered with the Securities Exchange Commission of Pakistan (SECP) as a Section 42 Company Ordinance 1984. We have been awarded the license to operate as a NGO since 2012. Since the start of the institution, we have been funding heart surgeries required for the deserving patients under the title of Project Mohsin. We use zakat and donation to financially support Paediatric cardiac surgeries & interventions from private as well as public hospitals by contributing operation/intervention charges on behalf of the needy patients. Each surgery costs approximately PKR 350,000 to PKR 450,000 and may vary case to case. We have provided financial assistance to 1440+ pediatric cardiac surgeries/interventions with a cost of Rs 494 Million as of Apr 2019. Zakat is collected and spent as per Islamic Sharia law with a compliance certificate from two recognized authorities. All funding is classified as Zakat, donation or for hospital construction and kept and used accordingly through dedicated Bank accounts. Pakistan Children’s Heart Foundation Page 5 Version 6.0 PCHF-201904A Date: Apr 2019 Moreover, It is vital to point out that the public funding (Zakat and other donations) is only utilized for funding patients' surgeries and do not incorporate our administrative cost such as office rent, marketing cost or salaries. The approximate cost of the CHHRI is estimated to be PKR 2,+ million with a land requirement of 5 Acres. We have hired Ernst & Young to write a feasibility for the complete project and can be share upon need. We have multiple donors and pledges that have been continuously and diligently shown commitment to this project. Among these, a private housing Society in Lahore has stepped forward as an exemplary supporter and friend of the foundation. They have generously donated 1.1 Acres of land to the foundation. Meanwhile fundraising campaigns for the hospital construction are actively underway. The location of the land owned by PCHF is the following; We are in close discussion with UOLTH and Indus Hospital to have a land close to their hospitals building so that a dedicated heart hospital can be built near a general hospital. © Copyright Pakistan Children’s Heart Foundation

We aim to build awareness in general public regarding CHD and its treatment, provide financial support to the underprivileged families, building capacity in different public as well as private hospital where currently CHD procedures are performed and ultimately build Pakistan’s first dedicated Children’s Heart Hospital and Research Institute. Medical Missions- We look forward to supporting hospitals via medical missions through expert medical personnel. 3. Project CHHRI: Building a non-profit Children Heart Hospital & Research Institute (CHHRI) to serve as a center of excellence in pediatric heart surgery and research where treatment will be provided, along with the highest quality of care for children with heart disease, irrespective of their financial means. 1.4 Zero Admin Expenses At PCHF, all travel and administrative expense are borne by the board of directors personally or through their circle of friends or corporate donors and no management expense is paid for through the funds acquired. He is Pakistan's most successful test captain with 26 wins. Misbah Ul Haq has joined Pakistan Children’ Heart Foundation to build Pakistan’s first dedicated Children Heart Hospital & Research Center. 1.6 Advisory Board Members A brief profile of Board of Advisors of the hospital is as follows: 1.6.1 Dr. Masood Sadiq (Tamgha-e-Imtiaz) BSc (Pb), MBBS (Pb), DCH (Glasgow), MRCP (UK), FRCPCH, FRCP (Edinburgh), CCST- Pediatric Cardiology (UK) • Dr. Masood Sadiq is one of Pakistan’s leading pediatric cardiologist and was awarded Tamgha-e- Imtiaz (Medal of Excellence), the fourth highest decoration given to any civilian in Pakistan based on their achievement. Dr. Masood is currently Dean and Professor& Head of pediatric cardiology at The Children’s Hospital (the Institute of Child Health) and Punjab Institute of Cardiology. He has 22 years of work experience in medicine and has also served in the United Kingdom. 1.6.2 Dr. Mohammad Asim Khan MD, FRCS •Dr. Mohammed Asim Khan is a specialist in Pediatric Cardiac Surgery. He is a graduate of Liaquat Medical College, Hyderabad, Pakistan and is a fellow of the Royal College of Surgeons, UK (FRCS) and of the College of Physicians and Surgeons of Pakistan (FCPS). He did his general surgery and cardiothoracic surgery training from the Aga Khan University Hospital and then did advanced adult and pediatric cardiothoracic surgical training from the world-renowned Cleveland Clinic in the United States. He is currently an Assistant Professor at Children Heart Hospital and Institute of Child Health, Lahore. He has the following interest in the field of cardiac surgery: - Cyanotic heart disease - Arterial Switch - Aortic root surgery, Mitral and aortic valve repair - Adult congenital heart surgery - Single ventricle management 1.6.3 Dr. Syed Salman Ahmad Shah MD. He completed all his postgraduate training from the United States and did his general surgery residency from Indiana University and his cardiothoracic residency from UNM. He did his pediatric cardiac surgical training from Vanderbilt University and did an additional fellowship in advanced pediatric cardiac surgery from Emory University (Children’s Hospital of Atlanta). He is board certified general in cardiothoracic surgery by the American Board of Surgery and the American Board of Thoracic Surgery, and is also a Fellow of the American College of Surgeons and a member of the Society of Thoracic Surgeons. He has several publications to his credit relating to cardiac surgery and pediatric cardiac surgery. Currently he is working as an Assistant Professor of pediatric cardiovascular surgery at Children’s Hospital and Institute of Child Health Lahore, Pakistan. His special interests include infant and neonatal cardiac surgery, ECMO and cardiothoracic transplantation. Shah holds the following memberships indicating his education, training, professional qualifications, surgical competence and ethical conduct: - Diplomate American Board of Surgery - Diplomate American Board of Thoracic Surgery - Fellow of American College of Surgeons 1.6.4 Dr. Saeedah Asaf MD. Diplomate of the American Board of Anesthesia • Dr. Saeedah Asaf is a gold medalist graduate of Rawalpindi Medical College. She did her postgraduate training from the United States and did her anesthesia residency from Indian University and UNM. She did her pediatric Anesthesia fellowship from Vanderbilt University. Before coming back to Pakistan, she was an assistant professor of pediatric anesthesia first at the University of North Carolina and then at Vanderbilt University, where she was the director of the Difficult Airway Program. She is a diplomate of the American Board of Anesthesia and a member of the Society of Pediatric Anesthesia. She has also published several research articles related to pediatric anesthesia and cardiac anesthesia in children. She is currently an Associate Professor of Pediatric Anesthesia at Children’s Hospital and Institute of Child Health, Lahore. 1.6.5 Dr. Najam Hyder MBBS, FCPS (Peds.), FCPS (Peds. Cardiology) •Dr. Najam Hyder did his Bachelor of Medicine, Bachelor of Surgery (MBBS) from Allama Iqbal Medical College Lahore, Pakistan. He is a board-certified pediatrician and pediatric cardiologist and is a Fellow of the College of Physicians and Surgeons of Pakistan. He is currently working as Assistant Professor Peds. His postgraduate qualifications are MCPS, FCPS (Pak), FCPS (BD) and FPAMS. He did on job training in cardiac surgery from St. Thomas Hospital London and worked as General Surgeon till 1978 and from 1978 onwards as cardiac surgeon. I’ve been the founder & CEO (Volunteer) of the Pakistan Children’s Heart Foundation www.pchf.org.pk, an NGO working on building a charity heart hospital in Lahore Pakistan. I together with the team have raised PKR 435M to fund 1300+ children heart surgeries. I’m COO Advisor with Indus Hospital Lahore 600 bedded free of cost hospital as volunteer. I’ve also been Co-Founder and Sr. Consultant at Maison Consulting & Technology a successful company in Pakistan, UAE, USA and Saudi Arabia. He has provided Management Consultancy to different organizations and considers his steadfastness and ability to listen as his biggest assets. Pakistan Children’s Heart Foundation Page 15 Version 6.0 PCHF-201904A Date: Apr 2019 1.8.11 Muhammad Sadiq BA Ex Bank Officer -UBL 2 PCHF Projects 2.1 Project Mohsin Funding Heart Surgeries As of April 2019 PCHF has financially supported 1440+ surgeries of children across Pakistan (as indicated by the map below) under the umbrella of 'Project Mohsin'. Through this project we hope to help the economically underprivileged families get the medical care needed for their child. In addition, a patient may require more than one surgery for the heart to function normally. It is hence, that PCHF has set up a rigorous process of cross verifications to ensure that funding reaches the eligible families who are unable to stand the heavy surgery expenses. Once the entitled family is recognized, PCHF launches an immediate action to perform the surgeries by contributing operation/intervention charges on behalf of the patient. The surgeries are performed at various private and public hospitals by a team of professionals headed by Prof Masood Sadiq. PCHF uses Zakat money and other donations to financially assist these surgeries and not a penny is used from these public donations to cover the administrative cost of PCHF such as office rent, salaries or marketing expenses. Our Zakat funding is certified by Jamia Ashrafia. By ALLAH’s Grace, ever since the launch of PCHF, we have been receiving generous donations locally and abroad. However, as more people learn about PCHF, the need for public funding is growing by two-fold. UOLTH is offering hospital services completely free of cost to PCHF for deserving heart patients. PCHF hire ICU staff, Medical Doctors and arranges Surgeons and the OT Team to perform these surgeries. This has helped us reduce our cost to 2/3rd of the normal surgery cost. 2.3 Capacity Building Medical Missions; PCHF First Pediatric Cardiac Medical Mission performed 20 cardiac procedures at Children Hospital & 25 in Faisalabad Institute of Cardiology. A 14-member medical mission performed 45+ cardiac surgeries and interventions at The Children Hospital and Institute of Child Health Lahore & Faisalabad Institute of Cardiology (FIC) for 4 days. This was told by the dean Children Hospital Prof Masood Sadiq in a medical briefing. This has been used for the first time in any public sector hospital in Pakistan. The medical mission included renowned cardiac surgeons, cardiologists, intensivists, anesthesiologists, nurses, respiratory therapists and technicians from different countries mainly Qatar, Saudi Arabia, Canada and USA. Few of the members were Dr. Salahuddin Ahmad, Chief of Cardiac Surgery having 20 years of experience in his career and right now he is working in Hamad Medical Qatar since 2012. Dr. Younes Boudjemline, Paediatric Cardiologist & Director of the Catheterization Laboratories, Paediatric Cardiology, in Sidra Medicine Qatar having 15 years of experience in career in Paris France. Dr. Arif Hussain working as Paediatric Cardiologist in IWK Children Hospital & Associate Professor of Pediatrics Dalhousie University Canada serving from last 30 years. Dr. Saif Ur Rehman, Peads Cardiac Anesthesiologist in King Faisal Specialist Hospital & Director Morbidity and Mortality conference in Department of Anaesthesiology, University of Missouri USA having 14 years of experience in career. Ms. Julianne Koehler Metcalf, Clinical Educator, PICU currently working in Sidra Medical Centre Qatar and served in different institutions as Nurse Educator, Cardiac Intensive Care and Heart Institute in USA. Dr. Babur Rafique is an Associate Professor of Anaesthesiology and started his career in 2004 as assistant Professor and now serving in UT Health McGovern Medical School USA. 2.4 CHD Center of Excellence - CHHRI Project The Children's Heart Hospital & Research Institute is a brainchild of PCHF where we aim to build a modern and technically advanced children's hospital in Lahore that would provide treatment to children with congenital heart disease coming from across Pakistan free of cost.
