Operational Plan 2019-2020 Swansea Bay Community Health Council Chair’s Introduction This year we have continued to ensure that our operational plan has been informed by the widest possible number of people. We have listened to local people and to community groups across Neath Port Talbot and Swansea to ensure our focus is on the things that truly matter most to those whose interests we represent. We have set a challenging programme of activities, one that we cannot deliver alone. We hope that many people reading this operational plan will have something to contribute either as an individual or as part of a local group or organisation. We know that the year ahead will be an important one for our NHS services. From 1 April 2019 Bridgend will become part of Cwm Taf Morgannwg Health Board, which means that we will relinquish responsibility for scrutinizing health services in the Bridgend area to our colleagues in Cwm Taf Morgannwg CHC. We will also hear more about the development of regional services like Major Trauma and Thoracic Surgery and see more collaboration between Swansea Bay University Health Board and Hywel Dda Health Board. Our health board continues to review how services are delivered locally. In addition to the activities described below we will work with the health board and others as they involve and engage patients and the public in discussions about how services should develop to meet the needs of our communities. I hope that you will continue to show an interest in our activities, share your views and experiences and help us hear from more people, to ensure that everyone has the opportunity to drive improvement and influence developments in our NHS. Operational Plan 2019-2020 Swansea Bay Community Health Council About our Community Health Council OUR VISION People in Neath Port Talbot and Swansea know that they can share their views and experiences of the NHS and in doing so, can have a real influence on the shape of healthcare services. People understand and value the role played by Swansea Bay CHC in supporting them to be heard and in representing the collective interests of patients and the public. Our Strategic Framework 2017 - 2020 In order to make our vision a reality, the CHC movement in Wales agreed a strategic framework incorporating five key priorities for a three year period. These priorities underpin all our planned activities for 2019-2020. They are: ▪ Making every voice count ▪ Improving our influence and reputation ▪ Building and developing an effective learning and values based culture ▪ Valuing our members and staff, and ▪ Strengthening our national voice 5
Operational Plan 2019-2020 Swansea Bay Community Health Council Our equality objectives To help us deliver our five key priorities, we have also identified these equality objectives: ▪ We will work with others to find better ways to hear from everyone, including those who are in the most vulnerable situations and those whose voice might not otherwise be heard. ▪ We will adapt our approaches to reach those whose care is delivered out of hospital, and develop our resources and advocacy service to ensure that they are widely accessible, available and relevant. ▪ We will develop our plans with the public and with our partners so that they focus on the things that matter most and have the best chance of making a difference. ▪ We will build on our existing partnerships, and forge new ones, where working together increased our chances of making a difference. ▪ Our membership needs to reflect the diversity within the communities in which they live. We will routinely monitor our membership and develop targeted and more inclusive ways to attract new members. 6
Operational Plan 2019-2020 Swansea Bay Community Health Council Resources – Our people MEMBERS Swansea Bay CHC is made up of 24 members, local people who volunteer their time to lead our work and undertake activity on our behalf. Our members are appointed in accordance with our regulations: 12 Welsh Government appointments, 6 Local Authority nominations and 6 members drawn from the 3rd Sector. We also have the ability to co-opt members from our communities to support our work. We currently have 3 co-opted members. A full list of our members is provided at appendix 1. STAFF Swansea Bay CHC has a small team of paid staff whose role is to support our members and to deliver the advocacy function. A full list of our staff is provided at appendix 3. OUR GOVERNANCE The way in which we organise ourselves to carry out our activities is set out in regulations passed by the National Assembly for Wales. The activities we carry out on your behalf are co-ordinated and overseen by 2 local committees, Neath Port Talbot and Swansea. These 2 committees make up our Full Council. We are led by an Executive Committee which is ultimately responsible for what we do and how we do it. You can find out more about how we organise ourselves on our website, including details of our meetings. Day to day and operational management is the responsibility of our Chief Officer. Swansea Bay CHC is one of 7 CHCs in Wales and all are overseen by the CHC Board. The CHC Board has a specific responsibility to set national standards for all CHC activities and to monitor and manage performance against these standards. The standards set out what level of service you can expect from Swansea Bay CHC as we carry out our activities on your behalf. 7

These can be described as: ▪ Systematically visiting and scrutinising local health services ▪ Continuously engaging with the communities we represent and the health service providers serving those communities ▪ Representing the interests of patients and the public in the planning and agreement of NHS service changes ▪ Enabling users of the NHS to raise concerns about the services they receive through an Independent Complaints Advocacy Service 8
Operational Plan 2019-2020 Swansea Bay Community Health Council Deciding what we do and how to do it In deciding our activities for 2019/2020 we asked people to tell us what mattered most to them about NHS services in Neath Port Talbot and Swansea. We did this by speaking to people directly at a range of local events; via social media and through our website and newsletter. Alongside this, we also considered evidence from a range of sources: ▪ Published information including: Health Board and Welsh Ambulance Services Trust Performance Reports, Referral to Treatment Times, Patient Experience Reports & planning documents ▪ Issues raised and considered by CHC committees ▪ Issues identified by CHC visits ▪ Issues raised by cases supported through our complaints advocacy service ▪ Wider patient feedback and concerns information from the health board ▪ Information we receive from partner organisations or the outcomes from other review work In▪planning and prioritising our activity across our functions we used the following principles: ▪ Recognise and develop opportunities to promote equality and diversity, contributing to the delivery of the annual equality objectives as set out in the annual equality plan for CHCs ▪ Recognise and develop opportunities to take forward the priorities identified in the strategic framework for CHCs ▪ Focus our activity where we are best placed to make a difference for patients and the public ▪ Recognise where others are better placed to act and refer or coordinate as appropriate ▪ Work collaboratively wherever there is added value in doing so 9
Operational Plan 2019-2020 Swansea Bay Community Health Council ▪ Invest our resources wisely; acting in proportion to the issue and balancing activity across our statutory functions ▪ Seek to widen our engagement to build the evidence base on which to deliver robust representation and scrutiny of the NHS ▪ Leave capacity to respond effectively to developments, issues and challenges as they arise in year and to fulfill our statutory responsibility to engage with the NHS where change is proposed 10
Operational Plan 2019-2020 Swansea Bay Community Health Council How we work with others and why CHCs have a responsibility to systematically scrutinise the NHS. We do this in a variety of ways including looking at information about NHS services and asking questions of NHS leaders. CHC scrutiny activity includes undertaking a programme of visits to NHS services to see how health care is being delivered and to ask patients, family and carers about their experience. We recognise that others have a role to play in scrutinising the NHS and that the interests of patients and the public can best be served when we work in partnership to avoid duplication and add value. In delivering our activities we will seek to work with others who share our priorities, ensuring that in doing so we do not compromise our independence or ability to act solely in the interests of patients and the public. In order to properly represent your views, we need to routinely and regularly engage with you directly, through community groups and third sector organisations and jointly with the NHS to find out what you think about NHS services. We recognise the importance of working with 3rd sector and community partners to extend our reach within the communities we represent. We will continue to build partnerships that extend our reach. We will ensure that all sections of our community have a voice in the design and delivery of health services and access to support to raise concerns if they have them. 11
Operational Plan 2019-2020 Swansea Bay Community Health Council DURING 2019/2020 WE WILL FOCUS SPECIFICALLY ON WORKING WITH PARTNERS TO HEAR ABOUT: Access to NHS services in Neath Port Talbot and Swansea for people with physical and sensory impairment Young people’s and children’s experience of NHS services in Neath Port Talbot and Swansea How well NHS dental services serve the people of Neath Port Talbot and Swansea How NHS services in Neath Port Talbot and Swansea can better meet the needs of transgender people Mental health services for young people in secondary and community settings Ophthalmic services for people with particular ophthalmic conditions 12
Operational Plan 2019-2020 Swansea Bay Community Health Council Our local themes and priorities in 2019/2020 SCRUTINY AND ENGAGEMENT During 2019/2020 we will deliver a programme of engagement and scrutiny activity in support of a number of local priorities. Please note; due to internal pressures, some of these priorities from 2018/2019 have been carried forward to our plans for 2019/2020: Access to services for people with physical and sensory impairment Link with the disability forum to identify any ongoing work in this area, share information and avoid duplication. Work with the disability forum and relevant 3rd Sector/community groups to seek feedback from people about their experience of accessing local services. Collect feedback from patients at hearing aid clinics about any delays in treatment. Healthcare services in prison Continue to liaise with those involved in prison inspection to identify any ongoing work in this area and any specific issues identified. Undertake group meetings with the prisoners to gather their experiences. Availability of NHS dentists Map current NHS provision to identify any potential gaps and target engagement with specific geographical communities. Engage with people across the Swansea Bay University Health Board footprint about access to, and experience of NHS dentists and Out of Hours dental provision. Children & Young People Speak to Swansea Bay University Health Board youth forum and 13
Operational Plan 2019-2020 Swansea Bay Community Health Council relevant youth and community groups to help scope activity. Engagement to take place over the summer including at events for children and young people and visits to relevant clinical areas to hear about young people’s experience of NHS services. Transgender Services / Gender Equality Engagement to find out what the NHS could do to better support transgender people living in Neath Port Talbot and Swansea. Late cancellation of Trauma and Orthopaedics operations Liaise with those involved in waiting list times and alternative procedures being put in place to alleviate the impact of cancellation for the patient and the use of surgeon’s time when these procedures are cancelled. Mental Health Services: PTSD Scrutiny via the Mental Health and Learning Disability Sub Group. Seek information as to the reasons patients receive long delays in accessing services/counseling. CAMHS Continue to scrutinise via the Mental Health and Learning Disability Sub Group including seeking information and assurance on timely access to treatment and how parents are kept informed of the next treatment stages for their child. Seek feedback from Swansea Bay University Health Board on transition services for young people between the ages of 18 – 25 years. Community CAMHS Scrutiny via the Mental Health and Learning Disability Sub Group including seeking information on waiting times for non-residential CAHMS. Engage with services i.e Trehafod to gather feedback from those who use the service and their families. 14
Operational Plan 2019-2020 Swansea Bay Community Health Council Discharge policies and arrangements We will routinely seek information on delayed transfers of care from our hospitals around: 1. Patients who are being discharged from hospital without an effective package of care 2. Patients being discharged at odd times of the day 3. Patients being discharged where there is no family support 4. Patients being discharged with a lack of knowledge on how to access support. Breastfeeding support for new mothers Engage with breast feeding support groups to help scope activity. Seek information from the Health Board to identify the approach taken in Swansea Bay University Heath Board to support new mothers with breast feeding. GP Triage systems and arrangements We will continue to work with the Health Board through the GP Access and Sustainability Forum to scrutinise GP services and represent the interests of patients and the public. We will monitor GP surgeries and collect feedback from patients about their experiences of GP Triage systems. GP photo consultation and diagnosis We will continue to work with the Health Board through the GP Access and Sustainability Forum to scrutinise GP services and represent the interests of patients and the public. We will monitor GP surgeries and seek feedback on the use of photo diagnosis at GP surgeries. How often is this method used and under what circumstances. GP referral to alternative services We will continue to work with the Health Board through the GP Access and Sustainability Forum to scrutinise GP services and represent the interests of patients and the public. We will seek feedback from Primary Care on the take up of the ‘Common Ailment Scheme.’ We will seek clarity on whether treatment is delayed when patients are referred to other services i.e opticians 15
Operational Plan 2019-2020 Swansea Bay Community Health Council before being transferred back to the GP for secondary care referral. Secondary Care Ophthalmic Services Engage with patients at Outpatient clinics to gather feedback on the service provided through Swansea Bay University Health Board. Map provision of services locally and what treatments patients travel out of area to receive. Collect feedback from patients receiving treatment for particular ophthalmic services such as AMD. Telephone and Entertainment systems Map patient entertainment and payphone systems fitted in hospitals within Swansea Bay University Health Board. Do patients pay to view a television? We will continue to routinely scrutinise the performance of Swansea Bay University Health Board (SB UHB) by attending and representing the interests of patients and the public at the following Health Board (HB) committee meetings: ▪ SBU Health Board (SBUHB). ▪ SBUHB wide Quality & Safety Committee. ▪ SBUHB Infection Prevention and Control Group. ▪ SBUHB Food Service and Nutrition Development Group. ▪ Local Dental Committee (LDC). ▪ SBUHB Primary Care Access and Sustainability Forum. ▪ SBUHB Morriston Sustainable Transport Group. ▪ SBUHB Singleton Sustainable Transport Group. ▪ SBUHB NPT Delivery Unit: Stakeholder Reference Group. ▪ SBUHB NPT Delivery Unit: Management and Delivery Unit. ▪ SBUHB NPT Prudent Board. ▪ Singleton Hospital Services Delivery Unit Board. 16

We will also visit NHS services in response to information gathered in the year. 17
Operational Plan 2019-2020 Swansea Bay Community Health Council National Themes CHCs work together to drive improvement and influence policy on a national basis. We do so wherever our planning processes identify shared priorities and issues that affect people across Wales, and where working together will maximise our impact. During the year, people across Wales have told CHCs the things they are worried most about. IN RESPONSE, DURING 2019/2020 WE WILL WORK TOGETHER AND WITH OTHERS TO LOOK AT: Ophthalmology Orthodontics Cancer Pathways General Practice (GP) sustainability Mental Health 18
Operational Plan 2019-2020 Swansea Bay Community Health Council Complaints advocacy We have a statutory duty to provide an independent complaints advocacy service to support anyone living in their area to raise a concern about NHS services. Advocacy is about helping people to be heard. Our independent complaints advocacy service is there to help and support people to ensure that their experiences and concerns are not only listened to but are acted upon. Our independent complaints advocacy service offers confidential support, advice and guidance for people who want to raise a concern about any NHS provider. Our advocates aim to: ▪ Enable patients, carers and relatives to access information about their concerns and to offer advice on the options available; ▪ Support patients to put forward their concerns and complaints to the NHS for investigation. Our independent complaints advocacy service is provided in a friendly, confidential and supportive manner, encouraging the NHS to learn from individual patient experience to improve the service overall. In developing our work programme we are committed to maintain a programme of continuous improvement within the service which consistently delivers excellent feedback from those who use it. DURING 2019/2020 WE WILL DEVELOP OUR SERVICE FURTHER BY: ▪ Increasing our outreach activities. ▪ Developing our approach to peer review, including building in routine opportunities for informal peer support. 19
Operational Plan 2019-2020 Swansea Bay Community Health Council Responding to change proposals In order that local health services meet the existing and future needs of their population and communities, it is essential that they are planned, designed and developed together with local people, from the start. CHC’s have a specific responsibility to engage with the NHS whenever service change is proposed. Our role is to represent interests of public and patients and ensuring that NHS organisations engage or consult as appropriate with those affected. DURING 2019/2020 WE WILL: ▪ Meet every two months with the Health Board through the Service Planning Committee to consider proposed changes and scrutinise plans for engagement. ▪ Work with the Health Board where appropriate to engage with the public and patients in relation to proposed changes. ▪ Respond as appropriate to consultations regarding proposed changes. ▪ Scrutinise change proposals and processes generated by or on behalf of the NHS locally to ensure that patients and the public are engaged and consulted as appropriate. 20
Operational Plan 2019-2020 Swansea Bay Community Health Council Scrutiny and engagement In the year ahead, we will identify opportunities and seek to work with our partners to deliver a programme of targeted scrutiny and engagement activity in support of our local themes and priorities. This will include visits to hear from people whilst they are accessing NHS services. Operational Plan 2019-2020 Swansea Bay Community Health Council NHS bodies have establised a wide range of committees and working groups to develop plans, monitor progress and scrutinise performance across all aspects of NHS care. Throughout the year, our members and staff will attend many of these committees and groups to represent your interests, providing constructive challenge and stimulating debate on the things that matter most to you. The NHS committees and groups we will be involved in during 2019/2020 are set out at appendix 2. Our CHC’s local committees provide a regular opportunity to consider the development and delivery of NHS services within our individual counties of Swansea and Neath Port Talbot. Rountinely attended by county based NHS managers and representatives from the Welsh Ambulance Service Trust (WAST), these meetings will provide an ideal opportunity for people to hear about key issues affecting local NHS service throughout the year. DURING 2019/2020 WE WILL DEVELOP OUR SERVICE FURTHER BY: ▪ Continuing to strengthen our information sharing and reporting to support wider CHC activities ▪ Routinely raising awareness of our complaints advocacy service through CHC members’ local community networks. ▪ Developing our approach to peer review. ▪ Building our arrangements to inform regular updates with key partners, including the Public Services Ombudsman for Wales (PSOW) and Healthcare Inspectorate Wales (HIW). 22
Operational Plan 2019-2020 Swansea Bay Community Health Council Responding to matters as they arise Because our volunteer members live within the communities they represent, we are often able to listen to local concerns and issues as they arise so that we can respond quickly and appropriately. Some people who contact us need the on-going support of our complaints advocacy service. Others may have a specific matter that can be addressed immediately. We have developed good relationships with the NHS so that we can help resolve problems where possible at an early stage. Sometimes a matter raised with us will need further action, perhaps by finding out more from our health, through scrutiny at our CHC committee meetings, or by carrying out a visit to see if a concern raised with us is shared by others. DURING 2019/2020 WE WILL: ▪ Continue to respond to enquiries and correspondence from individual members of the public or others about NHS matters. ▪ Where appropriate, respond to requests to provide feedback and information about NHS services, e.g., through calls for evidence or consultations. 23
Operational Plan 2019-2020 Swansea Bay Community Health Council Developing ourselves to deliver effectively We have identified a range of measures which will help us to improve our performance against the National Standards for CHCs, across our four statutory functions. DURING 2019/2020 WE WILL: ▪ Continue to build links with 3rd and community sectors. ▪ Develop a more robust approach to evaluating activities. ▪ Increased member development opportunities. ▪ Develop our ways of working in relation to duties under information governance and equality legislation. How to get involved in our activities or find out how we are getting on with delivering our plan We are always keen to hear from anyone who wants to share their views and experiences of NHS services. You can contact us using any of the details included at the end of this plan. You can also tell us if you would like us to include you on our distribution list for surveys and updates. If you are part of a group or organisation and would like to work with us on any of our activities we would love to hear from you. We are also happy to come and hear from any groups who have a story to share about any aspect of NHS services We publish a calendar of meetings on our website so that you can see when and where we will be discussing key issues affecting NHS services. Our committee meetings are held in public so you are welcome to attend. Please let us know if you would like to receive a set of committee papers in advance. We will publish all our reports on our website. We publish a monthly brief that tells you what we have been doing, what we have said in our reports and committees and how the health board has responded. 24
Operational Plan 2019-2020 Swansea Bay Community Health Council We publish a newsletter 3 times a year which gives more detail about our upcoming activities and the impact we have made. We are active on twitter and this year will be building a presence on Facebook and Instagram. We publish an annual report which gives a full account of the work we have done and how it made a difference. And… we are always looking for new members! The vast majority of our work is undertaken by local volunteers who have an interest in helping their communities to have a say in how their NHS is designed and delivered.
