As part of the Gwent Frailty Programme, referrals to the rapid HDS service are received via members of the intermediate care team. The prevention of admission to hospital (PATH) service is a fast response, time- limited service, that supports older people in their own homes, during a time of temporary incapacity, for example, after a fall at home. Following referral by a GP or medical staff, the service offers up to 7 days of support, which may include meal preparation, dressing/undressing, putting to bed, shopping/collection of prescriptions, basic household tasks, such as laundry, and help with personal hygiene, including assisting with the toilet. The PATH service is part of the Gwent Frailty Programme and is 8
provided free of charge 7 days a week. Parents were welcomed off the cold yard into a friendly environment to chat freely regarding issues at home, school or to just have a good catch up. Tea, Coffee and chocolate biscuits were enjoyed, especially by the younger siblings! The plan is that at each Forum, Parents will be invited to set the agenda for discussions. In future meetings we can invite guest speakers in, or have any advice needed to hand. 20
