PART A: PROCESS OVERVIEW 1 SUMMARY OF ACTIVITY BEING SOUGHT MPHN are seeking Expressions of Interest (EoI) from General Practices within the MPHN catchment area to participate in the Murrumbidgee Winter Strategy 2021. This activity will support general practices to keep their most vulnerable patients well and reduce their likelihood of being hospitalised over the winter period. The Winter Strategy 2021 primary care approach involves: identification of high risk patients within the practice; actively developing or updating care plans prior to the flu season (including sick day plans); ensuring follow-up for vaccination; empowering patients to manage their condition(s); the use of HealthPathways; and providing links to providers / groups that can provide support. The overarching winter strategy will also target improved communication between acute and primary care for identified vulnerable patients and will be supported through a comprehensive marketing and media campaign promoting hand hygiene and flu vaccinations. 2 Timeline for the Expressions of Interest (EOI) process Milestone Date EOI documents released via email to all General Practices 2 February 2021 within the MPHN catchment area CLOSING DATE for EOI (via tenders@mphn.org.au) 23 February 2021 Evaluation of submissions and contract negotiations Commencing 25 February 2021 This timeline is provided as a guide. 4 About Murrumbidgee Primary Health Network MPHN is one of 31 local and independent PHNs established nationally to support the primary health care sector. MPHN works to: support the development of a local sustainable health care workforce through providing quality improvement and professional development for our primary health care workforce of GPs, nurses, pharmacists, allied health care professionals and specialists; ensure efficient and effective delivery of high quality patient-centred care and improve health outcomes, particularly for those at-risk or vulnerable populations. Part of this role is to integrate local services and systems and improve coordination of care; and continually monitor, assess and analyse the health needs of our region through our health needs assessments to ensure we are able to commission services addressing the health needs in the community. All PHNs have been set seven key priority areas guiding our work including: Mental health Aboriginal and Torres Strait Islander health Population health Health workforce Digital health Aged care Alcohol and other drugs MPHN has a population of more than 242,000 people, living across 521 communities in a land mass of 124,413 square kilometres. PART B: STATEMENT OF REQUIREMENT 5 Program overview 5.1 Background The Murrumbidgee Winter Strategy 2021 is a joint initiative with the Murrumbidgee Local Health District (MLHD), General Practice and NSW Ambulance and aims to: 1. Support general practices to keep their most vulnerable patients well and reduce their likelihood of being hospitalised in winter; 2. Improve respiratory hygiene and vaccine uptake, slowing the seasonal epidemic of infectious respiratory conditions; 3. Establish an efficient and effective process for transfer of care from hospital to community during winter. The activity in general practice will build on the achievements of: the 2019 Murrumbidgee Winter Strategy which supported over 240 patients from 11 general practices across the region the 2020 Murrumbidgee Winter Strategy which supported 470 patients from 19 general practices across the region. Participating general practices were able to implement various strategies to support their most vulnerable patients including teach back appointments, utilisation of SMS reminders and the winter wellness bags. The 2021 strategy will have an increased focus on ensuring that appropriate communication and monitoring is in place for enrolled patients who may present to acute care services, ensuring admission and discharge planning includes the patients GP enabling follow up care in the community. As with previous years, MPHN will continue to implement comprehensive communications and marketing campaign, promoting hand hygiene and flu vaccinations. The advertising campaign utilises a multimedia approach via TV and social media and posters to educate community on the importance of flu vaccination and slowing the spread of infectious respiratory conditions. 5.2 Opportunity on offer MPHN are seeking Expressions of Interest from General Practices within the MPHN catchment area to participate in the Murrumbidgee Winter Strategy 2021. As part of this strategy, General Practices will be required to undertake the following core activities: 1. Practice preparation: a. Complete a practice planning guide. b. Identify the team and their responsibilities. c. Update PenCS software to support rollout and reporting for the program 2. Create a winter watch list: adult patients >18 years with one or more condition who are likely to benefit from heightened care. 3. Register patients with the practice as part of the Winter Strategy watch list. 4. Monitor watch list patients throughout to ensure: EOI – Murrumbidgee Winter Strategy 2021 - Guidance Document | 5 of 10
a. health conditions and care plans have been recently reviewed b. self-care is promoted and patients have a sick day action plan (SDAP) c. patients are monitored regularly by their practice team d. specialist or other services are involved if needed e. there is a contact person at the practice for each patient so they can discuss concerns and receive care in a timely way f. patients at risk are supported throughout the thunderstorm asthma season 5. Integrate with local Multi-Purpose Service (MPS) / hospital 6. Participate in community of practice events 7. Provide a minimum data set to MPHN and gaining patient consent for evaluation 8. Participate in an evaluation which may include a survey and interviews for staff and patients. 9. Provide regular updates to MPHN on local implementation. 5.3 Start-up workshop and community of practice Participating general practices will be required to participate in a start-up workshop to be held early March 2021 in Wagga Wagga. Online options to participate in this activity will be available. Practices will also be required to participate in a community of practice to enable the sharing of ideas and progress with winter strategy activities. These community of practice meetings will be held via teleconference on a monthly basis. At least one GP and at least one other clinician (practice nurse or allied health professional) OR at least one GP and the practice manager are expected to attend the ongoing community of practice meetings (teleconference). 6 Eligibility to apply All General Practices within the MPHN catchment area are eligible to apply for funding to participate in the Murrumbidgee Winter Strategy 2021. 7 Funding The funding on offer for this activity is: $8,000.00 (GST exclusive) per General Practice. Funding is based on an anticipated enrolment of 25 or more patients per practice. Funding is for resourcing the practice to undertake this activity which may include increased staffing and/or the provision of patient resources, as identified by each practice in their proposed activity plan. Payments will be made in two instalments; 15 April and 15 July 2021. 8 Evaluation and provider support 8.1 Evaluation Pricewaterhouse Coopers has been engaged by MPHN to undertake an evaluation of the Murrumbidgee Winter Strategy 2021. All General Practices participating in the Winter Strategy will be EOI – Murrumbidgee Winter Strategy 2021 - Guidance Document | 6 of 10
required to participate in the evaluation which may include interviews and/or facilitating the completion of surveys. 8.2 General practice support MPHN will be seeking to engage an external provider to undertake additional targeted support to general practices participating in the winter strategy program. The provider will support practices with the development and implementation of strategies to improve current processes and systems for managing their cohort of vulnerable patients throughout the winter period and will facilitate the community of practice meetings. 9 Contract term The term of the contract to be awarded will be from contract execution to 31 October 2021. 10 Monitoring and reporting requirements Upon execution of the contract, general practices will be required to provide an activity plan which will detail the following: the practice team and their responsibilities Immunisation and cold chain management planning for winter creation of a winter watch list: adult patients >18 years with one or more condition who are likely to benefit from heightened care process for registering patients with the practice as part of the Winter Strategy watch list and ensuring the following monitoring occurs: a. health conditions and care plans have been recently reviewed b. self-care is promoted and patients have a sick day action plan (SDAP) c. patients are monitored regularly by their practice team d. specialist services are involved if needed e. there is a contact person at the practice for each patient so they can discuss concerns and receive care in a timely way MPHN have a system of regular reporting for all commissioned service providers to ensure programs are achieving the outcomes intended. General Practices will be required to submit progress reports against the approved activity plan and minimum requirements including, highlights and challenges, risk management, and incidents; and financial declaration at the conclusion of the contract period. Practice will be required to provide a minimum data set to MPHN for contract monitoring and evaluation purposes. Practices will be provided with training on how to collect and submit the data and will include clear guidelines and definitions on what data needs to be provided. Data entry will be through an online reporting system. At a minimum the following deidentified data will need to be collected: Has a Consent form been completed Does the patient have a Sick day action plan? Vaccination status Number of GP visits Number of hospital presentations Referrals to other support services or programs Number of practice contacts with the patient Age and gender 11 Developing and submitting your Expression of Interest An EOI Response Form (in Word format) was emailed with this Guidance Document. General Practices are invited to complete the Response Form and submit it in accordance with the submission details outlined below. Please note that the Response Form contains mandatory requirements that should be considered carefully before your practice decides whether or not to proceed. Submission Details Email: Please email your completed EOI Response form to: tenders@mphn.org.au File names: Please include the name of your practice in the file name of any attachments. The response form template includes the following specification for pre-conditions for respondent organisations. # Pre-condition criteria Compliance i. Identification of a lead General Practitioner for the activity Name ii. Declaration of any matter or issue which is or may be perceived to be or Yes / No may lead to a conflict of interest regarding the submission or participation in the supply of the Services described. iii. I understand that at least one GP and at least one other clinician (practice Yes / No nurse or allied health professional) OR at least one GP and the practice manager are expected to attend the ongoing community of practice meetings (teleconference). # Evaluation criteria i. Within your practice, who will be responsible for managing the project activities working closely with the lead General Practitioner? ii. Provide a brief overview of any current activities your practice implements to support those at risk during winter and how your involvement in the Winter Strategy 2021 Program would add benefit to your patients. iii. Utilising your practice data, please provide an estimate on the number of patients in your practice that would benefit from increased support and monitoring during the winter period. 12 Conditions of this Expression of Interest (EOI) (Conditions) General 1. Nothing in the EOI gives rise to a process contract or any other right. Your Proposal 2. You must: a. familiarise yourself with this EOI document and documents included with or referred to in this document; b. ensure your Proposal complies with the requirements set out in the EOI; c. satisfy yourself that you are not participating in any anti-competitive, collusive, deceptive or misleading practice in or in relation to Your Proposal; d. submit your Proposal as set out in the EOI; e. be an entity with the capacity to contract or a natural person at least 18 years of age with legal capacity; and f. have an Australian Business Number (ABN). 3. You agree that Your Proposal is open for us to accept for 60 working days (or any longer period specified in the EOI) from the Closing Time. 4. Verbal representations by or on our behalf do not bind us. 5. Your participation in the EOI process is at your sole risk and cost. 6. Proposals may not be amended after submission. Evaluation 7. We will only agree to extensions to the Closing Time in exceptional circumstances and, if approved, the extension will apply equally to all Respondents. 8. We will not consider any Proposals received after the Closing Time unless the Proposal is late due to Our mishandling or error. 9. We intend to evaluate proposals in accordance with these Conditions and the EOI. 10. We will not evaluate any Proposal that does not meet a Mandatory Condition in the EOI. 11. We reserve the right to contact your referees, or any other person without notifying you. 12. We may engage individuals outside our organisation to assist with evaluation or evaluate Proposals. 13. We reserve the right at Our discretion to: a. reject any or all Proposals; b. evaluate or not evaluate any or all Proposals; c. accept or not accept any or all Proposals; EOI – Murrumbidgee Winter Strategy 2021 - Guidance Document | 9 of 10
d. negotiate with one or more Respondents; e. seek clarification or additional information from, or enter into discussions and negotiations with, any or all Respondents; f. terminate or suspend or alter the EOI for any reason; g. resume a terminated or suspended EOI; h. accept Proposals in relation to some and not all of the scope of activity described in the EOI, or contract with one, more than one or no Respondent on the basis of the Proposals received; i. take any course of action We consider appropriate in relation to the EOI. 14. We will tell all Respondents of our final decision. Legislative and other requirements and privacy 15. You acknowledge that We may be subject to requirements under Our Funding Deed. This may involve disclosing Your details and details about any contract We enter with You. The Freedom of Information Act 1982 may apply to information we hold about You. 16. You agree to comply, and to ensure that Your officers, employees, agents and subcontractors comply with the Privacy Act 1988 (Cth). 17. All applications and any accompanying documents become Our property. Ownership rights of all information, reports or data remain unchanged. Probity and confidentiality 18. You must tell us immediately if any actual, potential or perceived conflict of interest arises. 19. If You consider that We have failed to provide a fair right to be considered as a Respondent or that You have been prejudiced by a breach of these Conditions, You must provide us immediate notice including details of the alleged failure or breach. 20. Attempting to exert influence on the outcome of the assessment process by lobbying Us, directly or indirectly, will be grounds for disqualification from the EOI process. 21. You must identify any part of your Response that you consider should be kept confidential and provide reasons for that. We will take reasonable steps to treat your information as confidential if it is consistent with the requirements of our Funding Deed to do so. Closing Time means the closing time specified in the EOI Conditions means these conditions Funding Deed means our funding deed with the Commonwealth of Australia Mandatory Condition means a condition of the RFP that is mandatory Proposal means a proposal submitted in response to the RFP Request for Proposals or RFP means any request for proposals, tenders or expressions of interest we issue. Respondent means any person or entity that submits a Proposal in response to the RFP We or Us means firstHealth Limited trading as Murrumbidgee Primary Health Network You means you the Respondent.
