Dear Goodyear Team Member, We are excited to present the 2020 Benefits Guide. It summarizes the key benefits offered to eligible team members. The well-being of our team members is a central component of our “Whole Employee” philosophy, therefore a focus on well-being continues to be a top priority for the City. In pursuit of our focus on well-being, we continually assess our benefit offerings to ensure our benefits effectively support our goals. The city offers a wide-range of benefits and well-being initiatives to help you maintain or achieve optimal health. Our benefit package includes low-cost medical, dental, vision, life insurance and access to the Goodyear Wellness Center. The programs are designed to support all aspects of your total well-being and that of your eligible family members. This year, we are pleased to introduce the following benefit enhancements: • HMO plan will no longer require referrals and will have a larger network • Three new Telehealth options are now available • Dental HMO plan will have a larger network • The Dental plan year will now align with the fiscal year (July1st- June 30th) • Avesis Vision contact lens allowance will increase from $110 to $130 • HSA contribution limit increased from $3,500 to $3,550 for individual and from $7000 to $7,100 for family • Medical Flexible Spending Account contribution maximum increased from $2,700 to $2,750 • Supplemental Life insurance — additional $10,000 guaranteed issue for employees who have a voluntary life insurance of $90,000 or less during open enrollment only • Elections to increase supplemental life insurance now available year-round (evidence of insurability will be required) Although, the city will experience a 3.0% cost increase for medical insurance, because of the method that we will begin capturing the expenses for the Wellness Center, telemedicine, wellness initiatives and other healthcare benefits, employees will experience a slight decrease in their medical premiums. Also this year, we are pleased to again have ZERO increase in costs for our dental and vision insurance plans. Health Savings Account (HSA) If you enroll in a city high-deductible health plan (HDHP), you may be eligible to open and fund an HSA account. An HSA is a personal health care savings account that you can use to pay out-of-pocket health care expenses with pre-tax dollars. Your contributions are tax free, and the money remains in the account for you to spend on eligible expenses no matter where you work or how long it stays in the account. CITY OF GOODYEAR CONTRIBUTION TO EMPLOYEE HSA HSA (1750) Employee Only $500 Employee + Spouse or Child $1,000 Employee + Family $1,000 Note: New Hire contributions are prorated based on hire date. The city contribution to an employee’s HSA is noted above. The employee may also elect to make contributions into the HSA. To help you save money on health care and dependent care costs, the city of Goodyear is happy to offer our employees Flexible Spending Accounts. The purpose of a Flexible Spending Account is to allow you to set money aside on a pre-tax basis to cover expenses that are not otherwise covered under a traditional medical, dental or vision plan. The city of Goodyear pays 100% for all administrative costs associated with the Flexible Spending Account. There are two types of Flexible Spending Accounts available to you: a Health Care Reimbursement Plan and a Dependent Care Reimbursement Plan. Both plans are explained below. Healthcare FSA Health Care Spending Accounts offer employees the opportunity to pay for eligible out-of-pocket medical costs with pre-tax dollars. The maximum annual allowable amount is $2,750. Health Care Spending Accounts cover an endless list of eligible expenses for which an employee can seek reimbursement. If you are covered by a High Deductible - Health Savings Account Plan, you are eligible to enroll in a “Limited” Healthcare FSA which allows you to use these funds for dental and vision expenses. Medical expenses must be incurred by June 30 of each plan year and submitted by September 30 to be eligible for reimbursement. Participants will be eligible to carry over up to $500 of the previous year’s balance into the next plan year.” ! If an employee terminates employment prior to incurring a planned expense, the money is FORFEITED. However, the employee could elect to continue participation through COBRA and submit expenses past their termination date. Dependent Care FSA Employees can use this account to pay for eligible dependent care expenses with pre-tax dollars up to $5,000 per family per year. The option is a replacement for the federal childcare tax credit. Each plan year, employees elect the amount they will use pre-tax for dependent care expenses to be deducted in equal increments from their paycheck. The monies are held in a separate account for each employee until the employee submits an eligible dependent care expense claim. Unlike the Health Care Account, only the amount that has been deducted to date can be reimbursed to participants. Important Notes: • Both spouses must work or attend school full time to take advantage of this benefit. One very important point to note is that once you determine what your pre-tax elections will be for the year for Health Care reimbursement and Dependent Care reimbursement, you cannot change those elections unless you have one of the following qualifying events occur: • Birth • Death • Adoption • Marriage • Divorce • Change in employment status of a spouse ! Unless you have a qualifying event, you must wait until the next open enrollment period to make changes to your contribution amount. An FMLA eligible employee is one who has been employed by the City for at least 12 months (not necessarily continuously) and has actually worked for at least 1,250 hours not including holidays, sick time, vacation time, or worker’s compensation during the 12 month period immediately preceding the commencement of the leave. Exempt employees are presumed to have met the 1,250 hours of service required for eligibility if they have worked for the city at least 12 months. The Family and Medical Leave Act (FMLA) gives eligible employees up to 26 workweeks of unpaid leave within a single, twelve month period: • For an employee to care for his or her spouse, son, daughter, parent, or next of kin recovering from an injury or illness suffered while on covered active duty in the armed forces. Such illness or injury must render the covered family member unfit to perform the duties of the individual’s office, grade, rank or rating (hereinafter referred to as serviceperson caregiver leave). There is a six-month waiting period, and the LTD administrator must approve the disability claim before you receive disability compensation. Long-Term Disability – Sworn Fire/Police only Disability insurance can help replace lost income and make a difficult time a little easier. Retirement Benefits It may seem like retirement is far away, but now is the time to plan for your future. If you are not already saving for retirement, the time to get started is now. 65% of households are “at risk” of not having enough to maintain their living standards in retirement. The city of Goodyear contributes to the following benefits to help you be one of the 35% who have enough to maintain your living standards in retirement. Arizona State Retirement System (ASRS) The ASRS Defined Benefit Plan is the primary plan for ASRS members and will provide, upon retirement, lifelong monthly benefits. Your ASRS pension is built through a “cost-sharing” model. Both you as the employee and your ASRS employer contribute equally towards your retirement. Your personal contributions fund only a relatively small part of your benefit. When you retire, you will recover your own contributions within approximately three to five years from the start of your benefit payments. Employer contributions and earnings on investments of the ASRS fund are used to provide the remainder of your lifelong retirement benefit payments. The ASRS is tax-qualified under section 401(a) of the Internal Revenue Code. Member contributions to the ASRS are exempt from federal income tax withholding under section 401(h) of the Code. Tuition Assistance All regular city employees who have fulfilled the initial trial period requirements for their job are eligible for tuition assistance for attendance at a regionally or nationally accredited college or university and qualifying certification programs. Courses eligible for tuition assistance shall be job-related to an existing position in the city and part of an approved degree program or a qualifying certification program. COBRA Coverage The right to COBRA continuation coverage was created by a federal law, the Consolidated Omnibus Budget Reconciliation Act of 1985 (COBRA). COBRA continuation coverage can become available to you and other members of your family when group health coverage would otherwise end.
