Welcome to Our Home The resident and staff members of the Dean Maxwell Community welcome you to our home. “The purpose of this community is the enablement of all our members to live healthier and more fulfilled lives”. Background The Dean Maxwell Community Unit nestles in ‘The Valley’ in the shadow of Knockbrack Hill and Saint Cronan’s Church in Roscrea. The Unit is called after The Reverend Dean Thomas Maxwell who was Parish Priest in Roscrea at the time the site was handed over by the Parish. The Dean Maxwell first opened in 1974 and has been home to many people over the years. New services have been developed. In 1991 the Day service started, this Day Unit is now open 7 days a week. The Respite service was developed and this gives a break to carers and the people they are caring for. The Laurels was opened in 1998 for people who require special support. The Dean Maxwell continues to grow and develop and we aim to provide a highly professional and flexible service to the people of Roscrea. 1
Dean Maxwell Community The Dean Maxwell Community consists of Resident, Day Unit and Staff Members. Family, friends and voluntary helpers are very important to the Community. Resident members come from the local Roscrea community, as far as Templemore on one side and Cloughjordan and Borrisokane on the other. Some members live here on a continuing basis and others come for a short break. Day Unit members come from the same areas as the Residents and it is wonderful when people from local communities meet together every week. Some members attend every day and some every week. Staff members are a dedicated team who are committed to providing a service that will facilitate members to live their lives in the most active and happy way possible. They aim to promote health, independence and individuality and to provide for all members’ needs in a sensitive and caring manner. Bed allocation is decided by a case conference team who meet once weekly. Referral to Case Conference is by family doctor or hospital doctor in consultation with patient and relatives. There is a basic charge for extended care, this is reviewed every year. For up to date information inquire at time of admission. There is no charge for respite or palliative care Referrals to the Day Unit can be by the person themselves, their family, GP, Public Health Nurse or hospital doctor or nurse. There is a small charge for day service. 2
Your Care and Support as a member of the Dean Maxwell Community This care team is lead by Matron, Anne Keevey who is supported by Nurse Manager Margaret Stanley. The care team consists of Nurses and Health Care Workers, who work together to support you and provide you with 24 hour care, the Occupational Therapist, Physiotherapist and Podiatrist are among the health professionals who provide valuable support and advice. By providing their essential services the Catering, Administrative and Maintenance staff complete this team. Medical Care If you have previously lived in this locality your own General Practitioner can continue to care for you. If not, you may choose a local GP to continue your care. If you are coming for a short/respite break, please bring your prescription. DO NOT GET THE PRESCRIPTION FILLED BEFORE YOU COME IN. Please tell us if you have any side effects from or allergies to any drugs. Staff members, Dean Maxwell Community 3
Your Admission Whenever possible we like to meet you before your admission so that we can get to know one another and give you some background information about your new home. We can also learn a little about you and that will help us. It will let us know the kind of help and care you need and would like, and how we might make your entry to our Unit more comfortable. Each Resident here has a named nurse and care worker; they are the two members of staff who will take a special interest in helping you settle in. Their names will be on your file and over your bed. The staff on duty at anyone time will manage your everyday needs but your named nurse and care worker will take care of your special needs including discussing and planning your care with you. They will arrange to show you the ropes and help you settle in. They will answer any questions you have and will be a contact for your family if they want to share any information with us. New members are encouraged to keep in regular contact with their home communities. They will be facilitated to avail of every opportunity to move out and about and attend family and community events. Saol Folláin Sásúil Care Plan Each resident, on admission will have their own individual care plan drawn up. This plan of care is agreed between nurse, patient and a named relative if desired. Your care plan will aim to make the best of your time here, improve your general health and give you the best Quality of Life.
