MESSAGE TO THE COMMUNITY Anderson Hospital continues to focus on the health of our community and their personal and family health care needs. Anderson also continues to expand its portfolio of clinical services, add new physicians to the medical staff, and increase the number of sites to access care. Anderson Hospital understands the responsibility we have to our community in improving and maintaining the health of its residents. We bring this community report forth with our assessment process, outcomes, and plan for the upcoming years. Although we have been in the midst of the COVID-19 pandemic, we acknowledge that there are also other health concerns. We have spent the last year reviewing information and data about the health of our service area. The Community Heath Needs Assessment Survey was disseminated to community members who live or work in Anderson Hospital’s primary service area and 315 surveys were completed. Upon completion of the survey, and after comparison of prior Community Health Needs Assessments, Anderson Hospital has developed an action plan to address the top health priorities with implementation plans for each. The top health priorities and community needs Anderson Hospital will address in 2022-2025 are listed below. ANDERSON HOSPITAL Anderson has been making a difference in the healthcare of Madison County residents since 1977. We are an independent, not-for-profit hospital dedicated to creating a health care setting in which quality of life and service excellence are experienced by our patients, their families, the medical staff, and the community. 2019-2021 TOP HEALTH PRIORITIES Anderson Hospital’s prior Community Health Needs Assessment was conducted in 2018 and four health priorities were identified based on the needs of the community. A Community Health Plan was implemented for 2019-2021 after further review and adoption by Anderson Hospital’s Board of Trustees. These health concerns are listed below with some of the accomplishments that have been achieved over the last three years. Cancer In spring of 2019, the Endoscopy lab staff provided literature to raise awareness of the increasing rate of colon cancer in young adults. In addition, staff was available for questions regarding the recommended age for screenings and the various screening tools. In fall of 2019, the Cancer Center Director, Physician, and Director of Radiology met to plan for the launch of lung cancer screenings in 2020. The number of screenings continues to grow in volume due to referrals. Various screenings and education were offered throughout the service area and promoted in the biannual Health and You magazine. Anderson Hospital was vigilant in providing a smoking cessation program, as well as education on second hand smoke and smokeless tobacco to employees, patients, and members of the community. Cardiovascular Disease Starting in 2019, Early Heart Attack Care education was provided to Anderson Hospital EMS System providers. EHAC educational handouts were brought to all community events and through 2020, were provided in every BLS and ACLS course. In spring of 2020, Zoom telehealth appointments were established for medical nutrition therapy and diabetic self-management training services. As COVID-19 made its appearance, telehealth services continued to be highly utilized. Anderson Hospital provided heart health education to community members and employees through health fairs, educational offerings, and various health screenings. Diabetes Management Services grew in offerings, including the development of the Diabetes Support Group and a partnership with the Walmart Neighborhood Market to provide guided supermarket tours with the Diabetes Educator. 7
2019-2021 TOP HEALTH PRIORITIES Mental Health Anderson Hospital staff received education on mental health awareness, with managers, directors, and supervisors receiving additional resources on how to respond to staff presenting in crisis. All staff also received education on suicide risk and prevention. To better recognize and understand patients’ and community members’ difficulties with postpartum mood disorders, all Obstetrics employees received education and training on these disorders. Anderson Hospital continues to support employees and community members through a large offering of support groups, including Diabetes Support Group, Grief and Loss Support Group, Airlink with Heart Cardiopulmonary Support Group, Ostomy Support Group, Stroke Support Group, SHARE Breast Cancer Support Group, and SHARE Infant Support Group. Due to the pandemic outbreak, Anderson Hospital focused on providing support to staff, as members of our community. Support and education focused on stress management, depression, anxiety, and the benefits of sleep, a healthful diet, and physical activity. Employees and their families were encouraged to utilize the Employee Assistance Program (EAP), Call4Calm, Chestnut Health Systems, and Emergency Childcare Resources. Substance Abuse Anderson provided hands-on and virtual education and awareness on substance use and abuse throughout local schools and organizations in our service area with education including alcohol, tobacco use, and vaping. Anderson Hospital continued to host meetings for Partnership for Drug-Free Communities and developed virtual offerings once the pandemic arose. This coalition continued to distribute brochures, resource cards, and posters to physician offices, community members, and patients, to educate on local drug and alcohol related resources available. As a healthcare partner, Chestnut Health Systems provided multiple Narcan trainings to the community throughout the year(s). Anderson Hospital staff began utilizing new substance abuse screening tools that allowed for physician notifications on high-risk results. This new system would then result in referrals to the Care Coordination team who would provide the appropriate and available resources to patients in need. Anderson created and offered a Freedom from Smoking cessation class to aid in the quitting process of tobacco products, including e-cigarettes. 8
COMMUNITY BENEFIT Community Benefit The Community Benefit is a planned, managed, and measured approach to the organization’s participation in meeting identified community health needs. Anderson Hospital offers education and screening programs that target health priorities, supports the community with opportunities that promote a healthy environment, and partners with area colleges and universities to provide student and volunteer experiences. Anderson Hospital tracks and maintains the Community Benefit reporting in a database called CBISA. Throughout the years, this database has become a comprehensive method for Anderson to measure the impact we have made on the communities we serve. Community Health Needs Assessment The Community Health Needs Assessment, reassessed every three years, is defined by the Public Health Accreditation Board as a systematic examination of the health status indicators for a given population that is used to identify key problems and assets in a community. The assessment includes a variety of resources and processes used to collect and analyze data through engagement of our community and collaborative partnerships. Listed below are the steps taken through the assessment process. The health plan forms an Implementation Strategy to improve the health status of our surrounding communities. The steps in developing the Community Health Plan are listed below. COMMUNITY HEALTH NEEDS ASSESSMENT Anderson Hospital began the Community Health Needs Assessment process in January of 2021. The assessment process included input from community members and participation from collaborative partnerships throughout Anderson Hospital’s primary and secondary service area. PRIMARY SERVICE AREA DEMOGRAPHICS Anderson Hospital is located in Maryville, Illinois in Madison County. According to the 2020 census from the US Census Bureau, Madison County holds 265,859 individuals. Anderson Hospital’s service area includes the communities of Maryville, Troy, Edwardsville, Glen Carbon, Collinsville, Granite City, Highland, and Bethalto. Together, these communities house 131,047 individuals. The following demographics are within Anderson Hospital’s primary service area. COMMUNITY HEALTH NEEDS ASSESSMENT STAKEHOLDER FOCUS GROUPS A valuable component of data collection and analysis of the Community Health Needs Assessment was preceded by the stakeholder focus groups. The purpose of a stakeholder focus group is to obtain pertinent information from our community leaders and staff members to establish key health concerns in our community. A focus group was held virtually on February 22nd, 2021. A variety of leaders from different organizations throughout Anderson Hospital’s community participated in the stakeholder focus group. The organizations that attended the focus group were: Anderson Medical Group, Obstetrics Department from Anderson Hospital, Cardiovascular Services from Anderson Hospital, Chestnut Health Systems, Main Street Community Center, Madison County Health Department, Southern Illinois University of Edwardsville, Edwardsville YMCA, Collinsville/Maryville/Troy Chamber of Commerce, and University of Illinois Extension. After background information on the assessment process was given to attendees, we then began the focus group discussion on the following criteria. General Questions (10 minutes each) What is important to the health of our community? What are the most urgent health concerns that you perceive in our community? What are the most urgent health concerns that the community perceives? What are the causes of these problems? (Physical health problems, social problems that affect health like substance abuse and violence, mental health problems, etc.) What are the strengths of the health services available in your community? What do you see as the greatest barriers to good health? In conclusion to the discussion, participants were asked to further their involvement in the assessment process, including dissemination of the Community Health Survey and participating in the Community Health Summit in the summer of 2021. Following the stakeholder focus groups, Anderson Hospital’s Health Education Planning Committee compiled data from the focus groups to mold the Community Health Survey. COMMUNITY HEALTH SURVEY As another invaluable part of the data collection and analysis component of the Community Health Needs Assessment, a survey was designed based on the feedback from the focus groups. The purpose of the survey was to obtain information from community members whom are living and/or working within Anderson Hospital’s primary service area, or utilize Anderson services, to gain an insight on what they perceive to be the major health concerns in our community. In light of the pandemic, we included a separate question regarding COVID-19, knowing its influence on the other questions. The survey questions are as follows. 14

COMMUNITY HEALTH NEEDS ASSESSMENT MEETING TO SET TOP HEALTH PRIORITIES Anderson Hospital has an interdisciplinary team, the Health Education Planning Committee, consisting of health educators from various departments of the hospital. Anderson Hospital’s Health Education Planning Committee meets on a quarterly basis to facilitate and maintain an annual dashboard focused on the top health priorities that are identified every three years in our Community Health Needs Assessment. The committee discusses objectives that have been met, as well as documents if a particular objective could not be met and why. On Tuesday, April 16th, 2021, the Health Education Planning Committee assembled to review data from the results of the community health survey and, together, discussed and voted on the top health priorities for 2022-2025. The committee commenced the meeting by reviewing and discussing the demographics from Anderson’s primary service area followed by the results of the Community Health Survey. A nominal group process was used to select the top health priorities for 2022-2025. Out of the top health concerns that the Community Health Survey revealed, each committee member selected what they perceived to be their top five health concerns throughout the community. The majority of the selected health concerns were then discussed further to focus on selecting top health priorities for 2022-2025. The top health concerns selected were reviewed and determined on account of the following: rationality, clear understanding by all members of the committee, selection based upon community input, and ability to obtain objective components. The meeting adjourned by naming the top health priorities for Anderson Hospital’s 2022-2025 Community Health Needs Assessment. The top priorities selected were Disease Prevention and Health Promotion, Healthcare Affordability, Mental Health, and Substance Misuse. Coronavirus Disease (COVID-19) was also established as a priority due to the prevalence and evolving progression. COMMUNITY HEALTH PLAN SUMMIT On Thursday, June 24th, 2021, a Community Health Plan Summit was held at Anderson Hospital to review and discuss the 2022-2025 top health priorities with key stakeholders in the community. A total of 22 participants from different organizations throughout Madison County attended the health summit to voice their opinions on certain outcomes they wish to see implemented in our communities. After receiving background information through presentations on the top health priorities, participants were asked to collaborate as a group and come up with intervention strategies that could potentially be implemented to produce change based on different health concerns. Presentations during the Community Health Plan Summit were given by the following: Disease Prevention and Health Promotion – Dr. Laura Bird and Bridget Hall, PA Mental Health – Orville Mercer, Vice President Behavioral Health, Chestnut Health Systems Healthcare Affordability – Michael Marshall, Chief Financial Officer, Anderson Hospital Substance Misuse – Nicole Gottfried, Prevention Services Coordinator, and Jeannette Obeius, Associate Director of Community Health, Chestnut Health Systems 20
COMMUNITY HEALTH PLAN OBECTIVES FOR THE IMPLEMENTATION STRATEGY On Tuesday, July 20th, 2021, Anderson Hospital’s Health Education Planning Committee assembled to design the Community Health Plan Implementation Strategy for 2022-2025 based on input from the Community Health Summit and the Community Health Survey. Available resources to address the health needs in our community were taken into account in the process of determining key objectives for each priority. Of the previous top health priorities that were identified in our 2019-2021 Community Health Needs Assessment, Mental Health and Substance Abuse/Misuse were identified again in our most recent Community Health Needs Assessment. Several subcategories of Cardiovascular Disease, Obesity, Diabetes, Heart Disease, Stroke, were all identified as top health priorities and were placed into the health priority Disease Prevention and Health Promotion. INFORMATION AND OBJECTIVES FOR TOP HEALTH PRIORITIES Disease Prevention and Health Promotion Prevention and health promotion are the keys to lowering risk for disease. There are many modifiable risk factors to chronic health conditions such as heart disease, diabetes, and cancer. These risk factors can be reduced by following a healthy meal plan, maintaining a normal body weight, and completing daily exercise to decrease sedentary lifestyle. Promoting health through age-appropriate screenings aids in early detection and awareness. Anderson Hospital offers many screening, diagnostic, and educational opportunities to support our community in disease prevention and health promotion. Objectives Promotes benefits of utilization of patient portal and explore other capabilities to provide education and hospital services Provide community education through electronic communication avenues such as social media and the Anderson Hospital website Collaborate with local organizations to promote health and wellness Create and distribute an Anderson Services brochure Continue to offer Anderson Hospital community support groups and educational events Substance Misuse Substance misuse and addiction, in the form of alcohol and other illicit drugs, is increasing and taking a great toll on our society. Substance misuse is highly prevalent within our community; therefore, Anderson Hospital proactively works with partnerships and community programs to address the issue. COMMUNITY HEALTH PLAN Mental Health Mental Health is an important factor to our everyday well-being. Having opportunities that promote well-being and reduce the stigma surrounding mental health can keep a positive atmosphere in the community. Anderson Hospital works with our community partners to improve upon the well-being of our community members. Objectives Enhance the Anderson Hospital website resource center Continue to grow and promote community support groups and program offerings Collaborate with local and national mental health organizations to develop discharge plans, community awareness and education, and staff education Educate providers on the relationship between mental and physical health with aids such as trauma informed care/consent, providing appropriate referral/resources to patients, and address teen/adolescent mental health Healthcare Affordability Patients are often challenged financially when they are forced to make medical decisions. There are a variety of reasons for financial challenges including; high deductible insurance plans, unemployment, or employers who do not provide medical insurance. Other challenges include price transparency and understanding options for bill payment and financial assistance. Anderson Hospital understands the financial constraints that our patients face and have worked hard to assist in appropriate and necessary health care for all members of our community. Objectives Provide more timely estimates for patients regarding their financial liabilities Evaluate price estimator software for AMG providers Evaluate expanded transparency for patients to have options for medical bill payments Continue to utilize the Anderson Hospital website and social media to inform the community of financial assistance options and payment programs Coronavirus Disease (COVID-19) With the outbreak of the novel coronavirus disease (COVID-19), many lives were impacted with acute illness, long term side effects, and even fatality. As a healthcare organization, we have felt the effects of this progressive disease and the strain on our community. Anderson Hospital has been challenged with bed availability, staffing constraints, and supply shortages, in addition to providing opportunities for vaccination and testing for our community. Objective Provide continued guidance on prevention, testing, vaccinations, and recommendations as they evolve 22
NEXT STEPS FOR 2022-2025 NEXT STEPS FOR 2022-2025 After completing the Community Health Needs Assessment and creating the Community Health Plan Implementation Strategy, our next steps will include: - Develop and adopt approaches to meet objectives - Update and maintain the Implementation Strategy throughout the three-year process - Continue quarterly meetings of the Health Education Planning Committee to ensure the Implementation Strategy is being attained In regards to all four top health priorities, Anderson Hospital will work towards: - Recruiting employees to be advocates on top health priorities - Partnering with local EMS to assist in education and community opportunities - Partnering with local school districts to educate youth on prevention - Encouraging the top 4 priorities to be discussed and presented at health fairs and events within the community BOARD OF TRUSTEES APPROVAL The 2022-2025 Community Health Needs Assessment and Community Health Plan was approved by the Hospital Board of Trustees and Administrative Council in November 2021.
