This will serve as routing/logistics instructions when shipping your product to Costco Wholesale Japan, Ltd. (“CWJ”). These terms and conditions cannot be altered, changed, or deviated from without written consent from CWJ. Deviations without prior approval from the Buyer, Logistics Department, or other authorized person will result in the issuance of charge back, which will be deducted from your invoice. 1
Standard Logistics Instructions Export to Costco Wholesale Japan, Ltd (CWJ) Last Update: Jan 9, 2014 Routing will appear on CWJ purchase orders as follows: ♦ CARRIER specifying a freight forwarding agent you are required to contact. ♦ FOB specifying a loading port/shipping point you agreed with the Buyer. ♦ CANCEL DATE specifying the request ship date. The shipping window must be a week earlier or later. ♦ FRT.TYPE specifying if freight is Collect(COL) or Prepaid(PPD). If FRT. TYPE is Collect – FOB or Ex-works, see the following A. If FRT. TYPE is Prepaid – C&F or CIF, see the following B. A. Collect – Inbound to CWJ (Costco selects carrier and pays freight) CWJ is responsible for arranging transportation and all other logistics in shipping the product from a loading port/shipping point to Japan. Vendor is required to contact the carrier specified in the purchase orders. The address and contact information of the forwarders is detailed in the following Freight Forwarder Contact Information. Shipping by an unauthorized forwarder/carrier without prior approval from the Buyer, Logistics Department, or other authorized person will result in the issuance of charge back of the freight difference. The following forwarder is specified as a carrier: Expeditors International of Washington – Shipments from United States Kuehne + Nagel – Shipments from the rest of the world B. Prepaid – Inbound to CWJ (Vendor selects carrier and pays freight) The vendor is responsible for arranging transportation and all other logistics in shipping the product from shipping origin to Japan. 2

IMPORT PO SHIPPING WINDOW New Procedures effective on January 1, 2013 Effective on January 1, 2013, all shipping windows for imported containers shipped to CWJ will change to the following. The Merchandise is loaded on board vessel at designated port and departs designated port as indicated on the Bill of Lading is within the ship window. The ETD can fall on the first date of the ship window, the last date of the ship window or in-between. The buyer’s authorization is required in the following exceptional events. The alert will be sent to the buyer by either K+N or Expeditors system for authorization (accept/reject) and returned to you by email. Paperwork Procedures - Important Invoice and Packing list must be prepared by vendor on all shipments, which includes the following information: Costco’s purchase order number(s); POs will be 10 digit numbers. Description of product Costco item number(s), as well as description(s) of product must appear together. Unit cost, extended cost and total cost with currency + Payment Term and Incoterms Actual scale weights and measures and total package quantity Express Bill (Seaway Bill) should be issued by forwarder/carrier unless the Ocean Bill of Lading is required for Letter of Credit negotiation. In any case, Costco’s purchase number should be indicated on the Bill. Ocean Bill of Lading should be issued per containers. “Costco Container 8-Point Inspection and ISO Seal Verification Checklist” should be completed prior to loading and submitted to K+N (Non-US only: See the following “Loading” section). IMPORTANT Once the product is shipped from the shipping point or the ocean vessel has sailed from the loading port, all vendors are required to e-mail all shipping documents to CWJ Import/Logistics Department (e-mail address : jpimportshippingdocs@costco.co.jp) and the Buyer within 24 hours. Please put the Costco’s PO# in the email title. Additional incurred charges caused by shipping document delays will be a charge back to the vendor. (E.g. demurrage/storage fees) If the shipment is arranged by Expeditors or Kuehne+Nagel, please copy all shipping document to them as well as to CWJ. If the purchase order was placed by Costco Wholesale Corporation, National/International Department regardless of whether the shipment is made directly from vendor/factory to CWJ, please send all shipping documents to the Buyer in Costco National/International immediately following on board date. If Phytosanitary/Health Certificate is required, the originals must immediately be forwarded by Express mail. The Certificate must arrive at CWJ prior to the shipment arrival in all circumstances. The extra cost incurred due to delay will be a charge back to the 8

All CWJ orders are considered “Shipper’s Load and Count” The vendor is responsible to ensure the order is properly packed and loaded to prevent any transit damage and tampering. If blocking and bracing is required – this is to be done by the vendor or at the vendor’s expense. It is also the responsibility of the vendor to ensure an accurate piece count of the load and the piece count is to be clearly stated on the packing list and Bill of Lading. Back Orders / Split Shipments ♦ Any back order or split shipments must be authorized by the Buyer prior to shipping. ♦ Any back order or split shipments resulting in additional freight charges will be charged to the vendor. Over Shipments, Unauthorized Substitution, or Defective Goods ♦ Shipments will be returned at the vendor’s expense. ♦ The vendor will be charged inbound and return freight charges and be subject to a handling fee. Loading ♦ All load capacities should be based on maximum utilization of container space. (Excluding LCL ship) ♦ A vendor not utilizing maximum space available may be subject to any additional charges incurred. ♦ Vendor is responsible to inspect all Merchandise prior to shipment to ensure quality, safety and conformity and to ensure that the Merchandise is properly packed and loaded to prevent transit damage and tampering. Any refusals or issues resulting from failure to do the above will result in a charge back to the vendor for all associated costs to deliver or return the Merchandise. ♦ Every container must be DRY, CLEAN and ODOR free. Palletized overstock is stored in overhead steel racking, also on our sales floor. Therefore, our pallets standards are quite high, structurally and aesthetically. We encourage our suppliers to use the lightest weight, most durable pallets in the marketplace. ♦ STRINGER/GMA #1 PALLETS are NO LONGER ACCEPTED as general-use pallets. ♦ Costco requires BLOCK PALLETS throughout North America. ♦ From North America, ship exports to Japan on pallets compliant with our North American Standards. - CHEP, PECO, and iGPS pallets cannot be exported to Japan. Standard Logistics Instructions Export to Costco Wholesale Japan, Ltd (CWJ) Last Update: Jan 9, 2014 Vendors shipping on Non Costco Specification or bad pallets are subject to charges for labor and materials to re-palletize or rework. Wood Packaging Materials (WPM) imported into Japan must be heat treated (to a minimum wood core temperature of 56°C for a minimum of 30 minutes) or fumigated with Methyl Bromide and marked with the International Plant Protection Convention (IPPC) logo and WPM mark, with the appropriate ISO country code designating the country location of treatment. Wood Packaging Materials (WPM) refers to wooden pallets, crates, boxes, boards used for inner packaging, and dunnage. For further information, please refer to the WPM Guidelines published by IPPC on their website: www.ippc.int Note that in general, Costco Japan does NOT permit the use of wooden pallets and other WPM unless the pallets meet the Costco wood pallet requirement. You must first send photo of pallets and then obtain the buyer’s approval in advance of P.O. issuance and in advance of shipment. In addition, for safety reasons, if the use of wood pallets is pre-approved by Costco, please make sure that the pallet used is made of GMA hardwood and of No. 1 standard. (Please also see the following “Wood Pallet Requirements” for more details.) For all pallets and crates – if pre-approved by Costco -- please make sure there are no safety-related hazards, such as nails or splinters, etc. WPM made entirely of manufactured or processed wood material, e.g. MDF, particle board, plywood is exempt from the treatment and marking requirements. Anchor shrink wrap to bottom of pallet. A minimum gauge of .79 is preferred for shrink wrap of pallet. ♦ No overhang or underhang of product on pallet. ♦ Tie/High that maximize pallet capacity. ♦ Optimizes cube. As close to but not more than 52” high, including pallet height ♦ Allows double stacking Support Posts. Top Cap. ♦ Full/Flat layer No Chimney tops. Distribution quantities adjusted to flat layers. Any exception to these standards must be pre-authorized by the buyer and/or Logistics BEFORE shipping. Data Logger Requirement for Perishable Shipments CWJ requires you to put temperature data logger monitor in our perishable shipment. It will enable us to make immediate accept or reject decisions, and have better visibility into every step of our cold chain. We are no longer requesting to use chart recorders with a rolled paper since we believe 13
