Welcome to East Sussex Healthcare NHS Trust On behalf of East Sussex Healthcare, we extend a very warm welcome to you all. We will do all that we can to make your stay enjoyable, educational and productive during your time with us at East Sussex Healthcare NHS Trust. This handbook has been prepared to provide you with some important information as well as general information about the Trust. The Trust provides services from two acute hospitals, Eastbourne District General Hospital and Conquest Hospital in Hastings. Further outpatient services are provided at Bexhill Hospital. There is Day Surgery and intermediate rehabilitation at Bexhill Hospital. Outpatient and inpatient intermediate care is provided at Rye and Winchelsea Memorial Hospital. We offer care to patients in their own homes, health centres, GP surgeries and schools. Year 3 Students’ Role As Year 3 students you are at a much earlier stage of learning, but you will be working towards the goals of Year 5 Students’ role as outlined below. You should work with your capabilities and under appropriate levels of supervision. Please refer to the relevant module handbook and logbooks for specific learning and skills appropriate to your level of study. Year 5 Students’ Role You will have a responsibility for a number of patients in your care and may be required to: Clerk your patients and write admission notes Arrange necessary initial and continuing investigations Explain and justify the plans for the patient to their satisfaction Visit your patients at least daily to check on their progress, examining and performing additional investigations as necessary and write up continuation notes in patients records Write up all medications (to be signed only by medically qualified staff) Liaise with nursing and other paramedical staff regarding the patient’s care Take part in the planning of their discharge (e.g. through the multidisciplinary meeting) Write discharge medication for supervisor’s agreement and signature. Write the discharge letter to the General Practitioner To be present when informed consent is being obtained or when any important communications take place involving their patients. Support and observe patients during any procedures or other significant activities You must not:- initiate, alter or stop treatment of a patient on your own diagnosis, any diagnosis you make must be confirmed by the doctor supervising you and any treatment you give must be confirmed by them in advance. Prescribe or request diagnostic investigations – if you complete an order form for any of these purposes, it must be signed by your supervisor before it is executed Take part in obtaining or witnessing a patients signature for consent to treatment Take history from, examine or treat a patient without his/her prior informed consent Hold your FY1 or FY2’s bleep under any circumstances Perform EPPs 7|P age
Student with Concerns An important part of your development as a junior doctor is to have the ability to solve management problems and learn how to represent your point of view in a sensitive and sensible manner. Whilst we would encourage you to try and resolve your own issues yourself, you are always welcome to seek advice and support via the Undergraduate administration team. Pastoral Care Our Nurse Education Fellows (Paul Gosling at Eastbourne DGH and Jolie Wilkinson at Hastings Conquest Hospital) are the first point of call for Pastoral Care for Medical Students. Unfortunately there may be occasions where an issue may have to be reported back to your medical school. Subsidised restaurant in main hospital. Internet Access NHS Wifi is available at Eastbourne. in Accommodation Students staying in Hastings, must use SUSSEX Wifi. Wi-fi is provided All Students will be sent SUSSEX login prior to arrival , with instructions on how to activate your accounts on a Trust computer. At Hastings a Trust ID swipe will be provide with your accommodation keys so that you can access the library computers outside office hours to active your SUSSEX logins. In the event of WiFi not working in the accommodation, a MiFi device, with unlimited data can be borrowed from Medical Education Offices. A £20 deposit is required which is refundable on return. Rules and Useful Information Alcohol No alcohol allowed in the hospital. Students must take responsibility for their own alcohol consumption in the accommodation, but excessive drinking is strictly prohibited and any excessive alcohol consumption will be reported to your school. Consumption of alcohol during breaks or immediately before work is discouraged. Staff working under the influence of alcohol or drinking at work are liable to disciplinary proceedings. Accident at work Report any accident to your supervisor or nurse. You will be required to complete an accident report form, available from Ward Clerks at the nursing stations. Anti-Bullying Policy The Trust’s Policy clearly states that no workplace bullying will be “DON’T condoned by the Trust SUFFER IN Examples of Bullying are: SILENCE” Derogatory remarks or public criticism, insensitive jokes or pranks. Withholding information. Teaching by humiliation Unrealistic demands Complaints can be made to your Line Manager / UG Admin and will be viewed seriously and treated confidentially. Care will be taken to treat all employees involved with consideration and support and Counselling will be provided if necessary. Attendance You should be here until the last Friday of your rotation unless otherwise stated by your Medical School. If you do leave early without such permission the Undergraduate Administrators are required to inform the Medical School accordingly Arrival Year 3 You can collect your keys anytime over the weekend before your Students placement commences. You will undertake your Monday tuition either online, or in person in Brighton. This will enable you to be ready to start your placement on Tuesday morning. Arrival Year 5 You can collect your keys and move into your accommodation anytime Students after 4pm on the Friday immediately before your placement. This includes anytime over the weekend. Behaviour You are expected to behave responsibly whilst on and off duty . You will be disciplined and will be liable to dismissal if you are found to have taken any illegal drugs. Cardiac arrest Please familiarise yourself with the location of the emergency resuscitation trolley in your area. Confidentiality Confidential information about patients must not be divulged to any person except with the express authority of the Trust. Dress Code All staff having contact with patients must comply with the Secretary of States ‘clear below the elbows policy’ and should ensure:- That they roll up long sleeves Remove wrist watches and jewellery (with the exception of wedding rings), prior to using alcohol gel or washing their hands Ties should be tucked in Adherence to this directive will, along with hand washing, be audited and appropriate action will be taken against individuals who do not comply with the policy. Procedures How to Deal with Personal Attack or Threat You must dial internally 2222 Always leave the scene where possible, if not; Call for help from someone in the vicinity. Sound personal attack alarm if available. Encourage the person to stop whatever they are doing. Encourage them to put down the weapon or instrument they have and try to prevent them from obtaining any others. Continue to talk to the person until assistance is available. Fire A continuous alarm will be heard in the area affected and an intermittent alarm sound will be heard only in the zones adjacent to the risk area. If you discover a fire, raise the alarm by breaking the insert of a RED fire alarm call point and dial switchboard on the internal emergency number (2222) to confirm the location. In the event of an actual fire do not attempt to tackle it but close doors and if possible close all windows as you leave. Make sure your escape route is clear at all times. In areas such as OPD or A&E it is important to know the exit routes from the building and the location of the external assembly area. 12 | P a g e
FY1 tutorials Conquest - every Wednesday in the Education Centre from 1300 - 1400. Students are welcome to attend if room capacity allows. Eastbourne – every Tuesday in Sara Hampson room, from 1300 to 1400 and Year 5 students are welcome to attend (Year 3 may attend but only if appropriate and if space allows) but on one Tuesday in every month the FY1s have a foundation workshop which for to FY1s only. Grand Round Students are welcome to attend the Grand Rounds currently via MSTeams. These take place as follows: Conquest : Tuesdays 1:15 – 1:45pm Eastbourne – Mondays 1-2pm. Hand washing Hand washing with soap and water is essential when your hands are dirty. Alcohol gel, available at every bedside, must be used before entering wards and after contact with every patient. Infection control Information can be found in the Infection Control Manual available on all wards and departments. Detail what happened, when, why, to whom and by whom and accessed only by authorised personnel at all times. Needles and Sharps Never leave sharps lying around. Do not keep syringes with needles attached in pocket. Sharps should not be passed from hand to hand. Request assistance when taking blood from or giving injections or infusion therapy to uncooperative patients. It is the responsibility of the person using a sharp to dispose of it safely. Sharps must be placed in a sharps container as soon as possible after use. Do not carry exposed sharps through the ward. If possible use a 13 | P a g e
mobile sharps container and tray and takes these to the patient. When using needles and syringes they must be discarded as a single unit, where possible. Do not re-sheath needles. Any device which requires the operator to move his or her hand toward the needle point – or the needle point towards the hand is not safe. Sharps containers must be closed using the temporary closure, where applicable, when not in use. Sharps containers must be closed securely when three quarters full, then source coded and placed in a disposal room. A new container must then be made available. Date of assembly and closure must be clearly written on the sharps bin. Non-clinical All incidents/accidents to both staff and patients must be reported on the incidents relevant forms obtainable from the Ward Clerks at the nursing stations. Occupational Health You will be asked to sign a declaration stating that you are up to date Services with DoH recommended vaccinations, serology and health screening, deeming you fit to work on a clinical placement within the NHS. If you are not up to date please can you arrange to have these done before you start your placement with us. Please also note that if you have any underlying medical problems it is in your interest to declare them to the Occupational Health Department. Any skin lesions, episodes of diarrhoea and vomiting should also be reported as this could pose an infection risk to yourself or those you are treating. You may also find it helpful to consult the Occupational Health Department which offers advice and support on all aspects of health and safety, and works to reduce risks at work which could lead to ill-health, staff absence and accidents. The department provides confidential help, advice and access to counselling where appropriate. Please ring to make an appointment. Staff are available Monday to Friday between 9.00 a.m. and 4.30 p.m. There are open clinics for immunisations at EDGH on a Thursday morning between 0930 and 1230 hours and via appointment for Hastings (ext 8357). Patient Advice & PALS provide information on NHS services and gives advice and Liaison Service support to patients or their relatives/carers who may have concerns. The (PALS) service liaises with hospital staff and appropriate, relevant organizations to negotiate solutions to problems to avoid the need for patients to make a formal complaint. Pathology The ‘users’ guide can be found on the Trust Intranet under Trust Policies and Guidelines Database. Pharmacy The BNF can be found on the Trust Intranet under Departments (Pharmacy – Electronic BNF). Resuscitation There are cardiac arrest carts on each ward/department each with defibrillators. There are three teams dealing with emergencies/ cardiac arrests:- Paediatric/neo-natal arrest teams .Adult arrest team Maternal arrest team The Resuscitation Department offers resuscitation training from basic to advanced in both adult & paediatrics. The training sessions are accessible to all medical students whilst they are on-site. Please contact the Resuscitation Dept direct (contact numbers can be found on the intranet. In addition to the training sessions, there is a spare bleep which medical students are invited to carry. This bleep is programmed for all of the resuscitation teams - both cardiac and trauma for adults & paediatrics. This will enable medical students to attend and participate in emergency calls wherever possible. If you wish to undertake any of the training sessions or to carry the bleep please contact Steve Rochester. Risk Management Remember – if in doubt, always ask a member of staff for advice. Security – in the event of verbal or physical aggression please – Do – call for assistance, continue to talk calmly to the person and try to remain calm yourself until help comes Don’t - attempt to manage the situation on your own, don’t stop talking to the person especially if they go quiet. Don’t panic. Rotation 3 (Year 5 use your last placement to: only) talk to your educational supervisors grab every opportunity to ‘act up’ as FY1 escort patients to ‘procedures’ list what you have seen / haven’t seen and set realistic goals help each other, use role play, practice, practice, practice Scrubs No scrub suits are to be taken or worn off site (including accommodation). Sharps injury Any skin puncture should be encouraged to bleed and washed liberally with soap water. Splashes into eyes or mucous membranes should be irrigated with running water. Conquest Accommodation Accommodation will be available from 4pm on Friday prior to the start of your attachment. When collecting your key from Switchboard (main hospital, Level 3, in A&E) please do not bombard them with questions, as their role is to simply hand over the key, and direct you to your accommodation. For those of you with cars, on arrival at the Conquest Hospital you will either need to park on Little Ridge Avenue (NOT in the residential area) or in the visitors pay and park within the hospital grounds. Anyone who requires a key fob can obtain one from the Accommodation Manager for a small returnable deposit of £10, which will give you access to the car park outside your accommodation, however spaces are very limited, so please share cars if possible. Duvets and duvet covers are now provided in your accommodation as well as Wi-Fi. 18 | P a g e
Eastbourne DGH Accommodation Accommodation will be available from 4pm on Friday prior to the start of your attachment. Keys can be collected from the Accommodation Office (Fleming House) Monday to Friday 8am – 4pm. You must vacate your room by 10am on your last day, to allow a turnaround for the next intake of students. Outside this time, keys can be collected from the Switchboard. When collecting your key from Switchboard, parking permits may also be collected. Please see location map of switchboard below. Please approach the entrance that is depicted in the map, when approaching these doors late at night or overnight, there is an Intercom on the external wall that is answered in the Switchboard. Once you have made contact they will buzz you in and you will then be able to go and collect your keys from them. Laundry Cards are available from Fleming House at a cost of £3.50 (Non refundable).
