To Applicant:

This document is a functional job description which describes, in detail, the physical, environmental, and
mental demands required to perform the essential duties of a truck driver. Please read this document very
carefully. You will be asked to sign this document on the last page and indicate whether, to the best of
your knowledge, that you either do, or do not, have the physical or mental capacities to perform this job
safely.
If you indicate by your signature that you believe that you have the necessary physical and mental
capacities to perform the essential functions of this job safely as described in this document, an offer of
conditional hire may be issued to you on condition that you can safely undergo medically-based job-
specific functional testing (e.g., lift and carry tests that simulate job demands while your heart rate and
blood pressure are monitored) to demonstrate that you are physically fit for duty. Medical questions and
functional testing are conducted on a post-conditional offer basis only.
A medical and functional work capacity evaluation will include completing a detailed medical history
form. For your safety and the safety of others, you will be required at that time to fully disclose any
medical problems that you may have. You will then undergo a physical (medical) examination and job-
specific functional capacities examination to ensure that you can be safely placed in the job. If you fail to
demonstrate that you can safely perform the essential duties of this job, the offer of employment will be
withdrawn.
If you do not fully understand these instructions or do not understand certain information in this
document, please notify the person who issued this document to you. A company representative will
answer your questions and clarify any issues as required.

 I. General Job Description
Employee is responsible for the safe, efficient, reliable, and environmentally compliant operation of
assigned area. Employee’s key job responsibilities include, but are not limited to, the following:
    1. Performing daily maintenance inspections of the vehicle and material handling equipment
       (forklift, pallet jack, boom).
    2. Overseeing proper loading and unloading of cargo. Driver may be required to assist the
       loading/unloading of material using a forklift, booms and/or by hand.
    3. Driving to delivery location in a timely, safe and professional manner.
    4. May be required to drive vehicle over varied terrain and roadways verifying route using maps,
       directions, customer contacts or computer maps prior to leaving facility.
    5. Securing and tarping material on the truck using supplied strapping, tie downs, tarps, etc.
    6. Verifying that the proper product delivered matches delivery ticket, purchase order, warehouse
       transfer or return merchandise form with loaded material.
    7. Operating forklifts and booms when delivering materials.
    8. Communicating with dispatcher as needed.
    9. Completing paperwork as required.

 II. Minimum Physical Demand Requirements for Safe Job Performance
Physical demands described in this functional job description have been validated by on-site job analysis
involving direct measurements. They are representative of the physical demands that must be met by an
employee to successfully and safely perform the essential functions of this job. Although specific
components of job demands are listed in detail in this document, the following information is a synopsis
of key minimum physical demand requirements determined from a detailed job analysis:

01. POSTURAL: Employee must have the ability to move extremities and spine throughout a sufficient
    range of motion that will permit the employee to work in a variety of postures involving a range from
    floor to overhead level. Flexing and twisting of the trunk and other awkward postures are
    occasionally required while climbing onto/off of truck, boom, forklifts and when loading/delivering
    product by hand.

02. DEXTERITY: Employee must have good dexterity and frequently use one and two hands
    throughout the work period when operating the trucks, forklifts, boom and when handling product.

03. GRIP STRENGTH: Employee must have sufficient grip to climb up and down on the truck, forklift
    and boom. Must have the grip strength to operate same equipment and to handle materials by hand.

04. CARDIORESPIRATORY DEMAND: Employee must have sufficient cardiorespiratory fitness to
    be able to do heavy work for short periods of time, handling heavy product in heat and humid
    conditions, inside and out.

04. MATERIAL HANDLING - Employee must be able to demonstrate that he/she can safely:

             lift at least 110 lbs from floor to knuckle on an occasional basis,
             carry at least 110 lbs with two hands for a minimum distance of 15 feet on level surface, and
             pull horizontally with a peak force of 75 lbs with two hands
             push horizontally with a peak force of 75 lbs with two hands

Frequency of material handling will vary considerably depending on operational conditions. The physical
demands for certain jobs may actually exceed those listed above in certain situations. However, the
employee is expected to seek assistance if material handling requirements exceed those listed above or if
the job demand cannot be performed safely.

 VIII. Physical Demand Level Classification:
Employees identified in this document must be able to perform Very Heavy physical labor as classified by the
United States Departments of Labor. Physical demands typically involve lifting, carrying, pushing, and pulling.

 IX. Cardiorespiratory Demands:
Employee must have sufficient cardiorespiratory fitness to perform the essential tasks of this job safely. Employee
must be able to safely tolerate heavy to very heavy demands on the cardiorespiratory system related to: handling of
materials with exposure to various weather conditions.

 X. Energy Expenditure
Employee must have an aerobic capacity sufficient for energy expenditure of over 7.5 METS which is consistent
with a PDL classification of heavy to very heavy as classified by the United States Department of Labor.

 XIII. Guidelines for Physical Demand Validation (PDV) Analysis:
Physical demand validation analysis was performed based on guidelines established by the U.S. Department of
Labor (USDL) and the Occupational Safety and Health Administration (OSHA). Validation of physical demands of
job tasks is required for developing an accurate and valid functional job description as required by guidelines
consistent with the Americans With Disabilities Act (ADA) and EEOC regulations for fair hiring practices. Findings
presented in this PDV are presented as representing typical physical demand requirements of the job tasks analyzed.
Physical demands may at times exceed those identified in this report. Employees are required to adhere to all safety
guidelines established by the employer and seek assistance to perform any task, which they may feel exceeds their
safe capacities to perform.

 XIV. Methods of PDV Analysis
1.   Personal interview, questionnaires, and on-site direct observation of job activities.
2.   Measurements of material handling forces using push-pull dynamometers and weight scales.
3.   Measurements of material dimensions, distances, heights, reaches using tape measures.
4.   Print and slide photography and real time videotaped recording.

KEY:
