FACULTY OF COMMUNITY AND HEALTH SCIENCES DOCTOR’S DEGREES 2020 DOCTOR OF PHILOSOPHY THEODOSIA ADOM SCHOOL OF PUBLIC HEALTH Supervisor: PROF T PUOANE Co-supervisor: PROF AP KENGE Thesis: Individual and environmental factors associated with obesity among young children in primary schools in Ghana Description: Overweight and obesity are risk factors for non-communicable diseases such as cardio- vascular diseases and some cancers. Understanding factors associated with overweight/ obesity among children present an opportunity for timely and appropriate interventions. Utilising the socioecological framework, the PhD assessed the prevalence and determinants of childhood overweight/obesity and explored population level policies that encourage supportive environments for healthy living. The prevalence of overweight/obesity was 16.4%, indicating that overweight/obesity is a public health issue in primary school learners. There were gaps in the literature in terms of population-based interventions. A number of modifiable risk factors were identified and recommendations were made to for relevant stakeholders. WOLDEKIDAN KIFLE AMDE SCHOOL OF PUBLIC HEALTH Supervisor: PROF U LEHMANN-GRUBE Co-supervisor: PROF D SANDERS Thesis: Unpacking capacity development: A systemic exploration of a partnership of African universities to develop capacity on health workforce development Description: It explores the partnership between UWC and 3 African Universities and Health Ministries (in Rwanda, Mozambique and Ethiopia) to develop capacity in health workforce development, geared to strengthening national leadership and training capacity in health workforce development. All three international examiners praised the thesis as an important, politically and culturally sensitive, contribution to our understanding of the importance and complexity of collaboration between academic institutions and between academic institutions and ministries of health, and to strengthening collaborative practice in future. FACULTY OF COMMUNITY AND HEALTH SCIENCES 2020 DOCTOR’S DEGREES DOCTOR OF PHILOSOPHY MARIO ANDRE CLAYFORD SCHOOL OF PUBLIC HEALTH Supervisor: PROF T PUOANE Co-supervisor: PROF P NAIDOO Thesis: Investigating predictors of health related quality of life and functional status in middle-aged to older adults with hypertension living in a selected urban community in South Africa Description: Hypertension is one of the leading risk factors for heart disease, strokes, and preventable cause of premature deaths in South Africa. Quality of life (QoL) is inherent to everyone, and a decrease in QoL increases susceptibility to other ailments. This PhD project investigated which socio-demographic, psychosocial and disease characteristics are the best predictors of QoL and functionality in individuals with hypertensive. The outcome of the study revealed higher QoL and functionality in married, less stressed using proactive coping, the higher educated, and disability free individuals. The study highlights the adverse impact of disability and presence of comorbid diseases on QoL and functional status. The value of the study lies in the fact that assessing how an individual perceives his or her health is necessary to develop appropriate treatment strategies which should lead to positive health outcomes. FACULTY OF COMMUNITY AND HEALTH SCIENCES DOCTOR’S DEGREES 2020 DOCTOR OF PHILOSOPHY GERALD MWILIMA KELA Department: SPORT RECREATION & EXERCISE SCIENCE Supervisor: PROF A TRAVILL Thesis: Sustainable health-promoting physical education in schools in the Zambezi region of Namibia: Developing guidelines for curriculum development Description: The study was conducted against a background in which Physical Education is rapidly disappearing from school curricula in Namibia. The demise of the subject is prevalent across Africa despite the proven health benefits associated with active lifestyles. The outcomes of the research highlights the tension existing between the aims and content of the Physical Education curricula which is brought about by a lack of input from stakeholders, the marginalised status of the subject as part of the school curriculum and the politics which influence Physical Education curriculum renewal mainly caused by governmental over control on what needs to be changed. The research concluded with a set of curriculum guidelines that should be adhered to if we wish to implement Physical Education curricula successfully. MMAPHUTI MARIA MAMALEKA Department: SOCIAL WORK Supervisor: PROF C SCHENCK Thesis: Developing guidelines for indigenous parenting practices: A case study of Sekhukhune District, Limpopo Province, South Africa Description: All cultural groups have their own way of parenting their children. South African families tend to follow western parenting practices, not always appreciating their own traditions and customs. The purpose of the study was to explore indigenous parenting practices to develop indigenous parenting guidelines together with parents. This qualitative, case study design sampled 52 participants from six villages in the Sekhukhune district in Limpopo, consisting of parents, grandparents, and traditional leaders. In-depth interviews, focus groups, and workshops were used, within the Afrocentricity framework to guide the study. Ms Mamaleka’s study produced rich, thick findings focused on a unique topic that brought traditional parenting practices to present-day social work policy, practice and education. FACULTY OF COMMUNITY AND HEALTH SCIENCES 2020 DOCTOR’S DEGREES DOCTOR OF PHILOSOPHY JULIA MANTSALI MODISE Department: SOCIAL WORK Supervisor: PROF C SCHENCK Thesis: Guidelines for sustainable women co-operatives Description: In 2005 the government of South Africa introduced legislation to promote the development of co-operatives as a vehicle for job creation and poverty reduction (Co-operative Act 14 of 2005). Despite the Billions of Rands that were invested in the promotion and development of co-operatives, close to 90% failed. In this multiple case study research the candidate studied in depth 15 cooperatives in four provinces in South Africa. The aim of the research was to learn from successfully functioning co-operatives, how the co-operative was formed, managed and sustained. Using Mazzarol, Simmons and Limnios’s Co-operative Enterprise business model as theoretical Framework to analyse the co-operatives, key elements emanating from the results were used to formulate the guidelines, in particular for women owned cooperatives. SULAKSHANA NANDI SCHOOL OF PUBLIC HEALTH Supervisor: PROF H SCHNEIDER Thesis: Equity, access and utilisation in the state funded universal insurance scheme (RSBY/MSBY) in Chhattisgarh State, India: What are the implications for Universal Health Coverage? Description: Sulakshana Nandi’s PhD examined the equity and access impacts of a major insurance scheme introduced in India in 2009, which enabled public funds to be used extensively for private hospital care for the first time. Through a series of empirical studies in Chhattisgarh State, where she has lived and worked as a health advocate for many years, Ms Nandi demonstrated the failure of the publicly funded health insurance scheme to achieve its goals of improved affordability, access and equity of health care. Her PhD raises important questions on the value on such financing models for advancing universal health coverage, particularly in the context of a highly commercialized private health sector, providing important lessons for other countries (such as South Africa) embarking on similar reforms. 6 UNIVERSITY OF THE WESTERN CAPE
FACULTY OF COMMUNITY AND HEALTH SCIENCES DOCTOR’S DEGREES 2020 DOCTOR OF PHILOSOPHY JENNY LEE ROSE Department: PSYCHOLOGY Supervisor: PROF N ROMAN Co-supervisor: PROF K MWABA Thesis: The effect of the Circle of Security Parenting Program on parental self-efficacy and internalizing behaviours in Early Childhood Development Description: In Early Childhood Development (ECD), the attachment relationship is pivotal in determining developmental outcomes of children. Parents act as a buffer against adversity for the child, but often lack the skills for this. The Circle of Security Parenting Program (COS-P) aims to equip the parent with skills and guidelines for more effective parenting. Using mixed methods, Jenny tested the feasibility and effect of the program on parental self-efficacy and child internalizing behaviours. The findings show a significant reduction in internalizing behaviours, as reported by parents and teachers; and significant improvement in parental self-efficacy after the implementation of the COS-P. These effects were also sustained at the follow-up time frame. This study makes a significant contribution not only to parenting and child mental health in ECD but also to the applicability of an international programme in South Africa. FACULTY OF DENTISTRY 2020 DOCTOR’S DEGREES DOCTOR OF PHILOSOPHY LESLEY ROSS VORSTER Department: COMMUNITY DENTISTRY Supervisor: PROF S NAIDOO Thesis: Economics of the South African toothpaste industry Description: In South Africa, an estimated 90% of caries remains untreated, disproportionately affecting the poor and detrimentally. Use of an effective fluoridated toothpaste is the best clinical practice available today in caries management. The availability, pricing and affordability of fluoridated toothpastes marketed to South African households was investigated and mechanisms to improve financial access among low-income households was identified. Following a survey of commercially available fluoridated toothpastes, affordability relative to household income brackets was assessed using a modification of the WHO/HAI’s essential medicines pricing protocol. Inequity in affordability was found between low income and affluent households with implications in terms of oral health and economic hardship suffered by the poor. To redress this social injustice, a number of price regulatory policies and funding mechanisms were proposed to stimulate dialogue and urge all relevant stakeholders to take action. 8 UNIVERSITY OF THE WESTERN CAPE
